An eye condition in which the naturally transparent lens of the eye becomes cloudy, cataracts interfere with the ability of the lens to properly focus light on the retina at the back of the eye. This unfocused light causes vision to appear blurry, hazy, distorted, or cloudy. Cataracts often begin as a slight cloudiness which increases as the condition worsens. DSAEK is a corneal surgery procedure for extreme instances of cornea problems, corneal disease, or for damaged corneas. The cornea is the outer surface of your eye. If you are not familiar with the cornea it is a remarkable structure that helps to focus light and protect the internal structures of the eye. Request Appointment The shape of the cornea is important to the focusing of light on the retina in the back of the eye. A properly shaped cornea and healthy lens will allow this light to focus clearly, providing crisp vision. It is also important that the cornea remains clear so the light can pass through to the retina without being interrupted. If the cornea gets hazy or cloudy, or if the shape of the cornea becomes irregular, vision can be impaired. Some causes of this include age, trauma, or disease. When vision is impaired enough to affect a patient’s daily functioning, it may need to be totally or partially replaced by a corneal transplant procedure. What is DSAEK? DSAEK is a relatively new, but well-tested corneal surgery technique that can help some patients who previously required a full thickness corneal transplant. The procedure is similar to the traditional cornea transplant because both use donor corneas to replace damaged or diseased corneas. Both the conventional corneal transplant technique and DSAEK require the use of a donor cornea, but DSAEK replaces only the damaged posterior section of your cornea. In a conventional corneal transplant surgery, the entire cornea is replaced. This procedure involves large incisions, many stitches, and a very long recovery time. In contrast, DSAEK requires minimal suturing, allows more rapid visual restoration, less discomfort, and a reduced risk of sight-threatening complications. In DSAEK surgery, the ophthalmologist will start by making a small incision in the sclera (white tissue, of the eye). This incision gives access to the inner surface of the cornea. The diseased endothelial tissue is removed through the incision, and the donor tissue from the eye bank is implanted. What happens in DSAEK cornea surgery? Fuchs’ Dystrophy patients are the primary group of patients needing a DSAEK procedure. Fuchs’ endothelial dystrophy (FED) is a degenerative disorder of the corneal endothelium leading to corneal edema and loss of vision. Patients first receive a full evaluation so that our doctors can best determine the patient’s treatment options, including the need for DSAEK surgery. On the day of surgery, the patient will arrive at the surgery center approximately one hour prior to surgery for preparation. The patient must not eat anything after midnight the day before surgery and will begin pre-operative medications three days prior to surgery as instructed. Prior to the patient’s surgery, the surgeon will prepare for surgery by determining the type of tissue to be used and selecting the technique needed to ensure the best surgical outcomes possible. After the (approximately) one-hour surgery, the patient must lie flat on their back so air can push up into the cornea and hold the new tissue in position. Once the tissue sticks to the cornea, it will begin to function and pump the water out of the cornea, clearing vision. Vision improves fairly rapidly, with final visual results obtained in approximately one to six months after surgery. Chart Showing a Healthy Eye Compared to One With Dry Eye Dry eye is a condition that occurs when your eyes aren’t producing enough tears, or the tears that you create are low in quality. Because there are not enough tears, it becomes difficult to keep a healthy film of tears covering your eyes. The disruption in tears could result from your body making fewer tears than needed. It’s also possible that your tears are deficient in one of the layers required for having a healthy tear film. Your tear film needs to have three layers: oily, water, and mucus. If any of these necessary components are missing or are lacking, it can result in experiencing dry eyes. What are the Symptoms of Having Dry Eyes? If you have dry eyes, your eyes may feel irritated and uncomfortable. Stop Smoking and Avoid Other Smokers If you’re a smoker, surrounding yourself with cigarette smoke is not doing your eyes any favors. Being around secondhand smoke from smokers can irritate your eyes, making them feel dry, itchy, or generally uncomfortable. While glasses and contacts are the most common, and LASIK is gaining in popularity, there is also a procedure called refractive lens exchange (RLE) that you might want to consider. RLE is an alternative surgical procedure to LASIK designed to treat patients with presbyopia (age-related blurry near vision), and it can be a viable option for individuals who are not eligible for LASIK. Sometimes RLE is referred to as refractive lensectomy, clear lens extraction, or simply lens replacement surgery. So how does it work? The goal of refractive lens exchange is to replace the natural lens of your eye with something called an “artificial intraocular lens.” These lenses come in