In-Office Dry Eye Treatments Intense Pulsed Light (IPL) Therapy Our IPL is delivered using Optimum Power Energy (OPE) technology. This utilizes polychromatic light delivered through a handpiece directly to the skin via thermal pulses to revitalize the tissue to resume their normal activities. OPE technology allows us to perform pain-free IPL and without having to use ultrasound gel. It was FDA approved for dermatologic use in 1995 to treat rosacea and over the last decade has gained popularity in the eyecare world to treat ocular rosacea, the root cause of many cases of evaporative dry eye. It removes vascular lesions, pigmented lesions, and has a powerful anti-aging effect. Studies have proven that IPL is anti-inflammatory and reduces most common inflammatory mediators involved in dry eye. Low Level Light Therapy (LLLT) LLLT is a unique technology of photobiomodulation (PBM) used for many years in various fields of medicine (dermatology, dentistry, etc.). We use 15-minute LLLT mask treatments. Red mask: The emission of red light improves cellular metabolism by triggering the mitochondrial production of ATP and increased oxygen utilization as well as an endogenous heating. This facilitates production and stimulation of the oil component released from meibomian glands into the tear film, thus stabilizing the top lipid layer of the tears, reducing the signs and symptoms of evaporative dry eye. This also has a powerful anti-aging effect by tightening collagen. Blue mask: The emission of blue light is antimicrobial, stimulating the release of perforins. Perforins punch holes in the cell membranes of the bacteria and mites that are known to colonize our eyelids and contribute to the inflammatory cycle of dry eye. It is also beneficial for patients with rosacea and acne. Meibomian Gland Puncture This is performed prior to Miboflo for patients whose oil glands are blocked by scar tissue that has formed as a result of chronic environmental stress such as contact lens wear and makeup. BLEPHEX EYELID AND LASH CLEANING This treatment cleans the eyelids and lashes of dirt, debris, bacteria, and the demodex mite, which are common causes of chronic eyelid inflammation and ocular irritation. It also exposes the oil glands of the eyelids, preparing them for further treatments. Amniotic Tissue Treatment This is a treatment during which a membrane (similar to a contact lens) composed of amniotic tissue is placed on the cornea to repair the damaged surface of the eye. REFRACTIVE LENS EXCHANGE Refractive lens exchange (RLE) is a type of vision correction surgery that involves using ultrasound to remove the eye’s natural lens and replacing it with an intraocular lens (IOL) to minimize the use of glasses and contacts. It is an option for some patients who are not a candidate for LASIK surgery. ACCOMMODATIVE LENSES Accommodation is a natural process that allows the eye to focus on objects at varying distances. As the lens becomes more rigid with age, accommodation declines. Accommodative Intraocular lenses (IOLs) are tiny, clear, plastic lenses that are surgically implanted into the eye and attach to the eye’s focusing muscles. Accommodating IOLs are primarily used for the treatment of cataracts. Accommodating IOLs are unique because they move inside your eye in order to accommodate focusing at different distances. The Siepser Eyecare team attaches a tiny hinge to the eye’s focusing muscles so that the patient’s eye can once again change the shape of the lens to focus. Dependence on reading glasses is dramatically reduced or even eliminated, along with the cataract and blurry vision. PRK Photorefractive keratectomy (PRK) is a type of refractive surgery for the correction of myopia (nearsightedness), hyperopia (farsightedness), and astigmatism. PRK uses a cool beam of light to reshape the surface of the cornea. It is a good solution for patients that are not a candidate for LASIK due to a thin cornea or other conditions. The recovery from PRK can take a bit longer and be slightly less comfortable than LASIK, however, the Siepser team has developed a process and care that ensures their patients are as comfortable as possible during their healing to restored good vision. LIGHT ADJUSTABLE LENS The solution to correcting cataracts is to undergo surgery where the surgeon will remove the cloudy lens and replace it with a clear intraocular lens (IOL). Historically, the surgeon would select a lens prior to the procedure that would best meet the patient’s needs and would have limited options to adjust the lens post surgery to further improve vision. RxSight™ now offers a Light Adjustable Lens (LAL) that allows the surgeon to adjust it after the procedure to optimize the patient’s vision. How does it work? The surgeon completes the cataract procedure as usual. The only difference is that the surgeon would implant the LAL instead of a standard lens. In the following two weeks after the procedure, the patient will return to the office to undergo non-invasive light treatments to adjust the lens to the specific patient’s needs to achieve customized, optimal vision. The light treatments last approximately 90 seconds, are non-invasive and painless. Patients may require 2-4 light treatments over 1-2 weeks post cataract procedure to achieve optimal vision. Because the lens does react and change with exposure to light, patients are required to wear specific UV protective glasses until their lenses have been adjusted to the optimal level and undergo a final treatment to “lock in” the results.
