Regenerative Medicine therapies are performed in our clinic as an outpatient procedure. Regenerative therapies involve the use of products such as NS-Pure, Vitti-Pure, and WJ-Pure. Regenerative Medicine Q&A What is Regenerative Medicine? Regenerative Medicine therapies involve an injection of concentrated growth factors from placental or amniotic tissue, or platelet-rich plasma to regenerate and heal tissues in joints, muscles, tendons or ligaments. Dr. Haynes uses these therapies to treat acute or chronic pain conditions related to degenerative disease. What can Placental Therapy and Platelet Rich Plasma treat?
Chronic Joint pain like Osteoarthritis. This type of arthritis is common in feet and patients will often complain of joint stiffness, throbbing, and swelling. There is no cure for osteoarthritis, but regenerative medicine therapies such as Platelet Rich Plasma (PRP) and bone marrow derived stem cell therapy are a new generation of treatments that have been proven to be safe, effective, and have lasting results. What can Stem Cell Therapy and Platelet Rich Plasma treat? Chronic Joint pain like Osteoarthritis. This type of arthritis is common in feet and patients will often complain of joint stiffness, throbbing, and swelling. There is no cure for osteoarthritis, but regenerative medicine therapies such as Platelet Rich Plasma (PRP) and bone marrow derived stem cell therapy are a new generation of treatments that have been proven to be safe, effective, and have lasting results. Where Do Platelet Rich Plasma & Placental Cells Come From? Placental Cells (NS-Pure, Vitti-Pure, and WJ-Pure) NS-Pure We use NS-Pure which is structural tissue harvested from human placental tissue that is minimally manipulated and manufactured under GTP, cGMP, AATB, and FDA HCT/P 361 guidelines. It is DMSO free and is absent of any harmful enzymes and chemicals. This structural tissue is used for reconstruction, repair, and replacement. It uses the original relevant characteristics of structural tissues such as support, strength, flexibility, cushioning, covering, and compressibility. Structural tissues may contain both extracellular matrix and cellular components because any alteration of these components that relates to the the structural tissue’s utility for reconstruction, repair, or replacement would be considered more than minimal manipulation. NS-Pure is used for via injection for joint pain, plantar fasciitis, ligament and tendon pain, as well as for wound healing. WJ-Pure WJ-Pure is also an umbilical cord product that contains Wharton’s Jelly. This a thick substance that is natural from the cord and is very viscous and thick. WJ-Pure works great in the setting of joint pain. Vitti-Pure Similar to NS-Pure, Vitti-Pure is an umbilical cord product. Manufactured under the same standards. Vitti-Pure meets all FDA 361 guidelines. Vitti-pure is more like a PRP injection and can be used for the injections of tendons, ligaments, joints, and other painful conditions. PLATELET RICH PLASMA
When your body is injured, its first natural response is to flood the area with growth factors that promote healing. These growth factors are released by platelets. They work to stimulate the healing process. Platelet rich plasma (PRP) therapy optimizes the body’s healing process by placing a high concentration of platelets directly into the injured area. To harvest PRP, a small sample of your blood is drawn into a specialized collecting system. This blood is centrifuged at high speeds to isolate the growth factors and platelets. Because your own blood is used, there is no risk of a transmissible infection, and a low risk of allergic reaction. WHERE DO THEY COME FROM? A high concentration of growth factors are found in placental tissues. These factors are derived from tissues acquired from living, healthy donors after full term pregnancy and a scheduled Caesarean section. These tissues are minimally processed and preserved to maintain the natural properties of the placental tissues. Stem cells taken from allograft tissues have the ability to be transferred into another human being. Regenerative Therapy can reduce the need for major surgery, & has a quick healing process. No surgery means no hospital stay, no prolonged recovery, and regeneration happens over the next 2-6 weeks and can continue for up to 6 months. How does Placental Therapy and PRP work? 1. Placental cells The products NS-Pure, Vitti-Pure, and WJ-Pure are ordered and stored frozen. They are then injected at the time of the visit. They provide growth factors that aid in repair, reconstruction, and replacement of damaged tissue. They actually heal the damaged tissue. 2. PRP Cells – Isolation of cells or platelets – A blood draw is performed from your arm. – A high concentration of cells or platelets are separated by being centrifuged at high speeds with a specialized system

