You don’t have to be an athlete to need sports medicine. Whether you have a joint, tendon, ligament, muscle, nerve, or other injury due to an accident during physical activity, the physicians at FoRM Health in Portland, Oregon can help. You’ll find a mix of traditional and specialty, non-surgical orthopedic (referring to bones, muscles, and related soft tissues) therapies aimed at helping the body to heal and recover. For many conditions, your FoRM practitioner may choose manual (hands on) therapies initially, before proceeding to advanced techniques for more stubborn conditions. If you suffer from chronic, longstanding injuries or arthritis, however, Platelet Rich Plasma (PRP) injections, MFAT or shockwave are particularly effective and may be chosen right away. These therapies help stimulate (or re-stimulate) your body’s healing response. FoRM also utilizes the advantage of ultrasound guidance to ensure safe and proper administration of solutions being injected. Of course, some conditions require surgical orthopedic intervention. If this is deemed the case during your assessment, or at any time during your care, we are happy to coordinate referrals and play a supportive role in both pre- and post-surgical rehab, as needed. What is Sports Medicine? Sports medicine focuses on treating injuries sustained during exercise or physical activity. Although professional and recreational athletes tend to encounter the most sports injuries, you can suffer the same injuries during daily activities, such as on a neighborhood walk or working in the garden. Some common sports medicine injuries include: Sprains and Strains Shin splints Meniscal tears Hip and Shoulder Labral tears Plantar fasciitis Rotator cuff tears Tennis and Golfers’ elbow Disc bulges and herniations What are other orthopedic or musculoskeletal conditions treated? Some injuries listed above may not result from a specific injury, but instead are more degenerative in nature, meaning they develop over time from a number of factors. Other conditions you may not develop immediately, but instead result from poor posture or movement patterns over time leading to stress on your soft tissues and nerves resulting in pain. Examples of these conditions include: Degenerative Joint conditions including disc disease, meniscus tears, and arthritis Tendinosis and Tendinitis Bursitis Carpal Tunnel Sciatica Nerve entrapments Tension headaches TMJ issues Motor Vehicle Accident sequelae Chronic loose ligaments (aka ligament laxity, hypermobility syndrome, Ehlers Danlos) How are sports and non-surgical orthopedic injuries treated? Your first visit is a full hour to get a complete history, detailed exam, order or perform imaging as necessary, and discuss a treatment plan. Our goal is both alleviating your pain and helping your body heal itself. Some treatment options include: Dry needling Sports Chiropractic Physical Therapy Platelet-rich plasma (PRP) injections MFAT Cell Therapy (Fat stem cell injections) Prolotherapy Focused Shockwave Therapy (ESWT) Sports Acupuncture Myofascial release Kinesio taping Cupping, GuaSha, IASTM Ultrasound-guided nerve hydrodissection Neural therapy Perineural Injection Therapy (aka Neural Prolo) Prolozone Steroid, micro-steriod, or hyaluronic acid injections Nutritional and Supplemental, as needed Beyond injections and localized therapies, FoRM Health in Portland, Oregon also offers comprehensive support to optimize healing time. If you suffer a sports or musculoskeletal injury, a general acupuncture approach may not be sufficient. The acupuncturists at FoRM Health in Portland, Oregon offer athletes and lay people alike a more specialized approach. Sports acupuncture and dry needling provide more effective, direct care for your injury by specifically helping reduce muscle and connective tissue (fascia) tension to decrease pain, increase circulation, and support healing. Sessions are tailored to the patient’s condition, and typically involve a mix of dry needling, standard acupuncture techniques, and complementary modalities (cupping, GuaSha, electric stimulation, infrared, and more). FoRM Health acupuncturists in Portland, Oregon have experience treating high level athletes with dry needling, including professional acrobats, USA track and field runners, and even multiple Olympic medalists. You definitely don’t have to be a professional, or even call yourself an athlete, but we can help you feel like one! Call us today or schedule online to get started. What Conditions is Dry Needling used for? Muscle spasms and cramps Tension or strain headaches Sprains and strains Low back and spinal related pain Tennis elbow Achilles tendonitis Plantar fasciitis Gluteal tendinopathy Shin splints FAQ What is the difference between acupuncture and dry needling? In a broad sense, acupuncture and dry needling are essentially the same; both techniques utilize small needles that are inserted through the skin. Beyond that, however, there is typically a difference in both the decision as to where to apply the needles and how they are used or manipulated. While some acupuncturists essentially use a style basically identical to dry needling, for many patients