Community Living Options

CLO has a strong reputation and extensive experience working with individuals with high needs due to behavioral, cognitive and medical challenges. CLO is licensed as a Rule 245 DD, CADI, & BI waivered service provider through the State of Minnesota, Department of Human Services. We are licensed as an Adult Foster Care/Community Residential Setting provider through Washington, Chisago, Anoka and Pine Counties, also providing Child Foster Care in Washington and Anoka counties.. CLO homes are single family style homes, typically located in rural country settings. The homes are built to meet fire and life safety codes and are designed so that our clients have their own private bedrooms. We will often pair 2-3 homes in proximity to each other so that staff resources may be shared for activities and emergencies. We provide 24 hour supervision due to the vulnerabilities and supervision needs of our clients. Our typical staffing pattern for a home is 2-3 staff during all waking hours, with awake overnight staff and on site management. The primary function of the third staff during waking hours is to enhance recreational and community integration activities. CLO strongly believes that each person is a unique individual and can be supported to safely live and integrate into their community. CLO supports each individual to be an active member of their community, based on their individual interests. Individualized community integration day support is available. CLO is also actively involved in and sponsor our own Special Olympics chapter. Clients participate in basketball, softball, football and bowling, including state and regional tournaments. There is participation at all skill levels, from full team play to individual skills. CLO staff provide positive, supportive coaching.

Community Living Options provides In-Home Waivered support services and Independent Living Services (ILS). For In Home waiver and ILS, CLO provides trained staff to offer support so that individuals develop more independent living skills and have access to community activities.
