Many factors of a person’s face determine the impression of age in that individual. Extra eyelid skin, extra neck skin and jowls are obvious signs of age, as are thin crepe-paper skin, gray hair and wrinkles. There are many effective procedures to improve all of these problems and rejuvenate the face. Aging comes in five forms: too much skin, too much fat, muscles that are loose and banding, volume deflation and quality of skin issue like wrinkles, pigmentation, red spots and texture issues. For a complete rejuvenation, each issue must be addressed. In general, small aging problems only need minimally invasive procedures for good results. This may mean non-invasive laser procedures, chemical peels, infra-red light techniques to reach rejuvenation goals. Chemical Peels Infra-Red Treatments Laser Treatments Larger aging problems must be treated with more powerful techniques to achieve a good result. This would be more invasive procedures like facelift, neck lift, brow lifts or blepharoplasty (eye lift). Eyelid & Brow Lift Facelift Before After Photos in Charlotte, NC In the face, extra neck tissue makes a person appear old, overweight or both. To correct this problem, one must determine if the tissue is loose skin, extra fat, sagging muscle or a combination of all of these. If the problem is simply too much fat then liposuction, done through a 1-centimeter incision, will produce a younger, thinner neck. If the problem is fat and muscle, then through a slightly larger incision under the chin, fat is removed and the muscles are tightened to improve the neckline. This is called myeloplasty. If fat, muscle and skin are involved, then the appropriate plastic surgery procedure is a neck lift, where the myeloplasty is done and the extra skin is removed in the hair behind the ears. None of the procedures leave noticeable scars, and they will all improve the neck significantly. If the patient is bothered by a deep fold in the junction of the cheek and upper lip, then he or she is a facelift candidate. A facelift improves the cheek fold and jowls and also treats the neck. Every facelift surgery includes a neck lift. For patients who have deep folds in their cheeks and excess jowl skin, but a good neck, a mini facelift can be performed. All these procedures are tailored to match each individual patient’s concerns. Facelift Overview Facelift suspends the sagging muscles to their original position to create a more youthful appearance. Extra fat is removed and the skin is then gently tightened for the final effect. Fat grafting can be done to restore lost volume in the cheeks or lips. A little volume goes a long way so care is taken for the results to look natural. Most of our facelift patients feel they look 5–10 years younger, but some have an even more dramatic improvement. Rhinoplasty: What Is It? Rhinoplasty is the medical name for reshaping the nose. It is a cosmetic procedure that is different than surgery to improve the nasal airway, which is a functional or reconstructive procedure. Frequently, the procedures are done together. Rhinoplasty Overview During your consultation, we will take a medical history, discuss what you would like to change about your nose, examine the structure and support of your nose, look for nasal airway problems, and finally, photograph your nose. From these photographs, we will make computer images of your current nasal shape. Using the images and your physical exam, we will be able to make a diagnosis as to what is causing the nasal shape to be unsatisfactory. We will then morph the nasal shape based on your facial proportions and your goals to give you an idea of what is possible, given your basic nasal structure. We must be careful to note that the images are a goal, not a guarantee. The Rhinoplasty Procedure Rhinoplasty is performed on an outpatient basis using general anesthesia, in which the patient feels no pain and has no memory of the procedure. Operative time is approximately two hours, depending on the changes being made. Our Charlotte rhinoplasty patients go home the same day with a splint on the outside of the nose to protect its new shape. They may also have an internal splint. These splints are removed approximately five days later. After the surgery, swelling will resolve rapidly and will be 80% gone at 6 weeks. However, the remaining 20% may take a year to resolve entirely. The dynamics of nasal surgery are very complicated and we must be realistic about what we can achieve. Nasal skin thickness and bone structure determine what is possible for each nose. Using your actual photo with computer imaging, we will show you what we believe is an achievable appearance for your nose. Your nose will not be perfect, but it will be safely, predictably, and reliably much improved. Rhinoplasty Frequently Asked Questions What is the procedure like? Many patients want their skin to look better. Using the appropriate technique, sun damage, wrinkles, color irregularities, acne scarring, and acne can all be improved. The appropriate technique depends on the patient’s motivation and the amount of improvement desired. Skin is made up of an outer epidermis of skin cells and an underlying dermis make-up of the building blocks of skin, i.e. collagen, elastic fibers, water, blood vessels, glands, and glycosaminoglycans. All skin treatments are based upon several concepts: (1) speeding up epidermal cell turnover rate, (2) thickening the dermis and (3) stop the breakdown of the skin with antioxidants and skin protection with sunscreen. The life span of a single epidermal cell that you feel when you touch the skin is approximately ten days. During this time the cell is exposed to environmental trauma that damages the outside of the cell. By the end of the tenth day, the cell has enough surface damage that together with similar effects on millions of surrounding skin cells, the skin has a very rough feel. If medicine like Retin-A or glycolic acid can produce a more rapid turnover rate so that the cells are pushed off the body in five days, rather than ten days, then the cells will not be exposed to as much trauma. These cells will then feel softer when they are replaced by new, undamaged cells. It’s that simple. Another