Catholic Health Home Care provides comprehensive, skilled and compassionate home-based care to patients throughout Nassau, Suffolk and Queens counties. Services include nursing care, occupational/physical/speech therapy, medical social work, nutritional counseling and pastoral care. We create individualized plans of care, coordinate with physicians, and participate in an integrated Care At Home program to support recovery, avoid rehospitalization, and improve health and wellness in the comfort of patients' homes.
