Doctor asked Kroger for the blank waiver and Kroger said the doctor should have it! What a freaking mess! This was after I made a special trip to get a paper prescription from my doctor and still after several phone calls it was not filled. Back to Walgreens who have never failed me yet. Bummed!!!
