Traditional Strip Surgery This used to be the gold standard in hair transplant surgery. Most of the larger hair transplant chains and physicians continue to use this technique as it is less expensive without the need for specialized equipment and allows several patients to be done on the same day. The negatives of this procedure are that it always leaves a linear scar that has the potential to enlarge over time. It is also considerably more painful as a large piece of scalp needs to be excised from the head and closed with staples or suture. Dr. Brown sees numerous patients each month who have undergone this procedure and are unhappy with the resultant scar. In addition, the complication rate is much higher with infections, pain, swelling, and a much longer healing time. Manual FUE The traditional approach to taking hair follicles for FUE is to use a manual punch and then extract each follicular unit with forceps. Though similar to NeoGraft®, the procedure is much slower allowing only several hundred follicles to be collected during a session. Viability of the grafts become an issue because of the increased time and handling involved. In addition, there are other physicians in Nevada who recently discovered the benefits and superior results with FUE. Due to financial considerations they are using various automated devices that are either not FDA approved, or much less expensive equipment and claiming that it equals NeoGraft®. Unlike the strip technique there is no use of a scalpel, sutures or staples. There's no postoperative tightening of the scalp, and very low risk of bleeding or nerve damage as you heal, which is completed in mere days, not weeks. Implanting the follicles to the area of hair loss: After a quick lunch break, the second part of the procedure begins with artistic and precise implantation of the new follicles. This is the most important part of the procedure to assure a natural-looking hair restoration. The correct angle of the new sites, along with the depth, is precisely made using a special implanting device. On the other hand, the strip procedure uses grafts that have been slivered or cut from a large strip of scalp into smaller units, usually consisting of several follicular units (plugs) to save time. With NeoGraft®, only pristine single grafts are used, consisting of 1-4 hairs, which is the follicular unit. This assures a natural look without the plugs. The grafts have a higher survival rate, as there is little handling of the units once extracted by the NeoGraft® machine. They are placed in special cooled solutions, and Dr. Brown may add PRP to the grafts to possibly improve early growth and survival. Results: The procedure ends with additional long-acting local anesthetics and special anti-inflammatory drugs to minimize or eliminate pain. A small amount of blood will be withdrawn, placed in a medical centrifuge and spun at a specific speed to separate the RBC’s from the plasma. A second centrifuge is then performed to isolate the Platelet Rich Plasma from the Platelet Poor Plasma (PPP). This second step is very important as it concentrates the platelets much more than the single centrifuge systems seen in the market. As PRP is processed from the patient’s own blood, there is no risk of allergy or rejection with this treatment. Once the scalp is numb with local anesthesia, the prepared PRP will be applied to the scalp as microneedling is performed. This assures a full application of the PRP over a wide area, and to stimulate the scalp. We do not inject PRP into the scalp. The other exciting use for PRP is during a Neograft® Hair Transplant procedure. The PRP is used on the fresh grafts as adjuvant to the storage solution. Not only do we see a faster rate of growth for the transplanted follicles, but they appear to be thicker in diameter. PRP Components Platelets are key components in hard and soft tissue repair mechanisms. They provide the essential growth factors such as fibroblast growth factor (FGF), platelet derived growth factor (PDGF), transforming growth factor (TGF), epidermal growth factor (EGF) and vascular endothelial growth factor (VEGF). These numerous growth features are involved in stem cell migration, differentiation and proliferation. The stimulation of fibroblasts and endothelial cells induces new extracellular matrix deposition and neo-vascularization. Plasma is essential for cell survival as it contains nutrients, vitamins, hormones, electrolytes and proteins. PRP works well in women and men. As thinning in women is due to a different mechanism than in men, usually only a couple of PRP hair treatments are needed. In men, we recommend starting with 3 treatments spaced 6-8 weeks apart. In order to obtain maximum density now and in the future, it is important to use adjunctive non-surgical therapy to preserve existing native hair. Non-surgical hair replacement or hair growth therapy may bring back some lost hair, but just as important, it may slow down or stop future loss. The use of non-surgical therapy can affect surgical planning in a number of ways. Sometimes the response is so good that patients decide they don’t need surgery. Sometimes it improves the situation so a smaller, less aggressive procedure is needed. Even if non surgical therapy is only minimally effective, a small amount of regrowth can significantly improve the appearance of density in transplanted areas. Minoxidil Hair Growth Treatment Minoxidil (Rogaine®) is a topical application approved for both men and women. It has been shown to either reduce hair loss or promote hair growth in about 80% of patients. Minoxidil acts as a direct anagen-prolonging agent. The original product, however, was oily and messy, and the propylene glycol additive could cause irritation in some patients. Recently, a new 5% foam delivery system has been released without any propylene glycol. It is much easier to use and better accepted by patients. Although the 5% is labeled for men, it can be used by both men and women. The active ingredient in Rogaine, minoxidil, was initially used to treat high blood pressure in the drug Loniten. A side effect of the drug was the reversal or deceleration of hair loss. In 1988, the Upjohn Corporation received approval to distribute Rogaine for men. In 1991, the Food and Drug Administration approved a Rogaine formula for women. Rogaine was only available through prescription until 1996, when Rogaine was approved for sale over the counter. Application Rogaine should be applied as directed by a physician or as stated on the packaging. The scalp must be clean and dry prior to application. Rogaine