Elite Foot and Ankle is the only podiatry group in Oregon using cutting edge MLS laser therapy for pain and inflammation. MLS is an FDA approved Class IV laser using concentrated light energy to stimulate the body’s own healing process. Everyone’s pain is different. Drs. Haynes and Belnap of Elite Foot & Ankle Associates will diagnose and treat your unique pain – whether it’s in the heel, arches, ball of your foot and/or toes – getting you back to your daily routine with comfort and ease! Class IV laser therapy is one of the most safe and effective ways to treat various conditions that cause foot and ankle pain! Laser therapy has a vast amount of benefits including: Accelerated cellular reproduction and growth – which is key in repairing the cells of cartilage, bone, tendons, ligaments, and muscle tissue. Reduced scar tissue that typically develops from specific injuries. When patients have less scar tissue, the likelihood of chronic pain is also reduced. Decreased inflammation and increased blood circulation. Understanding Laser Therapy for Pain Management With 26 bones in the foot alone, as well as major ligaments, tendons, joints, and nerves – it’s not surprising that millions of Americans suffer various types of foot and ankle pain. Class IV laser therapy is used for the relief of pain, to accelerate healing and decrease inflammation. Laser therapy is great during the post operative recovery period to help speed recovery and decrease the swelling and pain associated with surgery. When the light source is placed near or against the skin, photons penetrate several centimeters down and are absorbed by the mitochondria (the energy producing part of a cell). This energy results in the restoration of normal cell function. The main goal of laser therapy is to stimulate the cell to perform its natural functions, but at an enhanced rate. Laser therapy is a quick and easy treatment allowing patients to relax during the procedure. Our lasers are painless, non-invasive, and there are no side-effects. Laser treatment has a 97% satisfaction rate according to a large study done by an orthopedic group performing laser on all types of musculoskeletal pathologies. Some of the most common source of foot pain include: Plantar fasciitis – which often creates moderate to severe heel pain Tendonitis including Achilles tendonitis Posterior tendonitis (or PTTD) Peroneal tendonitis (tendonitis affecting the midfoot and arch) Ankle sprains Neuroma (or a pinched nerve) Arthritis and inflammation Bunions Hallux limitus (or a stiff big toe joint) Laser treatment typically requires around 6 -12 treatments. Treatments are twice a week. Any one of these conditions can create chronic (long-term) pain or acute (short-term) pain. To schedule an appointment for laser therapy for your pain. Elite Foot and Ankle is the only podiatry group in Oregon using cutting edge MLS laser therapy for pain and inflammation. MLS is an FDA approved Class IV laser using concentrated light energy to stimulate the body’s own healing process. More than 20 million people in the United States are estimated to have Peripheral neuropathy (PN). Peripheral neuropathy occurs when nerves are damaged or destroyed by disease or injury. Injured nerves either no longer communicate impulses, much like a broken wire, or they signal inappropriately, like static on a telephone line. Other times, signals can be distorted like a wavy television picture. Neuropathy can also impair the function of small blood vessels in the extremities. Often the furthest nerve endings in the feet are affected first, or are the most seriously affected. Symptoms can be brought on by diseases like diabetes, or as a result of viral infection or chemotherapy, and include tingling, numbness, and shooting pains in the feet and legs, which can also be worse at night. When managed properly, some people may be able to reduce their medication dosages or control their neuropathy without medications. Diligent treatment can permit some recovery of the nerve function, as long as the nerve cells have not died. Nerve health can be improved by a commitment to healthy lifestyle habits, like maintaining optimal weight, avoiding toxins, eating a balanced diet, and correcting vitamin deficiencies. It is particularly important to stop smoking because it constricts the blood vessels that supply nutrients to the peripheral nerves which can worsen neuropathic symptoms. An exercise program can improve blood flow, bringing oxygen and nutrients to the furthest nerve endings, improving muscle strength and protecting against muscle atrophy. With dedicated self-care, it is possible to manage symptoms and oftentimes to create conditions that encourage nerve regeneration. Strict control of blood glucose levels may reduce neuropathic symptoms and help people with diabetic neuropathy avoid further nerve