  Eric worked with my father several years ago, even though my father was unable to drive at the time. All of the paperwork Eric recommended has been needed, including Health Care Surrogate and POA. My family and I are so grateful that Eric was able to work with my father and help him through this process. Several years later, we are using all of these forms to ensure my father’s wishes are met and that he gets the care in the manner he wanted and needs.
