Outreach clinics support transplant evaluations, follow-up care and ongoing coordination for patients across Texas.

Physicians may refer patients to the Transplant Institute for evaluation. Patients may also request a consultation — the Transplant Institute accepts referrals via an online form or by contacting the transplant team by phone.
