The heart failure program at Novant Health Heart & Vascular Institute Frontis Plaza works in conjunction with other care plans to help patients better manage their health. Our providers work in outpatient facilities and inpatient settings at Novant Health Forsyth Medical Center. Our care team will coordinate services for you, to make sure you have the information and tools you need to stay as healthy as possible. Frontis Plaza Boulevard just south of its intersection with Hanes Mall Boulevard in the Williamsburg Square neighborhood. The location just south of the Pavilions Shopping Center provides easy access to Interstate 40 via the Stratford Road (U.S 158) interchange.