damage. At Elite we recommend a regimen of our EBM vitamins to correct for vitamin deficiencies as well as for promoting nerve health, coupled with laser therapy. Lasers can simultaneously reduce inflammation and act as an analgesic to ease pain. A recent study by Dr. Andrew Rader, DPM, has shown that laser therapy applied to leg nerves in persons with documented diabetic peripheral neuropathy leads to objective improvement in the nerve function, demonstrated by NcS evaluation. This improvement can lead to improved stability in the feet, which interrupts the causal pathway leading to ulceration, infection and amputation. There are 10 benefits to laser therapy Anti-inflammatory Laser therapy causes vasodilation and activates the lymphatic drainage system resulting in a reduction in swelling caused by bruising and inflammation Analgesic pecific synchronized wavelengths of laser block pain signals transmitted to the brain, decreasing nerve sensitivity. Due to decreased inflammation, there is less edema and pain. In addition, high levels of pain killing chemicals (endorphins and enkephalin) from the brain and adrenal glands are activated by the laser light, blocking pain signals. Accelerated Tissue Repair Photons of laser light penetrate deeply into tissue and accelerate cellular reproduction and growth. Laser light increases energy available to the cell so that cells can take on nutrients faster and get rid of waste products. As a result of exposure to laser light, damaged cells are repaired faster. Improved vascular Activity Laser significantly increases the formation of new capillaries in damaged tissue, speeding the healing process, closing wounds quickly and reducing scar tissue. In addition, the acceleration of angiogenesis, causes temporary vasodilation and increases the diameter of blood vessels, improving blood flow. Increases Metabolic Activity Laser therapy creates higher outputs of specific enzymes, greater oxygen and food particles loads for blood cells. Trigger and Acupuncture Points Laser light non-invasively stimulates muscle trigger and acupuncture points providing musculoskeletal pain relief. Reduced Fibrous Tissue Formation Laser reduces the formation of scar tissue from cuts, burns, or surgery. Improved Nerve Function Slow recovery of nerve function in damaged tissue can result in numbness and impaired limbs. Laser speeds nerve cell reconnection and increases the amplitude of action potentials to optimize muscle healing. Immunoregulation Laser emissions have a direct effect on immunity by stimulating immunoglobulins and lymphocytes. Laser emissions are absorbed by chromophores (molecule enzymes) that react to laser light. The flavomononucleotide enzyme is activated and starts the production of ATP (adenosine-triphosphate), which is the major carrier of cell energy and the energy source for all chemical reactions in the cells.

Are Bunions Limiting Your Activities and Lifestyle? Bunions can be very painful. With each step, your entire body weight rests on that bunion. Ouch! A common misconception about a bunion is that it is an overgrowth of bone that can simply be “shaved off”. In reality, bunions are complex deformities caused by an unstable joint. Lapiplasty® 3D Bunion Correction Relief is here. Now, there’s a new, patented treatment for bunion correction – Lapiplasty® 3D Bunion Correction. Dr. Haynes at Elite Foot & Ankle Associates is offering this exciting, new treatment option which does more than simply remove “the bump”; it addresses your bunion in three dimensions to correct the root of the problem. Advanced fixation technology is used to secure the correction in place, allowing patients to walk within days of surgery. > Why Was Lapiplasty® 3D Bunion Correction Developed? For 87% of people with bunions, traditional surgery has failed to get to the true source of the problem – a 3-dimensional deformity caused by an unstable joint. This is why many people who have had traditional surgery have had their bunions return. In fact, 1 in 3 patients have been left dissatisfied with their surgical outcome in the past and up to 70% of those treated have had their bunion return. Traditional Surgery Is A 2D Solution For A 3D Problem Traditional bunion surgery has been limited to an incomplete, two-dimensional (2D) approach to correction; cutting the bone in half and pushing the top part over to reduce the appearance of the bump. This 2D “cut and shift” approach primarily addresses the bunion’s symptoms, not the root cause – the unstable foundational joint – leaving the bunion significantly more likely to come back to some degree. Traditional 2D Surgery Like trying to fix The Leaning Tower of Pisa by cutting it and shifting it. How Does Lapiplasty® 3D Bunion Correction™ Work? Instead of cutting the bone in half and shifting the top part over, the patented Lapiplasty® Procedure uses