Buckeye PACE Ashtabula provides coordinated health care and support for eligible adults age 55 and older who want to remain safely in their homes and communities. PACE stands for Program of All-Inclusive Care for the Elderly. Our interdisciplinary team coordinates medical care, medications, therapy, transportation, home and personal care, nutrition, social services, and activities through one personalized care plan. Participants can also receive services, meals, activities, and social support at our Ashtabula adult day health center. We help older adults, families, and caregivers explore long-term care options and determine whether Buckeye PACE may be a good fit.
