After the incision is made, the nipple (which remains tethered to its original blood and nerve supply) is then repositioned. The areola is reduced by excising skin at the perimeter, if necessary. Underlying breast tissue is reduced, lifted and shaped. Occasionally, for extremely large pendulous breasts, the nipple and areola may need to be removed and transplanted to a higher position on the breast (free nipple graft). The incisions are brought together to reshape the now smaller breast. Sutures are layered deep within the breast tissue to create and support the newly shaped breasts; sutures, skin adhesives and/or surgical tape close the skin. What is fat transfer breast augmentation? Fat transfer breast augmentation essentially uses liposuction to take fat from other parts of your body and inject it into your breasts. This is a breast augmentation option for women who are looking for a relatively small increase in breast size and would prefer natural results. How does fat grafting to breasts work? During your fat grafting procedure, Dr. Brou will harvest fat from areas where you have excess such as the abdomen, thighs, upper back (aka “bra rolls”) or arms. The right spots are going to depend on each patient’s individual needs. By removing fat from certain areas such as the abdomen and placing it into the breasts, Dr. Brou can restore harmony to your body, accentuate your waist, and re-contour your torso. The end result is typically a narrower waist, shapelier breasts, and a sexier figure. When you come in for a fat grafting consultation, Dr. Brou will examine you from a 360-degree perspective, determining the best places for harvesting fat. It is not a one-size-fits-all approach. How much down time should I expect with fat grafting surgery? Most patients can expect about a week of downtime after fat grafting. You will be able to get up and move around freely, but you will have swelling and some soreness. We will give you a compression garment to wear for the first month to help with the swelling. The swelling will take several weeks to completely go away. This surgery is outpatient and does not require an overnight stay. What can I expect as a result of fat grafting surgery? Just as every person’s body contour is unique, results of your fat grafting procedure will also vary. It is important to discuss all of your concerns and goals with Dr. Brou. During your consultation, Dr. Brou will advise you of likely outcomes for your appearance. It may take weeks or months for post-fat grafting swelling to completely subside. As swelling dissipates, your new breast contour will begin to appear and develop. It is important to maintain a healthy consistent weight after your surgery in order to retain your results. It is important to note that most breast fat grafting procedures will require additional procedures to achieve your desired breast size. Are there any risks associated with fat grafting surgery? Fat grafting is a surgical procedure, and therefore risks are always involved as a result of receiving anesthesia as well as the procedure itself. Dr. Brou can help assess your unique health needs and advise you of any concerns to be aware of prior to undergoing surgery. Potential risks associated with fat grafting include pain, swelling, irregular contours and loose skin, skin or nerve damage, infection, blood clots, fluid accumulation, scarring, bleeding or asymmetry, although rare.

What is a Neuromodulator? Neuromodulators such as Botox, Dysport, Jeuveau and Daxxify are used to treat facial wrinkles in areas like the forehead, between the eyes, and around the eyes known as ‘crows feet’. These substances weaken the muscles responsible for facial expressions. This results in the relaxation of the muscles’ pull and flattening of the overlying skin, which in turn reduces wrinkles in a natural way. This method differs from dermal fillers that fill in wrinkles or depressions. How Long Does it Last? Injection of this material into the small muscles between the brows, in the forehead, and at the corners of the eyes causes those specific muscles to halt their function (be paralyzed), thereby improving the appearance of the wrinkles. This paralysis is temporary, and lasts about three to four months. How is it Used? The expert performing the treatment injects small amounts of the substance into the targeted muscles using a very tiny needle. They may require 5-15 (or more) needle pricks, depending on the areas being treated. However, discomfort is minimal. Side Effects The injection may puncture a small blood vessel and cause bruising, which can last for 5-10 days and may require makeup to cover. To decrease the risk of bruising, you should discontinue aspirin, ibuprofen or other blood thinning substances three weeks prior to the treatment. If the botulinum toxin reaches a facial muscle that is not intended for treatment, it may cause partial weakness in that muscle for several days to weeks. To prevent this, you should avoid massaging the injected area for 12 hours following the injection. If an untreated area experiences muscle weakness, the only solution is to wait for the effect of the toxin to wear off. There are no recognized permanent side effects of neuromodulators. Though rare, complications may include paralysis of a nearby muscle resulting in its temporary loss of function, such as a drooping eyelid. Botulinum toxin has been safely used in the treatment of muscular facial spasms for many years. Neuromodulators in Combination with Facial Surgery Neuromodulators injected in conjunction with nearby facial surgery has caused unwanted facial weakness in at least one patient. For this reason, we will inject neuromodulators only as an isolated facial procedure or treatment. Note The determination of specific risks and the appropriateness of this procedure for a particular individual can only be made during the consultation. All procedures carry some level of risk, with occasional occurrence of minor complications that do not impact the final result. Major complications are rare.

What are dermal fillers? Dermal fillers are gel-like substances that are injected beneath the skin to restore volume loss, smooth lines and wrinkles, plump and hydrate lips, and soften creases. Dermal fillers can also enhance facial contours like the chin and jawline. This can be a cost-effective way to look younger without surgery or downtime. What fillers do you offer at Premier? Most commonly, we use Hyaluronic Acid (HA) fillers. Hyaluronic Acid is a naturally occurring substance that is already found in our bodies. HA helps keep the skin hydrated and plump. This type of filler is soft and gel-like and most HA-fillers are combined with Lidocaine to help minimize pain during and after the treatment. Hyaluronic Acid filler results typically last 6-18 months. All HA fillers can easily be dissolved and are considered the safest of fillers for this reason. Your provider may suggest you take a medication a few days prior to prevent a breakout. What do I need to do after my injections? No exercise for at leasts 24 hours after treatment. Avoid alcohol and foods high in sodium for approximately 48 hours after treatment. Do not pick, massage or rub the area treated. Avoid applying makeup over injection sites for 8-12 hours post treatment. Arnica cream may be applied to minimize inflammation and bruising. Ice periodically (20 minutes on, 20 minutes off) throughout the day following your injections. This will help minimize bruising, discomfort and swelling.

What is PRP? Platelet Rich Plasma, or also known as “PRP” is an injection or topical treatment whereby a person’s own blood is used. A small amount of blood (15cc-50cc) is drawn up from the individual patient into a syringe. The blood is spun down in a special centrifuge to separate its components (Red Blood Cells, Platelet Rich Plasma, and Plasma. The platelet rich plasma is first separated then activated with a small amount of calcium to allow the release of growth factors from the platelets which in turn amplifies the healing process. PRP is then injected into the area to be treated. Platelets are very small cells in your blood that are involved in the clotting process. When PRP is injected into the damaged area it causes mild inflammation that triggers the healing cascade. As the platelets organize in the clot, they release several enzymes to promote healing and tissue responses including attracting stem cells to repair the damaged area. As a result, new collagen begins to develop. As the collagen matures it begins to shrink causing the tightening and strengthening of the damaged area. When treating injured or sun and time damaged tissue they can induce a remodeling of the tissue to a healthier and younger state. What are the benefits of PRP? Along with the benefit of using your own tissue therefore eliminating allergies there is the added intrigue of mobilizing your own stem cells for your benefit. PRP has been shown to have overall rejuvenating effects on the skin as in improving skin texture, fine lines and wrinkles, increasing volume via the increased production of collagen and elastin, and by diminishing and improving the appearance of scars.
