Indications for Tummy Tuck Changes in the abdominal wall occur as a result of pregnancy, weight loss, and aging. The amount of skin and fat and the weakness and separation of the abdominal muscles, directs the extent of surgery necessary. Most often a full abdominoplasty is needed, however we do tailor the operative procedure to the specific needs of our patients to achieve a flat abdomen. Abdominoplasty surgery is part of the “mommy makeover” trend and for good reason! Often times during pregnancy, the abdominal muscles stretch and no amount of crunches will return them to their pre-pregnancy position. Part of the abdominoplasty procedure helps to decrease the separation of these muscles helping to get your pre-baby shape back. Tummy Tuck (Abdominoplasty) Surgery The abdominoplasty surgery includes removal of excess skin through a lower abdominal wall through an incision, possible liposuction to selected areas, and tightening of rectus muscles. Time 2-4 hrs. Location for Surgery Abdominoplasty (Tummy Tuck) surgery is performed at Ellis hospital conveniently located just outside of Albany, NY. The procedure is done under general anesthesia. It requires a 1-2 day hospital stay. Trade Offs Moderate surgical discomfort, bruising, swelling, numbness of the abdominal wall skin, and a permanent lower abdominal and periumbilical scar is expected. Risks of Tummy Tuck Surgery Although uncommon, infection, bleeding, delayed healing, skin loss, poor scarring, phlebitis and blood clots are complications that can prolong recovery or necessitate a return to surgery. Recovery from Tummy Tuck Surgery After Abdominoplasty (Tummy Tuck) surgery, our patients wear a supporting garment or girdle for 3-4 weeks to encourage good wound healing and minimize complications. Indications for Smartlipo Triplex Laser Assisted Liposuction Smartlipo laser assisted liposuction targets trouble spots like the abdomen, arms, buttocks, thighs, and neck, and permanently eliminates fat. This is especially helpful for exercise resistant areas. Smartlipo Triplex Laser Assisted Liposuction The Smartlipo Triplex laser fiber is inserted through a very small tube into your body, the laser ruptures the fat cell membranes and the fat cells are permanently destroyed. The liquefied fat is then gently suctioned out. Smartlipo Triplex actually tightens the skin through coagulation, so there isn’t as much dimpling and sagging skin as you would get with traditional liposuction. The downtime after Smartlipo Triplex is less than traditional liposuction because the fat is liquefied before it is suctioned out. This is less traumatic on the body. Time Varies, but usually 1-3 hrs. Location for Smartlipo Triplex Laser Assisted Liposuction Smartlipo Triplex laser assisted liposuction is performed with a local anesthetic and/or sedation in our office or a general anesthetic at the hospital. It is often paired with Abdominoplasty surgery (tummy tuck), thigh lift surgery or arm lift surgery. Trade Offs Light bruising and some minor swelling may occur. Much less than with traditional liposuction. Support garments must be worn for 3-4 weeks following the procedure. Risks of Smartlipo Triplex Laser Assisted Liposuction Light bruising and minor swelling may occur. Recovery from Smartlipo Triplex Laser Assisted Liposuction After Smartlipo Triplex laser assisted liposuction, our patients wear a supporting garment or girdle for 3-4 weeks to encourage good wound healing and minimize complications. Indications for Breast Reconstruction Post mastectomy deformity Breast Reconstruction Surgery TRAM Flap, Latissimus Flap +/- implant, Implant/ expander. The operative technique is decided in consultation with the patient and may involve the use of her own tissue repositioned to create a breast mound, or the use of an implant/expander to create a breast mound. Surgery may be performed at the time of the mastectomy or at a later date following completion of any additional required tumor treatment. Several procedures are needed for total breast and nipple areola reconstruction. Surgery on the opposite breast may be considered for symmetry. Location Surgery is performed at the hospital with a general anesthetic. The length of stay varies according to the technique selected for reconstruction. The procedure is performed at either Ellis Hospital, Bellevue Hospital or Albany Medical Center all of which are conveniently located near Albany, NY in the Capital District. Indications for BOTOX® Aging changes around the eyes specifically crows feet and frown lines. Botox fox and schingo BOTOX® Procedure The technique involves injection of Botulina Type A Toxin (Botox®) into the overactive muscles causing a temporary paralysis minimizing the active lines. Improvement lasts 3-6 months. Location Botox is a quick outpatient procedure done in our Schenectady Office conveniently located outside of Albany, NY. For optimum comfort, it is performed with local anesthetic (numbing the area). Trade Offs of BOTOX® Mild, temporary surgical discomfort, bruising and/or swelling. Risks of BOTOX® On a rare occasion there is temporary difficulty opening the eye, but this is extremely rare. Recovery from BOTOX® Patients usually continue with their normal routine following their appointment. Some make up may be required to cover any redness or bruising. Ear Surgery (Otoplasty) The operative correction involves repositioning the cartilage so that the ear sits closer to the head. Time 2-3 hours Location Outpatient, hospital, general anesthesia, a cooperative patient can be treated in the office with local anesthesia and sedation. Trade Offs There is mild temporary surgical discomfort, bruising, swelling, and numbness. Permanent scars are present but are behind the ear. Risks Although uncommon, complications can occur and include bleeding, infection of cartilage that can lead to deformity, asymmetry, and the occasional need for additional surgery if the prominence recurs. Recovery Our patients wear a headband for 3-4 weeks to facilitate good wound healing and minimize complications. We encourage our patients to continue to wear a headband for an additional 6 months as tolerated. Most patients return to work or school 1 week following surgery, and their normal exercise routine at 4-6 weeks.

