Spitex Wyland AG is the joint public home care organization of the municipalities of Andelfingen, Kleinandelfingen, Ossingen, Stammheim and Thalheim an der Thur. Help and Care at Home We are committed to enabling our clients to lead as independent and self-determined a life as possible in their familiar home environment, despite health limitations. This includes: - Assessment (with RAI-Home Care) and Advice: Comprehensive assessment of individual care and support needs, including consideration of the social environment - Basic Care: For example, assistance with activities of daily living, personal hygiene, getting up and dressing, as well as support with meals - Medical Care: For example, wound care and dressing changes, administration of medication (including injections and infusions), blood sampling, and monitoring of pulse, blood pressure and blood sugar levels - Palliative Care and End-of-Life Care: Palliative care includes the support and treatment of people with incurable, life-threatening and/or chronically progressive illnesses. Patients are ensured the best possible quality of life adapted to their situation until the end of life, while close relatives receive appropriate support. - Psychiatric Care: Support for people with mental illness in crisis situations and in managing everyday life (in cooperation with specialized institutions) - Acute and Transitional Care: Provided directly following a hospital stay for a maximum of two weeks - Care and Support / Companionship: For example, in crisis situations - Household Services: For example, home cleaning, shopping, cooking, and laundry - Support for Self-Help: For example, counselling, involvement and guidance of relatives, coordination with other outpatient services, and arranging additional services such as assistive devices, meal services, transport services, administrative support, etc. For specific target groups such as children or people with psychiatric conditions, cooperation or service agreements exist with specialized organizations.
