Monument Health Home+ Home Health and Hospice - Rapid City

Compassionate, skilled providers work with patients and families to meet physical, emotional, social and spiritual needs for patients nearing the end of life. Care and support are provided by an experienced team including nurses, social workers, nursing aides, physical therapists, counselors, chaplains, physicians, pharmacists and volunteers. Hospice provides acute (24/7 in-home care), respite (temporary) and residential (patient resides at Hospice House) care. Hospice is on-call 24 hours a day, 365 days a year.