Indications of Eyelid Surgery (Blepharoplasty) Aging changes around the eyes including sagging upper eyelids and puffy lower lid bags can cause a tired look. Eyelid Surgery (Blepharoplasty) The operative technique includes removal of excess upper lid skin, fat and muscle and either removal or repositioning of the lower lid fat. This operation is frequently combined with a brow lift, C02 laser resurfacing and /or facelift. Time 2-4 hrs. photo eyelift before Location Outpatient, office, local anesthesia and sedation Trade Offs There is mild, temporary surgical discomfort, bruising, and swelling rarely there is temporary difficulty fully closing the eyelids. Scars are carefully placed and are rarely a concern. Additional surgery although rare is sometimes required when there’s a downward shift of lower lids. Extremely rare is risk of blindness. Recovery Our patients are instructed in the use of cool compresses for the first 48 hours to minimize swelling and bruising. Indications for Facelift As we get older gravitational aging changes occur in the mid- face, jowls, and neck. Botox, fillers and Ultherapy may help to add some lift, but they can not eliminate excess skin like a facelift. In 2016 facelifts re-made the top 5 cosmetic surgery procedures in the United States (ASPS 2016) after having dropped in previous years. People are realizing that as great as the non-invasive procedures are, the gold standard in facial rejuvenation is still the facelift. Facelift Surgery A facelift procedure re-positions the soft tissues and muscles of the face, and redrapes the skin to restore a more youthful appearance. Excess skin is often removed leaving a sleeker profile. The operation is frequently combined with blepharoplasty (eye surgery), C02 laser resurfacing, and/or fillers and botox. Time 3-5 hrs. photo facelift before Location Depending on the extent of surgery it may be performed with a local anesthetic and sedation in our office or a general anesthetic at the hospital. Patients may elect to spend a night at the hospital. Trade Offs There is mild, temporary surgical discomfort, bruising, and swelling. Scars are carefully placed and are rarely a concern, in many patients a short scar technique can be utilized. Temporary numbness and tightness of the cheek and neck skin is expected. In men re-positioning of the beard may alter the area that requires shaving. Risks Although uncommon, problems with facelift may include infection, bleeding, minor asymmetries, or injury to the facial nerve resulting in muscle weakness. Additional surgery although rare is sometimes required when there is an area of hair loss, poor healing or excessive scarring. Recovery After their facelift surgery, our patients are instructed in the use of cool compresses for the first 48 hours to minimize swelling and bruising. Most patients return to work 10-14 days following surgery and return to their normal exercise routine at 2-4 weeks. Although the downtime is longer than some other procedures, patients report that it is worth the wait since the results are more significant.

