Lake District Wellness Center is a team of service professionals partnering with the community, families, and individuals to improve the mental and behavioral health of Lake County citizens. We perform the Early Childhood Service Intensity Instrument (ECSII), Child and Adolescent Service Intensity Instrument (CASII), and the Level of Care Utilization System (LOCUS). We offer courses in dialectical behavior therapy (DBT). We offer psychiatric medication services for children, adolescents, and adults through telemedicine. Andrew Vilius, a registered nurse with a Master of Science degree and a psychiatric mental health nurse practitioner, is our prescriber. In addition to telemedicine, he does on-site visits four times a year. We provide moral reconation therapy (MRT), a batters intervention program, and sexual offender treatment through referrals from Lake County Community Justice. Our Services Long-term Care At Lakeview Gardens, we are implementing our philosophy in person-centered care by building two brand new small home cottages for long-term care and one for our assisted living facility. Each home will consist of 12 private bedrooms, each equipped with its own European-style bathroom (shower included). All residents are encouraged to decorate their rooms to their personal preference, but must get approval from administration. Adding this element of individualization is the wave of the future for long-term care facilities and our elders. Lakeview Gardens is proud to be one of the few facilities in Oregon adopting this concept. Meals In our small home community, meals are prepared in the home, which allows our residents an opportunity to assist with cooking meals, baking snacks, and setting the table just like they would in their own homes. Meal hours will be flexible and allow residents to eat when they would like. Laundry Each home will be equipped with high-end washers and driers that will allow our staff do all residents’ personal laundry within the home. Visitors Within the long-term care facility, we will have 24-hour visitation hours. Since this is our residents’ home, visitors will be required to ring the doorbell. Our attentive staff will allow entry to visitors upon our residents’ request. We will have a sleeper sofa located in each home’s den for out-of-town visitors for brief stays or for hospice residents’ families. Individualized care planning At Lakeview Gardens, we understand that each person is different in many ways. This is why we have implemented individualized care planning. Stemming off of our person-centered care philosophy, individual care planning means upon admission, our staff ask specific questions about each resident’s life to create the best routine for every new resident. This can be changed as often as needed in order to create the best quality of life for each person at Lakeview Gardens. Activities We pride ourselves in offering an abundance of activities that allow our residents to keep active and to maintain or improve their quality of life. We provide activities such as bingo, gardening, visiting pets, arts and crafts, holiday festivities, and much more. Assisted Living Lakeview Gardens Assisted Living’s approach to Assisted Living is founded on the philosophy of the small home concept. You can depend on our compassionate team for excellent care and individualized services. Dining When you move into Lakeview Gardens Assisted Living, you have the freedom to choose your dining experience. Open Monday – Friday, 7 a.m. – 5 p.m. Based on provider availability, call 541-947-3366 for information. Warner Mountain Medical Clinic patients can also be seen at the walk-in clinic when a same-day appointment with a WMMC provider isn’t available. Lake District Hospital Emergency Care The emergency care department is open 24-hours a day, 7 days a week and treats serious illness and injuries including: Deep cuts or broken, displaced bones Heart attack symptoms – chest pain, pain in jaw or arm, sudden weakness, dizziness or sweating Stroke symptoms – sudden numbness, slurred speech, severe headache or weakness on one side of the body or face Severe breathing problems Sudden confusion or disorientation Visits to emergency care can be expensive and should not be used to bypass regular primary care visits to your provider. When to call 911 Ambulance crews assess and begin treatment at the scene and continue on their way to the ER. 911 calls are for critical situations including: Gunshot or other violence Heart attack symptoms – chest pain, pain in jaw or arm, sudden weakness, dizziness or sweating Stroke symptoms – sudden numbness, slurred speech, severe headache or weakness on one side of the body or face Labored breathing or no breathing Severe injury that should not be moved Sudden collapse without getting better Unconsciousness Utilizing an ambulance service can also be expensive and should only be used in a critical emergency. When you call, be prepared to give location, type of emergency, description of injuries or illness and description of patient.