way Retin-A and glycolic acid work is that by stimulating the dermis to make more epidermis, the dermis becomes thicker itself. And a thicker dermis has more blood vessels for a pink glow, more elastic fibers for more recoil, more new collagen for volume to remove wrinkles, and more water content to plump up the skin. These are all qualities for youthful skin that we lose as we age. Newer science now recommends retinol rather than Retin-A just to increase the speed of the epidermal cell turn over and thicken the epidermis. The dermis itself is better stimulated with polypeptides that turn the cellular building blocks on more efficiently with less irritation than the older Retin-A. Procedure Differences The difference between Retin-A, retinol, and glycolic acid and the stronger trichloroacetic acid (TCA) chemical peels or croton oil peels is the depth of penetration. The deeper the treatment into the dermis, the greater improvement of the skin. The downside of a deeper treatment is, of course, a greater risk of scarring and much greater healing time. Retin-A, retinol, and glycolic acid barely penetrate the dermis; therefore, do little to thicken the dermis. The effects are to remove fine lines, improve superficial color irregularities and make softer skin. Medium to deep wrinkles and deep color irregularities will not be improved, but the recovery time is zero and there is essentially no risk. Microneedling is a minimally invasive technique that produces a clinical improvement of fine lines medium lines and enlarged pores with three treatments. The good news is that it works and that the downtime is zero. Chemical Peels Microneedling If one wants to improve deeper wrinkles, deep color problems, or acne problems, deeper penetration into the dermis with TCA, dermabrasion, or laser must be used. All of these agents produce damage to the dermis, removing old damaged skin elements and regenerating them with new collagen, new elastic fibers, increased water, and more blood vessels. The recovery time is increased with deeper treatments. The skin will be completely healed anywhere from one to two days for light penetration, and from ten to fourteen days for deep treatments. If all the treatments remove dermis, why is one technique better than another? Dermabrasion removes the dermis by mechanically sanding away tissue whereas laser removes the dermis by vaporization of the tissue. Chemical peels remove the dermis by chemical damage. Each technique does the same thing but in a different way. A laser using “Fractional” technology employs lasers to partially treat skin to produce an overall skin change. For example, the HALO fractional laser uses ablative or non-ablative light to just a third of the area to be treated. This produces an effect on the entire skin’s surface, though only a fraction was treated. The treated areas’ rejuvenating effects spill over into the skin areas immediately adjacent but not directly treated, thereby rejuvenating all the skin while only actually treating a fraction of the skin. Ablative lasers require one to two treatments and have 3-4 days of downtime post-treatment. Non-ablative lasers require 4-6 treatments but have only a day or two of down-time. These treatments produce mild to moderate improvement. The latest technology comes with a greater expense. We offer this technology but find many patients choose chemical peels for their effectiveness, safety, and lower cost. For superficial or deep treatments, chemical peels are safe, inexpensive, and effectively produce moderate skin changes.

Abdominoplasty is a very common procedure for patients who want to improve the appearance of their abdomen. Many of our tummy tuck patients desire to improve aesthetic flaws created by several pregnancies and/or weight fluctuations. Following these events, their skin becomes loose and may have stretch marks. The muscles are stretched to their limit and may even separate, causing a hernia. Many of our abdominoplasty patients are surprised to learn that repairing the weak abdominal muscles during surgery not only helps them look better, but may also greatly reduce their back pain. Tummy Tuck Overview A full abdominoplasty or tummy tuck is a procedure where extra skin and fat are removed and the loose muscles are repaired. The surgery is done through an incision that is from hip to hip, but even the skimpiest bathing suit would hide it. Frequently there is enough loose skin that all of the skin between the belly button and the pubic bone can be removed. When that skin is removed, so are all the stretch marks on that skin. Extra fat is thinned from the underside to trim the abdominal contour. Then the muscles are tightened to recreate the waistline and flatten the tummy. The incision is closed with dissolvable stitches and the patient is put in a compression garment to decrease swelling and bruising. How Tummy Tuck Is Performed The surgery is performed at our fully accredited plastic surgery facility under general anesthesia, in which the patient is totally asleep. The surgery can be painful, as muscles are made very tight to achieve the best possible look. But these tight muscles are like having a pulled muscle in the abdomen and everyone knows that can be tender. We also require our patients to walk with their waist bent for the first seven days to lessen the tension on the skin closure. This is very important to produce healthy skin that is as tight as possible post-operatively. We see our tummy tuck patients five to seven days after surgery in our Charlotte office. At that time the dressing is changed and the patient is allowed to shower. In seven days, the patient is allowed to stand up straight and lie flat on a bed. In order to allow the muscle and skin to adhere to each other, we recommend the abdominal area be kept quiet by limiting the bending and twisting motion of the waist for three weeks. The abdomen may seem tight at first, but will loosen with time. At three weeks, the compression garment can come off. We advise that it be worn with exercise for additional support from then on until three months. We don’t allow any isolated abdominal muscle exercise for three months, but all other exercises can be