three categories: Monofocal IOLs allow you to see clearly and focus in three distinct distances, but does not allow for in between, or a combination. Multifocal IOLs function much the same as monofocal, but with more available distances. Accommodating IOLs actually adjust inside the eye as your eyes change focus. RLE vs. LASIK Refractive lens exchange and LASIK are both safe and effective, but each has distinct advantages and disadvantages. For example, RLE is typically reserved for patients with farsightedness, and while it can be used for nearsighted patients, it is more of a last resort. In general, LASIK is more effective, has a lower procedure and recovery time, and is preferred by doctors and patients alike. However, LASIK is not right for people who have abnormal corneas or an extreme refractive error. In those severe cases, RLE is the better option. If you suffer from keratoconus, your eye doctor may recommend a treatment called corneal cross-linking to slow down the condition’s progress. Before having any eye surgery, it’s necessary to understand what’s involved. At RGB Eye Associates, we’re dedicated to providing our patients with only the best medical and surgical treatment available, whether in the form of corneal cross-linking or other care. What is Keratoconus? Keratoconus is an eye condition that occurs when your cornea begins thinning out. When this happens, it starts changing shape, bulging, and may look cone-shaped. As the cornea changes shape, it causes your vision to become blurry. If your keratoconus is mild, your eye doctor may prescribe eyeglasses or contact lenses to correct your vision. But if your eyesight continues to get worse, your eye doctor may recommend other treatments such as corneal cross-linking. What is Corneal Cross-Linking? Corneal cross-linking is a treatment for keratoconus that strengthens the collagen in the cornea. Collagen is a natural protein in the cornea that helps provide structure. The cross-linking procedure causes the collagen fibers to bond, strengthening the structure of the cornea and making it more stable. Cross-linking won’t make your cornea return to a rounded shape, but it can keep your keratoconus from getting worse. In addition, undergoing corneal cross-linking may prevent you from needing a corneal transplant, a major surgical procedure. What Happens During a Corneal Cross-Linking Procedure? Corneal cross-linking is an outpatient procedure. Your eye doctor at RGB Eye Associates can perform this in-office. The treatment takes about an hour to 90 minutes. Because corneal cross-linking is an outpatient procedure, you’ll be able to go home after it’s over. Corneal cross-linking uses riboflavin, also called vitamin B2, and ultraviolet light to help strengthen the collagen in your eyes. Before starting the procedure, your eye doctor will have you lie down and put numbing drops in your eyes. If you’re nervous, your ophthalmologist may also give you a sedative to reduce any anxiety and help you calm down. Next, your eye doctor will put special riboflavin eye drops in your eyes and let them absorb into your cornea. It usually takes about 30 minutes for your eyes to absorb the drops. After your cornea takes in the drops, your eye doctor will use a specialized device to shine ultraviolet light rays into your eye. The light activates the riboflavin, helping to form new bonds between strands of collagen in the cornea. You may receive up to thirty minutes of light treatment. Once the light treatment is complete, your eye doctor will place a bandage contact lens over your eye. The bandage contact lens will help protect it and help it heal. You will not be allowed to drive yourself home after having corneal cross-linking treatment. Your vision will be blurry, so make sure you have a friend or someone in your family who can get you home. What is the Recovery Process After Undergoing Corneal Cross-Linking Treatment? It’s important to follow all instructions from your eye doctor while your eyes are healing after having corneal cross-linking. A crucial component of your recovery will be using prescription eye drops. Make sure you take them as directed. Let your eye doctor know if you have any issues or if you’re experiencing any pain. Feeling any pain could be a sign of a complication or an infection. For the best possible recovery, avoid the following until your eye doctor clears you: Rubbing your eyes Applying or wearing eye makeup Getting water in your eyes Heavy exercise Smoky or dusty areas Are There any Side Effects After Having Corneal Cross-Linking? After treatment, you may feel some mild discomfort. Common complaints after the procedure include: Dry eye Sensitivity to light A sensation like there’s something in your eye Blurry vision Mild pain in your eye Your eye doctor can prescribe medication to help with any mild discomfort. Contact your eye doctor immediately if you have severe eye pain or experience a sudden change in your vision, including losing your ability to see. You should also call your eye doctor if the bandage contact lens falls out of your eye. Do not try to put the contact back in. How Long Does It Take to Recover After Corneal Cross-Linking? Although every patient is different, most people can start returning to their usual activities about a week after corneal cross-linking. It’s best to check with your eye doctor to find out when you can resume these.

