Human Performance Training Facility and Physical Therapy Clinic. Training Services: We train Kindergarten through Pro level athletes, Adult Fitness, Online Coaching, Consultation, and more. Physical Therapy: We are an outpatient orthopedic physical therapy clinic taking patients of all ages dealing with orthopedic issues. We are directly in network with Blue Cross Blue Shield, Medicare, and United. We also work with all major insurance providers. In order to verify benefits we need an active insurance card, identification, and for minors we need their parents identification.