gradually increased beginning at three weeks. Most patients are back to work at approximately three weeks. Mini Tummy Tuck Surgery Mini abdominoplasty is a modification for the patient who only has excess skin and loose muscle from the pubic bone to the belly button. After liposuction of the entire abdomen, a C- section size incision is made and the repair done to the muscles. A smaller amount of skin is removed and the patient is put in the same compression garment as with the full procedure. Mini tummy tucks are also done in the office and do not have to walk bent at the waist at all. This procedure is great for those patients who only have a problem below the belly button. The recovery is easier than that of a full tummy tuck with a back-to-work time of 7-10 days for office-type jobs. Potential Risks of Tummy Tuck Like any surgery, bleeding and infection are possible risks, but occur very rarely. Incisions that don’t heal well because of decreased blood supply have occurred when the skin closure is too tight. That is why we are very cautious about keeping the waist bent for seven days after surgery. If the skin doesn’t heal well we keep it clean and moist and eventually it heals itself. This is an inconvenience we would rather avoid than treat. Hypertrophic thick scars occur occasionally and are treated with massage, vitamin E, and compression if needed. Keloids are rare. Unresponsive scars can be surgically revised, but this is unusual. Fluid collections under the skin can occur if the abdominal tissue is not compressed to the underlying repaired muscle. Aspiration is a sterile process where the fluid is removed by passing a needle through the skin and tapping the excess fluid. This is a painless procedure for the patient because the abdominal skin is numb for approximately 6–12 months after surgery and makes aspiration very easy. Still, we would rather prevent this fluid by wearing the compression garment than treat it with aspiration.

By using the liposuction procedure, we are able to remove unwanted fat deposits and reshape our patients’ shape and physique through small incisions less than half an inch long. Almost every area of the body is amenable to liposuction, but some areas are safer and produce more predictable results. Who Is a Good Candidate for Liposuction? Realistic expectations are critical in cosmetic surgery. We tell all our patients that this is not Hollywood, where the star emerges after cosmetic surgery with a perfect body, looking 15 years younger in a stunning new outfit, and 20 more points on their IQ. Cosmetic surgery results take time to develop and they will not be perfect. We feel very confident, however, in being able to safely produce reliable, predictable improvements that our patients are very pleased with. The Liposuction Procedure Traditional liposuction was first introduced decades ago and has had many changes and technical refinements along the way. The ideal patient for liposuction is someone within 10 to 15 pounds of their ideal body weight, with isolated areas of body fat that are resistant to diet and exercise. They should have good skin tone that can contract into the now smaller volume of fat and should have realistic expectations. If patients have none of the other qualifications, but have realistic expectations, they can still greatly benefit from liposuction. The reason liposuction is so popular is that it is safe and effectively removes fat from where it is not wanted and leaves fat alone where it is wanted. For instance, to get the same results of removing two pounds of fat from the abdomen using liposuction, one might have to lose twenty to thirty pounds from the entire body. Many people cannot lose twenty pounds, and if they did they might lose it from areas where they want to keep it such as their breasts or face. Liposuction allows us to selectively remove fat from exactly where we want it removed. The current technique involves making 1 centimeter incisions in the creases around the periphery of the area to be suctioned. Even though the procedure is done under IV anesthesia, the area is infiltrated with high volumes of local anesthesia to provide both intense vasoconstriction and long-term postoperative anesthesia. The fat is then simply vacuumed out with specially designed cannulas. The process can be thought of as a type of body whittling or sculpting; hence, the name lipo-sculpture. The patients are placed in compressive garments postoperatively, and because of the local anesthetic, are discharged home in virtually no pain. After Liposuction Patients are seen within 5 to 7 days at our Charlotte surgery center to check their progress and remove stitches. Most of our liposuction patients have surprisingly little bruising and may have such minimal discomfort that they are able to return to work within 2 to 3 days. In general, the more extensive the liposuction the longer the recovery time. The compressive garment is worn continuously for 3 to 6 weeks to limit swelling and ensure proper skin contraction. Patients are allowed to shower without the garment, beginning the second day postoperatively. Though improvements in body contour are noted immediately, the final results will take 2 to 3 months to be complete. Regular exercise may be resumed at 3 weeks. Possible Liposuction Complications Complications after liposuction are infrequent but include infection, bleeding, over-correction, under-correction, asymmetries, and skin irregularities. We try to remove just the right amount of fat to produce the desired improvement, but if we err, we always try to err on the conservative side. We can always take a little more fat in a minor touch-up procedure, but it is more difficult to put fat back into areas that have been over-resected. Our non-surgical fat removal approach is with CoolSculpting, where the fat is frozen here in the office with no need for anesthesia and no incisions. You can expect as much as 22% fat reduction after one treatment and 40% after two treatments. After the procedure is done there is no downtime and minimal if any pain. As with liposuction, the results are visible in about 3 months. CoolSculpting Realistic expectations are critical in cosmetic surgery.
