Back pain is the second most common reason for a visit to a physician in the United States, and 70 to 85 percent of people have significant back pain at some point in their life. Types of Back Pain Back Pain can be described as acute or chronic. Acute back pain is commonly described as sharp and severe; it tends to come on suddenly but also improve with time and short-term conservative treatment, such as medication, exercise, physical therapy or rest. Chronic back pain is typically over 12 weeks in duration and is commonly described as a deep, aching, dull or burning pain. Chronic pain generally arises over time as the body ages and as the back is subjected to the wear and tear of daily living. Chronic pain can also be approached with conservative, nonoperative measures, though it tends to last a long time and may not respond as well to conservative care. Many sudden attacks of acute back pain are the result of overstretched muscles (strains) or ligaments (sprains). The pain may be most severe immediately after injury, or it may worsen gradually over a few hours. In most instances, back pain as a result of strain or sprain can be resolved following a conservative course of treatment – usually within two to six weeks – provided there are no serious underlying medical conditions. People suffering from chronic pain may be experiencing mechanical pain, which means that a specific part of their spine, such as a disc, a ligament, or a joint is damaged and is not working correctly. Examples of problems that lead to mechanical pain include degenerative disc disease, spondylolisthesis,spinal stenosis, herniated discs and facet joint degeneration. Causes of Back Pain Causes of back pain consist of fractures (including those due to osteoporosis, or thinning of the bone), developmental disorders of the back such as scoliosis and kyphosis, inflammatory and infectious disorders, and tumors. Patients should consult a physician if they are experiencing back pain as a result of a physical trauma to the spine, if they are experiencing numbness in, or having difficulty moving, their extremities, or if they experience bladder control loss or impairment. Acute back pain treatment often consists of physical therapy or epidural steroid injections initially. If these treatments fail, surgery may be offered. Chronic back pain responds less well to physical therapy and epidural steroid injections but these treatments may still be indicated depending on the problem being treated. What is Minimally Invasive Spine Surgery? Minimally invasive spine surgery has been developed as an effective technique for the treatment of spinal conditions. The technique involves smaller incisions and less blood loss, resulting in faster recovery. There are three main types of radiculopathy that you might deal with and your neurosurgeon will examine your pain and pinched nerve to determine which kind you are dealing with. Dealing with Radiculopathy There are three main types of radiculopathy that you might deal with. These types are known as cervical, thoracic, and lumbar. A laminectomy is a surgery that creates space in your spine by removing bone spurs and growths. Nonsurgical Neck Pain Relief Neck pain can occur for a variety of reasons. Most of them are best treated without surgery. These include neck arthritis, muscle spasms, whiplash from a car accident, and even stress, among many others. Most of these conditions can be managed with gentle anti-inflammatory medicines, rest, or muscle relaxers, and physical therapy. Chiari Malformation is a type of structural defect involving the cerebellum, the part of the brain that controls balance. The cerebellum and parts of the brainstem sit in an indented space at the lower rear of the skull, above the foramen magnum (a funnel-like opening at the base of the skull leading to the spinal canal, through which the brainstem connects to the spinal cord). Normally, only the spinal cord passes through the foramen magnum, but in Chiari malformations, the lower part of the cerebellum extends downwards and into the foramen magnum. Schedule a Consultation Chiari Malformations may develop when the bony space where the cerebellum is housed is smaller than normal, causing the cerebellum and brainstem to be pushed downward into the foramen magnum and into the upper spinal canal. The resulting pressure on the cerebellum and brainstem may affect functions controlled by these areas and block the flow of cerebrospinal fluid (CSF) — the clear liquid that surrounds and cushions the brain and spinal cord — to and from the brain. Treatment options vary. For patients who are asymptomatic surgery may not be needed. For those with significant symptoms, surgery is required to provide more space for the cerebellum and halt the progression of damage to the central nervous system. Unfortunately, there are no medicines that can correct Chiari malformation. Chiari Malformation Treatment Center Chiari Malformation Surgery involves making an incision at the back of the head and removing a small portion of the bottom of the skull around the foramen magnum, thus creating more space for the lower cerebellum. Often, surgery also requires laminectomy of the first portion of the cervical spine, where part of the arched bony roof of the spinal canal (the lamina) is removed. The chiari malformation surgeon also will make an incision in the dura (the covering of the brain) to examine the brain and spinal cord. Additional tissue may be added to the dura to create more space for the cerebellum and allow normal flow of cerebrospinal fluid. What are Meningiomas? Meningioma is generally a benign growth on the brain. About 15% of all brain tumors are meningiomas and they are the most common primary brain tumor. These tumors typically attach to the dura. They are twice as common in women than men. Brain and meningiomas are more common with older age. These tumors arise from the covering of the brain. Most meningiomas grow inward toward the brain as discrete, well-defined, dural-based masses. Most of them are spherical or lobulated. Rarely, en plaque meningiomas infiltrate the dura and grow as a thin carpet or sheet of tumor along the convexity dura, falx, or tentorium. Meningioma Brain Tumor Symptoms Meningiomas can remain clinically asymptomatic for years because of their slow growth. They can sometimes reach a large size, especially in the frontal lobes, with few symptoms. The primary complaints of patients often include weakness, numbness, seizures, cognitive dysfunction, depression, and headaches. Although the vast majority of meningiomas are benign, a few can be malignant, or cancerous. Meningioma Treatment for Brain & Spinal Tumors Treatment for Meningiomas Tumors usually consists of either surgery, radiation, or observation. There is no medical treatment or chemotherapy available for benign brain and spinal meningiomas. For tumors causing pressure on critical brain structures, surgery may be the only treatment option. For certain small tumors, or asymptomatic tumors, radiation can be a very effective treatment. The most effective type of radiation is focused beam stereotactic radiation, also called radiosurgery.
