First, Dr. Will creates and folds back the corneal tissue flap. The cornea is the transparent, dome-like structure covering the eye’s iris and pupil. By creating a flap in the cornea, Dr. Will can perform laser vision correction on the inner cornea layer, which practically eliminates discomfort and allows for rapid visual recovery. Traditionally, LASIK created the corneal flap with a handheld mechanical device with a metal blade called a microkeratome. Although this method worked well, the performance of these devices could be unpredictable. Although LASIK complications are infrequent, this mechanical device is frequently the source of many reported complications. The meibomian glands, located on our eyelids, secrete the oils that lubricate the eye, reduce evaporation of tears, and stabilize the water layer – helping it to remain balanced between blinks. Due to current lifestyles, which involve the use of prolonged exposure to computer screens, these glands described above frequently become blocked – even in young patients. The images from our advanced LipiView® imaging system seen below demonstrate common problems with the meibomian glands including blockage, gland tortuosity and total gland loss. LipiScan meibomian gland imaging at Will Vision & Laser Centers Gland Dysfunction Causes of MGD MGD (meibomian gland dysfunction) is the most common cause of dry eye in the United States. It is prevalent, progressive, and creates the following eye symptoms: Stinging and burning Sandy or gritty feeling Redness of the eyes Fluctuating vision Contact lens discomfort Difficulty while reading or using electronic devices Once these glands are damaged, they cannot be replaced, but we can improve the function of those glands that remain. For most patients, successful treatment of MGD requires removal of the gland obstruction. Removal can be performed using the LipiFlow® or MiBo ThermoFlo® systems. Modern and Effective Treatments for Dry Eye With over 30 million people suffering from chronic dry eye in the United States, no one solution fits all. On one end of the treatment spectrum, there are lubricating eye drops and eye medications, which are convenient and provide short-term relief; on the other end, patients can choose MiBo ThermoFlo® or LipiFlow® as one of the longest-lasting and higher-end treatments available. LipiView System®, LipiScan®, Vision Break Up Time, & LipiFlow® MiBo ThermoFlo® or MiBoFlo® At Will Vision & Laser Centers, we offer the LipiView® II and LipiScan® technologies, which analyze over one billion data points in each eye. They are the only instruments available that measure the lipid (oils and waxes) layer and evaluate blink dynamics. We can capitalize on real-time information to understand dynamic eye response and lid closure while blinking. The Vision Break Up Time test (VBUT) from Visiometrics allows us to assess how clear the vision is in the eye and are able to assign how much loss is attributable to the tear film. Using double pass wavefront optical engineering, this test provides highly accurate information regarding the degree of light scattering caused by dry eye. The VBUT test also demonstrates improvements in the tear film over time associated with the treatments. The LipiView® II and LipiScan® systems also assess the meibomian gland’s structure, producing high definition pictures of these glands. This is especially useful, as meibomian gland dysfunction is prevalent amongst patients seeking laser vision correction and treatment for chronic dry eye. We are one of a small handful of clinics in the Pacific Northwest to use LipiFlow® and MiBo ThermoFlo® for the treatment of MGD. LipiFlow® is the only FDA approved single-dose treatment for MGD that has been shown to improve gland function and dry eye symptoms with long-lasting results. In the FDA trial, 86% of patients that received only a single LipiFlow treatment did not require additional prescription therapy for dry eye symptoms. Unlike hot compresses, lid scrubs, or older treatments for MGD, the LipiFlow® system applies heat to the inner eyelid, causing the meibomian oils to become liquid and the glands to open. A pulsing massage is then precisely applied to the eyelids. This combination of pulsation and massage gently cleans the blocked material out of the glands and allows them to function properly once again.