specially-designed instrumentation to rotate the entire deviated bone back into its normal anatomical position, naturally straightening your toe; removing the “bump” and associated pain. Innovative titanium plating technology, permanently secures the unstable foundation, the root of your bunion problem. Most patients are able to walk within days of surgery. No cast is required; allowing patients to get back to the shoe wear of their choice and activities quickly. Lapiplasty® 3D Bunion Correction™ Like addressing the unstable foundation (root cause) of The Leaning Tower of Pisa by rotating it back into position and permanently securing it. What Does A Correction Look Like On X-Ray? Normal Foot Traditional 2D Surgery “Cut & Shift” approach leaves an unstable joint Lapiplasty® 3D Bunion Correction™ Alignment is restored & unstable joint is permanently secured Lapiplasty® Patented Titanium Plating Technology — Enabling Early Weight Bearing The Lapiplasty® Procedure utilizes two low-profile (petite), anatomically-shaped titanium plates to permanently secure the unstable foundation of your corrected bunion. This provides buttressing on multiple planes and allows you to walk within days of surgery.1 Think of this in terms of how a basketball player might hold a basketball. When a player tightly grips the ball with both hands, the ball is held more securely than if only one hand is used. Unlike other surgical approaches, the Lapiplasty® Procedure utilizes two plates for this very reason – “Two plates are better than one”. What Should I Expect In Terms Of Recovery? The patented titanium plating technology used with the Lapiplasty® Procedure enables patients to bear weight on their foot early.1 The ability to do so can dramatically improve your recovery process, allowing you to walk up to 6-8 weeks earlier than with the more traditional Lapidus approach (which was previously the only option for addressing the unstable joint). Being able to walk in a surgical boot, as opposed to being completely non-weight bearing (in a cast with scooter or crutches) for 6-8 weeks, means that you will be getting back to a better quality of life faster; may return to most occupations more quickly; and will be less inconvenienced by bunion surgery.

Radial Shock Wave Therapy Q & A What is Radial Shockwave Therapy? Radial Pulse Shockwave Therapy is an FDA cleared technology proven to increase the rate of healing for soft tissue tendinopathy. It is the most advanced non-invasive and highly effective treatment method that enhances blood circulation and accelerates the healing process causing damaged tissue to gradually regenerate. What conditions can be treated with RSWT? Plantar Fasciitis Achilles Tendinitis Shin Splints Wound Care How does RSWT work? When you experience chronic pain, your body no longer recognizes that there is an injury to that area. As a result, it shuts down the healing process and you feel no relief. The ballistic sound waves of the Enpuls penetrate deep through your soft tissue, causing a microtrauma or new inflammatory condition to the treated area. Once this occurs, it then triggers your body’s natural healing response once again. The energy emitted also causes the cells in the soft tissue to release certain bio-chemicals that intensify the body’s natural healing process. These bio-chemicals allow for the building of an array of new microscopic blood vessels in the soft tissue. What do studies show? When you experience chronic pain, your body no longer recognizes that there is an injury to that area. As a result, it shuts down the healing process and you feel no relief. The ballistic sound waves of the Enpuls penetrate deep through your soft tissue, causing a microtrauma or new inflammatory condition to the treated area. The energy emitted also causes the cells in the soft tissue to release certain bio-chemicals that intensify the body’s natural healing process. These bio-chemicals allow for the building of an array of new microscopic blood vessels in the soft tissue. There have been extensive studies done on Low Energy Shockwave Therapy over the past 20 years. These studies show between a 72% – 80% success rate. How is RSWT performed? The system is a machine with an applicator that looks like an ultrasound. It is administered once a week for a series of 4 to 5 treatments. Each treatment takes approximately 5 minutes depending on the size and number of areas treated. All treatments are performed by experienced professionals. The Doctor applies the applicator to the area and administers the ballistic waves by moving over the affected area in a very slow, circular motion. During the treatment, there may be some minor discomfort. Painful areas are helpful in isolating the truly injured areas. Why consider Radial Shockwave Therapy? RSWT has a proven success rate that is equal to or greater than that of