Indications for Laser Surgery, Laser Resurfacing Damaged skin with textural changes, fine lines, sun damage, pigment irregularities, and acne deformities and facial scars. Patients are prepared pre -operatively with an aggressive skin care program and educated in proper skin care by our full time esthetician. Laser Surgery The carbon dioxide (C02) laser is the most effective laser for skin resurfacing. We utilize the Coherent ultra pulse C02 laser. Frequently performed in conjunction with a blepharoplasty, facelift, or brow lift. Time 1-2 hrs. Location Surgery is done in our office which is conveniently located in Schenectady, NY, just outside of Albany. It is performed with local anesthesia and sedation or a general anesthetic at the hospital. Patients return home the same day unless done in combination with the more extensive procedure. Trade Offs Temporary, swelling, sun sensitivity, and milia (small whiteheads) are common. Healing takes 7-10 days with redness persisting 6-12 months camouflaged with make up as necessary. Risks Although complications are rare, infection, scarring, and cold sores can occur. Preventive antibiotics and antivirals are used routinely. Recovery New skin forms in 7-14 days, but full healing and maturation of the skin color requires 6 months. Optimal results are achieved when used in conjunction with a structured skin care program.

Location Outpatient, hospital, general anesthesia, a cooperative patient can be treated in the office with local anesthesia and sedation. Trade Offs There is mild temporary surgical discomfort, bruising around the eyes, swelling, and numbness. Risks Although uncommon, complications can occur and include bleeding, infection, and the occasional need for additional surgery if some deformity persists. Recovery Most patients return to work or school 1-2 weeks following surgery and their normal exercise routine in 2-3 weeks. Final results are permanent and are evident at 12-18 months.

Indications Lipoatrophy: the loss of fat beneath the skin, seen as sunken cheeks, and indentations, and hollow eyes and temples. Surgery Sculptra® is composed of poly-L-lactic acid, a synthetic material. Sculptra® is not made from human or animal sources and does not require a skin test. Poly-L-lactic acid (PLLA) is a biocompatible (does not harm the body), biodegradable (able to be broken down by the body) material that has been widely used for many years in surgical products. Sculptra® is injected below the surface of the skin in the area of fat loss. It provides a gradual and significant increase in skin thickness, improving the appearance of folds and sunken areas. The results from Sculptra® are not immediate, but gradual. The provider will see you again in a few weeks to decide if additional injections are needed. Touch-up treatments may be needed to maintain the desired effects. People with mild lipoatrophy will require fewer treatments than those with severe lipoatrophy. Location Sculptra® is done in our office with a local anesthetic to numb the area. Trade Offs Mild, temporary discomfort, bruising, and swelling. Risks Side effects are usually mild, lasting 7 days or less and include temporary injection site reactions like redness, pain, firmness, swelling and bumps. Recovery Most patients return to their normal routine that day. Any redness can usually be covered up with make up. The effects of Sculptra were shown in a clinical study to last for up to 2 years after the first treatment session. Our doctors will recommend the dermal filler to best fulfill your cosmetic goals.

Non-Invasive Treatments

Indications The HydraFacial MD® treatment improves the appearance of fine lines, wrinkles, congested and enlarged pores, oily or acne-prone skin, hyperpigmentation and brown spots. Hydrafacial treaments are suitable for most skin types, including thin or aging skin, ethnic skin, and dry skin or oily skin. They are perfect for those who are looking for smoother, brighter-looking skin without the longer recovery time needed with more evasive procedures. Procedure The HydraFacial treatment is often described as feeling like “a cool paintbrush moving over the face”.