the experience can be quite different. Acupuncture is based on a 2000+ year old system of Chinese philosophy which at a very basic level looks at the human body as essentially an energetic system existing in a state of fluctuating balance/imbalance. Meridians, or channels, in the body related to an organ network (similar, but not synonymous with anatomic organs) help identify sites of blockage or imbalance where needles can be inserted. Systemic imbalances, again related to the organs and the overall state of the person, are further treated with medications, herbs, food/nutrient choices etc that aim to restore the imbalance. This Chinese Medicine view is complex, needing extensive training in the philosophy to fully understand it, or at least its application from a health standpoint. Attempts to prove or understand it from a Western medical or anatomical view are difficult as basically it is like speaking two different languages or religions, i.e. intermixing French and Japanese when speaking, or trying to explain Christianity through Buddhist terms. Acupuncture applied from within its own context can be very effective for a variety of conditions, as proven both in studies and by its presence in medical care for over 2 millennia. Dry needling, in contrast, is based on a strict Western anatomical basis. Regions of tight, constricted muscles are identified. Needles are inserted into (often) predictable sites of the muscles, usually motor or trigger points, and manipulated until a twitch in the muscles is elicited. This twitch is basically a quick contraction of the muscle. The twitch is essential; when a muscle contracts it typically then causes a reflexive relaxation of that muscle, thereby calming or re-setting the tone of the muscle. Interestingly, and where the debate often comes in across different practitioners, is that acupuncture points often coincide or overlap with the points used in dry needling. This is of no surprise, however, and helps prove both therapies when you consider that two different viewpoints or philosophies end up with the same therapeutic site. Does acupuncture or dry needling hurt? Do I have to rest or limit activity after a dry needling treatment? Do you only treat or see patients from Portland, Oregon?

If you have suffered the sharp, burning intensity of persistent nerve pain, you are aware of the equal frustration of finding a treatment to actually make the pain go away. The physicians at FoRM Health in Portland, Oregon may have an answer for you using advanced ultrasound imaging to get a needle very close to the affected nerves. They have used this relatively new technique to successfully treat common conditions such as carpal tunnel to more hard to identify causes of pain and sensitivity. What conditions can Nerve Hydrodissection be effective for? Carpal tunnel Tarsal tunnel syndrome Cubital tunnel syndrome Sciatica, when related to entrapment at the piriformis or gluteal region Meralgia Paresthetica (lateral femoral cutaneous nerve entrapment) Peroneal nerve entrapment, foot drop Pain that mimics joint or tendon problems without (or with) specific damage of these structures (for example, “tennis elbow” from radial nerve entrapment) Regions of skin hypersensitivity and persistent, burning pain How does nerve hydro-dissection actually work? Certain nerves are prone to getting compressed or entrapped, especially between tissue layers affected by scarring, tension and chronic injuries. The nerves can get stretched, irritated, and then inflamed, causing persistent pain and hypersensitivity. Using ultrasound, your doctor can visualize the swollen nerves and then get a small needle very close, injecting fluid around the nerve to relieve pressure and adhesions. The result? Pain relief, often without the use of any anesthetic at all. FAQ What is actually injected during nerve hydrodissection? Typically just dextrose and water is injected. No anesthetic, no steroid, no other drugs. We are relying on the fluid pressure to release the nerve constriction. Additionally, dextrose has a calming or analgesic (pain relieving) effect on nerves due it binding a specific receptor present when a nerve is constricted and inflamed. Often, we even don’t use anesthetic beforehand as the needles are quite small, plus we like to see immediate pain relief from the hydrodissection, without it being covered up by an anesthetic. Why is it called dissection? Doesn't that actually hurt the nerves? How many treatments of hydrodissection are needed? Am I sore afterwards? When can I resume activity? Do you only treat or see patients from Portland, Oregon?

Dr. Paul Molina is a magician! I am a competitive Olympic Weightlifter, I have been seeing him for a year and I would not be able to compete without him. When I first went to him I had seen a good amount of PT's and other Chiros, none of which could offer me relief, let alone an action plan. I was in a lot of pain when I started, Dr Paul is so knowledgeable he knew exactly what to do. I continue to see him about once a month because I know how valuable it is to maintain good practices and he does that for me. I honestly cannot rave about him enough!!
