Face

In time, gravity, sun exposure, and the stresses of daily life take their toll on our faces: deep creases appear beside the mouth, the jaw line slackens, and folds and fat deposits appear on the neck. Facelifts counteract these problems by removing fat and tightening skin and muscles, giving your face a fresher, youthful look. After surgery, some patients appear 10-15 years younger. A facelift can be combined with a browlift, eyelid surgery or nose reshaping for more dramatic results, or it can be restricted to the neck (necklift) if the patient's problems center there. Incisions are made in inconspicuous places such as behind the hairline and in natural folds of the face and ears, and scars fade to near invisibility in time. Results of a facelift do not last forever, but in another sense, the effects are permanent; years later, your face will continue to look better than if you had never had the procedure. Mini Face Lift A Mini Facelift is also called a "weekend" facelift because the procedure can be done in a shorter period of time than the traditional facelift. Unlike the regular facelift that covers the entire face, a mini facelift only focuses on specific areas. The areas that are focused on are the jaw line, neck, and the folds of the nose. This particular procedure is normally catered to those in their 30s and 40s who want to remove minor sagging. Results depend on the patient's nasal bone and cartilage structure, facial shape, skin thickness and age (teenagers should have had their growth spurt). Insurance may cover rhinoplasty if it is done for reconstructive or medical reasons, but likely not for cosmetic purposes. Rhinoplasty is usually an outpatient procedure performed under either local or general anesthesia and lasts one to two hours unless more extensive work needs to be done. Surgeons use one of two techniques: the incision is either made within the nostrils, thus hiding scars after surgery, or across the columella (the vertical strip of tissue separating the nostrils) in an "open" procedure, where scars are small and hidden on the underside of the base. In both procedures the skin is lifted, the bone and cartilage sculpted, and the skin replaced and stitched closed. For a short time after surgery you may experience puffiness, nose ache or a dull headache, some swelling and bruising, bleeding or stuffiness. Most patients feel like themselves within two days and return to work in about a week. Contact lenses can be worn immediately but glasses will have to be taped to your forehead or propped on your cheeks for up to seven weeks. Complications are rare and, when they occur, minor. These may include infection, nosebleed, or a reaction to the anesthesia.

Breast

Breast augmentation is the most frequently performed cosmetic surgery procedure in the U.S. It can give women with small or unevenly-sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. Women may elect to undergo breast augmentation for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery. The procedure may be combined with others such as a breast lift for more satisfying results. Implants are silicone shells filled with saline (salt water) and are placed behind each breast, underneath either breast tissue or the chest wall muscle. The procedure lasts one to two hours and is typically performed with general anesthesia. After surgery the patient's bustline may be increased by one or more cup sizes. Incisions are made in inconspicuous places on the breast to minimize scar visibility (in the armpit, in the crease on the underside of the breast, or around the areola, the dark skin around the nipple). The breast is then lifted, creating a pocket into which the implant is inserted. Placement behind the chest wall muscle offers a few advantages over placement beneath the breast tissue only. These include reduced risk of capsular contracture (post-operative tightening around the implant) and less interference with mammogram examinations. Possible disadvantages include need for drainage tubes and elevated pain in the first few days following surgery.

A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have sagged as a result of weight loss, pregnancy, loss of the skin's natural elasticity or simply the effects of aging and gravity. The procedure can also reduce areolar size (the dark skin surrounding the nipple), and it can be combined with breast augmentation for added volume and firmness. It is usually done under general anesthesia, and lasts from one to three hours. Women planning to have children are advised to postpone surgery, since pregnancy and nursing can counteract its effects by stretching the skin. However, breast lift should not affect your ability to breast-feed. Breast lift may be performed in a hospital, an outpatient surgery center or a surgeon's office-based facility. It is usually done on an outpatient basis under general anesthesia, and lasts from 1-½ to 3-½ hours. A number of pre-operative steps are typically taken such as a mammogram, measurement of the bustline, and discussion with the surgeon about the desired size and shape of the breast and placement of the nipple. During the procedure an anchor-shaped incision is made from the location of the new nipple down to and around the crease beneath the breast. The surgeon removes excess skin, relocates the nipple and areola, and reshapes the breast using skin from around the areola before closing the incisions with stitches. Patients with small breasts and minimal sagging may be recommended for smaller-incision mastopexy. One such modified procedure is concentric ("doughnut") mastopexy, in which two concentric circular incisions are made around the areola and a doughnut-shaped swathe of skin is removed.