traditional treatment methods, including surgery, without the risks, complications, and lengthy recovery periods. There are no incisions and no risk of infection, or scar tissue, like with surgery. There is no need for anesthesia to be administered during the treatment, as opposed to high-energy shockwave therapy. Patients treated with RSWT may be active immediately and resume their normal routine the same day. Many patients find a significant and immediate reduction in their pain. Some patients will feel results gradually after each treatment. Why RSWT instead of Physical Therapy? Physical therapy is 2 to 3 times a week, for 6 weeks and you are at the facility for no less than an hour. More importantly, there is usually a co-pay required for every visit which could be $40 or more. Over a six week period or 15 visits, this can add up to $600 in out of pocket co-pays. The RSWT treatments are only once a week, for 5 minutes each. This is a highly effective modality which is faster and more effective than Physical therapy, and the cost of the RSWT treatments are usually LESS than the total of your PT co-pays. If you want fast results in less time, and would like to save money, the RSWT treatment is a better choice. What are the possible side effects? There have been very few side effects reported. In rare cases, skin bruising may occur. Patients may also feel soreness in the area for a day or two afterward, similar to a strenuous workout. Will I be in pain afterward? A day or two after the treatment you may feel a slight discomfort like a bruise, but that is normal and is a sign the treatment is working. Is it covered by my Insurance? No. RSWT is not covered by insurance plans, but ask your Doctor how they can make it affordable for you.

You no longer need to feel embarrassed to wear open-toe shoes and sandals – or just go barefoot – because of toenail fungus or discoloration. Elite Foot & Ankle Associates is the area’s leader in restoring nails back to health, providing a natural and luminous appearance. We use an FDA approved Class IV laser, that is painless with no downtime or recovery time, and no side effects. Class IV laser therapy is one of the most safe and effective ways to treat toenail fungus (onychomycosis)! Laser therapy penetrates the nail bed and kills the fungus living underneath the nail – without pain and without any harmful side effects. When the colony of fungus under the nail is “attacked” by the laser, the elements that make it grow and thrive are destroyed. This allows your nail bed to begin growing new, healthy nail. Nails that won’t give you feelings of discomfort or embarrassment! Laser therapy is a quick and easy treatment allowing patients to relax during the procedure. What is Onychomycosis? Fungal foot infections. They can create major discomfort and even embarrassment. They are also EXTREMELY COMMON; affecting millions of people each year. Additionally, fungus is easily spread. Individuals with fungus may have contracted it through shared bathroom space, public locker rooms, showers, and pools, shared clothing or shoes, even carpeting. For some, fungus is simply genetic. When this is the case, treatment can become even more difficult given the likelihood of reoccurrence. One of the most prevalent types of fungal foot infections directly affects your toenails. This type of condition is known as onychomycosis. You may have onychomycosis if you notice any of the following symptoms: • yellow or other discoloration of your toenails • thickened or brittle toenails • an unpleasant odor coming from the nail area • pain It’s not uncommon to feel discomfort and embarrassment due to the unsightly appearance and these symptoms of onychomycosis. Fungal nail affects nearly 10% of the US population. While many find the condition to be, simply, an eye sore, fungal nail can also lead to a larger problems incuding: • infection of other toenails • infecting friends and family • increased pain • ingrown toenails Individuals with diabetes are also more susceptible to fungal nail given their compromised immune systems. Elite Foot & Ankle’s Laser Nail Treatment includes: Treatment for nail fungus done with six laser treatments done every two weeks for three months. During the three months of laser we recommend application of a topical antifungal cream or solution daily. We also recommend an antifungal shoe spray to be applied to the shoes daily. Oral Terbanifine when indicated. This method has about an 80% effective rate all without having to take medication. Dr. Haynes or Dr. Belnap will follow up with you in 8 months following your last treatment. While we cannot guarantee your treatment (treating nail fungus is difficult, and in some patients it cannot be eradicated), if you still have some residual or lack of improvement, we will provide four additional treatments at no cost to you.