Photorefractive keratectomy or PRK is the “original” laser vision correction procedure. The concept behind PRK drove the first wave of laser vision correction procedures in the early 1990s. As a surgical procedure, PRK has continued to undergo refinements, and has a place among modern vision correction solutions used today. While PRK is less prevalent than LASIK, there are many circumstances where this treatment method is the recommended course of action to supply your personal best vision. PRK vs. LASIK Vision Correction Surgery The primary reason why PRK is not as common as LASIK is the slow recovery process. While LASIK patients notice a difference in the quality of their vision within a few hours or a day, healing from PRK can require a week; patients take several days off work for recovery after their surgery, and their vision slowly becomes clear over the following weeks. While both PRK and LASIK use laser technology to treat the cornea, PRK does involve the use of some manual tools, while LASIK can be laser-only and bladeless. Dr. Will recommends PRK in specific circumstances for patients who do not otherwise qualify for LASIK-only procedures. He carefully examines patients to suggest the procedure that will ensure their personal best vision and a safe procedure with minimal side effects. If patients will not be able to achieve clear vision and improved quality of life with LASIK, PRK bridges the gap and provides the customized treatment necessary. Understanding the PRK Procedure The PRK technique differs from LASIK in that, before applying the laser to reshape the cornea, Dr. Will removes the top layer of cells on the cornea, called the epithelium. Having completed this step, he performs the laser vision correction process to change the cornea to the desired shape. Dr. Will then applies a medication to the cornea and to reduce the chance of after-effects, such as corneal haze which creates a cloudy appearance. Finally, we rinse the medication away with a saline solution, and place a contact lens in the eye to serve as a temporary covering while you heal. You’ll need to keep the contact lens in your eye continuously until the epithelial layer grows back over the cornea, which typically occurs within three to five days of your procedure. From a quality-of-vision perspective, there is no fundamental difference in the results delivered by LASIK vs. PRK. LASIK is not superior to PRK or vice versa. Like LASIK, the goal of PRK is to correct errors in refraction such as nearsightedness, farsightedness, and astigmatism. With advanced excimer lasers, like theJohnson and Johnson Vision –VISX Star S4 IR, it is also easy to use PRK to perform Wavefront-guided treatments directed at correcting both simple and complex vision abnormalities. Dr. Will can complete your PRK procedure in as few as 15 minutes if he is treating both eyes – even less if you are only having one eye targeted. Post-Operative Care During the healing phase, you will need to be diligent about applying anti-inflammatory, antibiotic, and lubricating drops into your eyes. We frequently send our patients home with medication to help with sleep and improve comfort for the first few days postoperatively. Dr. Will ensures you have complete instructions for what to expect immediately after your procedure and in the following days and weeks as healing progresses. We will ask you to wear the contact lenses placed after surgery until Dr. Will approves you to remove them. Your vision will be blurry for several weeks before it begins to clear. Ultimately, the quality of vision will be indistinguishable from LASIK. Do You Qualify for PRK in Portland? At Will Vision and Laser Centers, we typically select PRK as an option when a patient’s cornea is not considered thick enough to undergo LASIK surgery. However, because we use the iFS Advanced Femtosecond laser, we can complete exquisitely precise surgical protocols, so we can often perform LASIK for patients that who told that they need PRK by surgeons who either don’t have access to advanced technology or who have inadequate experience in treating patients with thin corneas.

When the environmental factors are ignored or left untreated, those people predisposed genetically to nearsightedness may experience even further progression of this common vision disorder. Nearsightedness, also known as myopia or shortsightedness, refers to an eye condition in which individuals can clearly view objects up close but struggle to see those that are far away. The further an object is in the distance, the blurrier it may appear. Those who are nearsighted may not realize it at first, as not every object will appear blurry. For example, those who are nearsighted generally have no difficulty reading a book or computer screen but may struggle to see traffic signs that are several hundred feet away. Those who are nearsighted may also experience such ailments as frequent squinting, eye strain, headaches, and feelings of fatigue while driving or playing sports. What Causes Nearsightedness? In the past, genetics were believed to be responsible for nearsightedness. However, environmental factors that appear to cause nearsightedness have recently been identified. Based on recent population studies, the most likely sources for this newfound prevalence in nearsightedness is the increased use of near vision for viewing smart phones, tablets and computers. In addition, large scientific studies have linked light exposure to progression of myopia. As a result of this new research, eye doctors can now suggest that nearsightedness may be worsened by: Prolonged reading in low light Looking at a smartphone, tablet or computer screen for excessive periods of time Spending too much time indoors These studies suggest that people who spend more time outdoors in bright sunlight experience less progression of nearsightedness than those who spend most of their time indoors in artificial light. Bright light appears to stimulate the production of growth hormones that decrease the tendency for the eye to continue to elongate and create myopia. Identifying and Preventing Nearsightedness Diagnosing this condition as soon as possible is for the better. Regular eye exams are important to help determine the issue, as nearsightedness often develops at an early age. If you are nearsighted, there’s a high possibility your child will become nearsighted, too.