Request Appointment What is the EVO ICL? The EVO ICL is an implantable collamer lens. It is a refractive lens that your surgeon implants in the eye to improve your vision. When you have the EVO ICL implanted, it’s placed behind the iris and in front of your natural lens. The iris is the colored part of the eye. The natural lens remains in place. You can have the ICL removed at any time if your prescription changes or if you decide to have it taken out. What is a Collamer lens? The ICL is made of a material called collamer. Collamer is a collagen co-polymer that contains purified collagen. The lens also contains materials similar to what you find in soft contact lenses. One essential advantage of collamer is that it’s biocompatible, meaning it won’t cause a reaction with your eye. This is an advantage if you’re predisposed to having dry eyes because the EVO ICL does not cause dry eye syndrome. The lens is stable and contains an ultraviolet light filter. What are the Benefits of the EVO ICL Procedure? The most significant benefit of choosing the EVO ICL is that it can permanently correct or reduce nearsightedness, allowing you to see clearly at a distance without glasses or contacts. It can also improve astigmatism. Unlike some procedures, the EVO ICL also provides excellent night vision, so you can drive or enjoy being outdoors at night and still see clearly. The EVO ICL also doesn’t contribute to dry eye, so your eyes may feel more comfortable than they do when you wear contact lenses. Other corrective procedures can contribute to dry eye. The EVO doesn’t require any maintenance and will remain in your eye for as long as you desire. If your eye doctor removes the ICL, you will no longer have corrected vision. Because it is reversible, implanting the EVO ICL does not involve reshaping the cornea. Once you remove it, your eyes will return to the way they were before you had the EVO ICL. Who is a Good Candidate for the EVO ICL? The only way to know if you’re a good candidate for the EVO ICL is to schedule a consultation with your eye doctor. During the consultation, your eye doctor at RGB Eye Associates will perform a complete eye exam to determine if the EVO ICL is a good fit for you. This may include your ophthalmologist studying the shape of your eye and then measuring the angle in the front chamber of your eyes. Taking these measurements will ensure there’s enough space to insert the EVO ICL. The ICL can correct moderate to high levels of nearsightedness. Your nearsightedness should be stable for at least a year. Your prescription for corrective lenses, including glasses and contact lenses, should remain stable for at least 12 months before you can undergo the vision correction procedure. The procedure is for patients between the ages of 21 and 45. Pregnant or nursing patients should wait at least three months to have the surgery, as it can cause their eyes to change. Will I Be Able to See or Feel the Lens? No one will be able to see the ICL after you have it inserted in your eye since it’s positioned behind the colored part of your eye, the iris. You shouldn’t be able to feel the EVO ICL after having it inserted. The lens won’t move around or attach to anything within your eye. What is Having the EVO Visian ICL Procedure Like? Having the EVO ICL implanted in your eye is an outpatient procedure, meaning you can go home the same day. It’s minimally invasive and only takes about half an hour to complete. Before it begins, your eye doctor will give you eye drops to numb your eyes and dilate your pupils. You shouldn’t be able to feel any discomfort during the procedure. Your eye doctor will create a small opening at the base of the cornea. The opening is where they will insert the lens. After making this incision, your eye doctor will fold the lens and insert it, adjusting the ICL as necessary for correct placement in your eye. Once they are confident the EVO ICL is in the right place, the procedure is over. You may start experiencing improved vision right away. Your eye doctor will prescribe eye drops to help with healing, and you’ll need to attend a follow-up appointment a day or two after going home. You can’t drive yourself home, so make sure you have a trusted friend or family member that can.

Examples of MIGS include: 1. Drug Delivery Therapy With the iDose and Durysta implants, patients have an easy and effective alternative to costly and cumbersome eye drops. These implants can be done in the office (Durysta) or in the operating room (iDose) to place a sustained-release medicated implant inside the eye that directly lowers eye pressure at the site of greatest effect. 2. Microstents Our ophthalmologists are skilled in the placement of the iStent and Hydrus microtrabecular bypass stents, which are among the smallest implants approved for use in the human body. These stents lower the pressure inside the eye in patients with glaucoma, and may be placed at the same time as cataract surgery to protect your eyes from vision loss. 3. Canaloplasty Pressure in the eye may become elevated due to narrowing and lack of function of the eye’s natural drainage system. With canaloplasty, a microcatheter is used to dilate this drainage system and restore the eye’s natural outflow.