Breast reduction surgery is usually done to provide relief from these symptoms. Stitches come out in a week and the surgical bra must be worn for about a month.

Breast reconstruction restores a natural, symmetrical appearance to the bosom, maintains body proportion, allows clothes to fit better, and most important of all, boosts self-confidence for women who have lost one or both breasts to mastectomy or who lack breasts due to a congenital or developmental abnormality. Reconstructive surgeons strive to create a new breast and nipple that resemble the woman's natural breast as closely as possible in shape, size and position. Women whose cancer seems to have been eradicated with mastectomy are the best candidates for breast reconstruction. Those with health problems such as obesity and high blood pressure and those who smoke are advised to wait. Others prefer to postpone surgery as they come to terms with having cancer, consider the extent of the procedure, or explore alternatives. The reconstruction itself consists of multiple operations, the first of which involves creation of the breast mound and is performed during or after mastectomy in a hospital under general anesthesia. Later surgeries, if necessary, may be done in the hospital or an outpatient facility, with either general or local anesthesia. There are several ways to reconstruct the breast, both with and without implants; your breast surgeon and plastic surgeon should work together with you in deciding which is best for you.

Gynecomastia is a common condition that affects nearly half of all men, resulting in localized fat and/or glandular tissue in the breasts. For most cases of gynecomastia, the cause is unknown, but this condition may be linked to certain medications or diseases. Many men are embarrassed by this condition and seek treatment to achieve a smooth, contoured chest through male breast reduction. Male breast reduction may be performed using liposuction, surgery or a combination of the two, depending on the amount and type of tissue found in the breasts. If the breasts consist of mostly fatty tissue, liposuction may be used to suction out fat from an incision in the nipple or underarm areas. For breasts with an excessive amount of glandular tissue, excision surgery may be performed, which requires cutting away the excess fat, skin and tissue through a larger incision. Your surgeon will determine which technique is best for you based on your individual goals for surgery. After breast reduction surgery, patients will usually be able to return home the very same day. Post-operative symptoms may include swelling and discomfort, which can be managed through pain medication and compression garments worn for the first few days after surgery. Patients will be able to return to work once they feel well enough, but should avoid sexual activity and strenuous exercise for a week or two while the chest heals. For most patients, breast reduction surgery produces a flatter, more well-defined chest that can last for many years, as long as patients maintain a stable weight.

Abdominoplasty is a surgical procedure that helps flatten and shape the abdomen and waist by removing excess fat and skin and tightening muscles. The best candidates are in good physical condition with pockets of fat or loose skin that haven't responded well to diet and exercise. Older, slightly obese people whose skin has lost some of its elasticity can be helped. Abdominoplasty can also be useful for women with stretched skin and muscles from pregnancy, although results from the procedure will diminish if they continue to bear children. For this reason, women who plan to have children again are advised to delay abdominoplasty. Patients who intend to lose a lot of weight are also encouraged to wait before undergoing the procedure, since skin in that area typically loosens after substantial weight loss. The procedure takes approximately two to five hours and can be performed with general anesthesia or local anesthesia with a sedative. Recovery time ranges from two weeks to a few months, but with a balanced diet and regular exercise results are long-lasting. Abdominoplasty will likely leave a permanent scar stretching between the hips, although it should be low enough for even a bikini to conceal it. Patients who have had previous abdominal surgery should know that their old scars could be raised, stretched or generally more noticeable after the procedure. Other, rare post-operative complications include infection, blood clots, and poor healing, which may require further surgery. Your surgeon will make two incisions: one from hipbone to hipbone close to the pubic area, and one around the navel. Skin is separated from the abdominal muscles, which are then pulled together and stitched into place for a firmer abdomen and narrower waist. The skin flap is stretched down and over the newly tightened muscles, excess skin is removed, and the navel is reattached. Finally, the incisions will be stitched, dressings will be applied, and a temporary tube may be inserted to drain excess fluid from the surgical site.
