- SWSharlene WilburnJul 3, 20263.0As we were about to leave, I was asked to sign a printout called a “Super Bill” where the worker drew an X and a line for me to sign. When asked what it was for she mentioned something about possibly charges the insurance may not pay for. I stated that should’ve been discussed prior to asking me to sign something. When asked for a copy, I was refused several times and wouldn’t show me this “Super Bill” again so I could take a picture of it. I stated “so basically you’re refusing me a copy of something you asked me to sign related to my services outside of my insurance?” On a follow up call from office, I was told I can now get a copy of this “Super Bill” and it would be emailed with diabetes form. Then that changed to I can drive all the way back out there to get a copy of what I was just denied several times in the office. Never have I ever had this happen anywhere else for an eye exam. Up until then, the eye exams were very pleasant and professional.