comes with a dropper for application of the liquid product. A typical treatment is 1 ml, which is inserted into the dropper and applied directly to the scalp two times daily. Rogaine’s foam formula can be applied directly to the scalp with the fingers. Rogaine does not need to be washed out after application. Side Effects Most side effects of Rogaine occur on the scalp and are mild. The treatment can irritate, burn or sting the scalp and cause itchiness. The alcohol content may also cause dryness in the scalp that can result in dandruff. The foam preparation does not irritate the scalp as much as the alcohol based product, but is more expensive. The drug may also cause an initial reaction of increased hair loss. More serious side effects of Rogaine are rare but include a rapid heartbeat, dizziness and swelling of the hands, feet or face. An allergic reaction to Rogaine can result in a rash and trouble breathing. Warning Rogaine is not recommended for pregnant women. Pregnant women should discuss taking this drug with their physicians. It is also unknown if the active ingredients in Rogaine can pass through a woman’s body into breast milk. If the scalp is irritated or has open wounds, Rogaine should not be used. This drug is also not recommended for use on any parts of the body aside from the scalp. People with heart, kidney or liver disease should seek medical advice before taking Rogaine. before after 0 Propecia (Finasteride) Hair Growth Treatment Propecia® (Finasteride) is a drug used to treat male pattern hair loss. Finasteride was initially approved by the FDA in 1992 under the brand name Proscar, for the treatment of an enlarged prostate. In 1997, the FDA approved finasteride for the treatment of male pattern baldness (MPB) under the brand name Propecia. This drug treats male pattern hair loss by blocking the body’s production of a male hormone called DHT. Finasteride is effective only for as long as it is taken; the hair gained or maintained is lost within 6–12 months of ceasing therapy. In clinical studies, finasteride, was shown to work on both the crown area and the hairline, but is most successful in the crown area. Finasteride is not indicated in women and is contraindicated in women who are potentially able to have children due to birth defects. Hair loss for both men and women is usually due to some hormonal changes in the body. DHT (Dihydrotestosterone) is the natural metabolite of the human body mainly responsible for hair loss. DHT exists when the male hormone Androgen gets involved with an enzyme called the 5 alpha reductase and combines with the male hormone testosterone. When DHT is abundant in an individual, it reaches the hair follicles and tends to prevent proteins, vitamins and minerals from providing the needed nourishment to sustain life in the hair of these follicles.

Most people think hair loss is a medical problem only impacting men. The truth is women make up about forty (40) percent of hair loss patients. Over 30 million women in the United States experience hair loss. At Hair Transplant Vegas, our goal is to excel in the management and treatment of female hair loss. Though the transplant procedure is similar in both men and women, there are important differences that need to be addressed. Design The frontal hairline is usually preserved in female pattern hair loss. Creating density in the forelock area is of the utmost importance in achieving an overall look of fullness. The way a person parts her hair is also important in the final design as to reinforce the density and fullness on the parted side with increase growth over to the non parted side. Should the hairline also be affected, redesign consists of a smoother, straighter hairline than seen in men with several mounds and possibly a widow’s peak to lower the hairline. A curved, soft angle is constructed with the hairline terminating into the temporal areas. Execution The NeoGraft® machine allows Dr. Brown to extract single follicular units, avoiding the painful linear scar seen with conventional hair transplant techniques used by most physicians. In addition, we are using a specially designed implanter that allows us to load up a single follicle, and then place it into the desired area in one motion. No additional incisions are needed and manipulation of the follicle is minimal as compared to the standard technique of making an incision, and then placing the follicle into the incision using forceps. Whether a high hairline is due to genetics, hair loss, or prior surgery procedures such as a browlift, we are able to not only lower and advance the hairline, but also change its shape by adding additional humps or a widow peak to create a more feminine appearance. Why Do Women Lose Hair? A post operative effluvium, or hair loss is more common in women than men and is directly related to the trauma and stress of the transplant procedure. The literature shows about 20% of women can see additional hair loss about 3-5 weeks after the procedure. The good news is this only temporary, and the hair will regrow about the same time as the newly transplanted hair. At Hair Transplant Vegas, we take every precaution to minimize this risk. Also, a lower concentration of epinephrine is used as there is much less bleeding with my technique. Epinephrine has been shown to contribute to the effluvium. But what exactly causes a woman to lose her hair? To understand that, it’s important to know a basic understanding about hair growth. Impact of Androgens on the Hair Cycle Hair grows at one-half inch per month and the growth phase lasts two to six years. With yet-to-be discovered physiologic signals, the hair goes into a resting phase for a period of time, then the hair follicle falls out and a new follicle starts the process all over again. In some patients with a genetic predisposition to hair loss, a group of hormones called androgens interferes with the growth cycle of the hair. These androgen hormones (testosterone, Androsteinedione, and Dihydrotestosterone (DHT)) are dominant hormones in men, but are present in women to a lesser degree. In many women, hair loss is due to the action of androgens on the hair follicles. In women who are genetically susceptible, when the testosterone comes in contact with enzymes residing in the hair cell, it is converted into the more potent androgen DHT, which then binds with receptors deep within the hair follicle. Over time, DHT in the hair follicle alters the natural resting and growth phases of the hair. Some of the hair follicles eventually die, while others stop continuing healthy hair growth.
