Surgical Procedures

A facelift can be performed alone or in combination with other procedures. A face lift addresses issues such as sagging, excess skin on the face. By carefully adjusting the remaining skin and muscles under the face, Dr. David Hayes at Montana Center for Facial Plastic Surgery can give you a more youthful, firmer, refreshed appearance. Face Lift Surgery in Missoula, MT Dr. Hayes is an experienced, thoughtful plastic surgeon in Missoula with the skill and artistry required to offer both minimally invasive and more extensive lift procedures, customizing treatment plans for each patient. If you're considering a facelift... As we age, the effects of gravity, exposure to the sun, and the stresses of daily life can be seen in our faces. Deep creases form between the nose and mouth; the jaw line grows slack and jowly; folds and fat deposits appear around the neck. A facelift (technically known as rhytidectomy) can reverse many of these signs of aging. By removing excess fat, tightening underlying muscles, and redraping the skin of the face and neck. A facelift can be done alone, or in conjunction with other procedures such as a forehead lift, eyelid surgery, or nose reshaping. The Best Candidates for a Facelift The best candidate for a facelift is a man or woman whose face and neck have begun to sag, but whose skin still has some elasticity and whose bone structure is strong and well-defined. Most patients are in their forties to sixties, but facelifts can be done successfully on people in their seventies or eighties as well. A facelift performed by Dr. Hayes will look natural and make you look like a younger version of yourself. You will not look overdone nor will your basic facial features be changed. Before you decide to have surgery, think carefully about your expectations and discuss them with Dr. Hayes and staff. Facelifting Techniques There are many different techniques for facelifting, you may have heard of some; the midface lift, s-lift, deep plane face lift, endoscopic facelift and many more. Dr. Hayes is very experienced in all techniques of facial surgery and will customize your surgery to fit your specific needs. Brow lift options include both an endoscopic lift and the traditional open brow lift to raise a heavy brow and open up the eye when blepharoplasty alone is not sufficient. If You're Considering a Forehead Lift... A forehead lift or "brow lift" is a procedure that restores a more youthful, refreshed look to the area above the eyes. The procedure corrects drooping brows and improves the horizontal lines and furrows that can make a person appear angry, sad, or tired. There are several techniques used to correct brow position depending on the hair line, the thickness of the hair, and the distance between the brow and the hairline. Dr. David Hayes, a pre-eminent plastic and cosmetic surgeon in Missoula, will explain which procedure is right for you. If you're considering a brow lift, this will provide a basic understanding of the procedure – when it can help, how it's performed, and what results you can expect. The Best Candidates for a Brow Lift A brow lift is most commonly performed in the 40-60 age range to minimize the visible effects of aging. However, it can also help people of any age who have developed furrows or frown lines due to stress or muscle activity. Individuals with inherited conditions, such as a low, heavy brow or furrowed lines above the nose can achieve a more alert and refreshed look with this procedure. Brow lift is often performed in conjunction with a facelift to provide a smoother overall look to the face. Eyelid surgery (blepharoplasty) may also be performed at the same time as a forehead lift, especially if a patient has significant skin overhang in the upper eyelids. Sometimes, patients who believe they need upper-eyelid surgery find that a forehead lift better meets their surgical goals. Patients who are bald, who have a receding hairline, or who have had previous upper-eyelid surgery may still be good candidates for forehead lift. Dr. Hayes will simply alter the incision location or perform a more conservative operation. Complications are rare and usually minor when a brow lift is performed by a qualified facial plastic surgeon. Yet the possibility of complications must be considered. In rare cases, the nerves that control eyebrow movement may be injured on one or both sides, resulting in a loss of ability to raise the eyebrows or wrinkle the forehead. Additional surgery may be required to correct the problem. Loss of sensation along or just beyond the incision line is common, especially with the classic forehead lift procedure. It is usually temporary, but may be permanent in some patients. Infection and bleeding are very rare, but are possible. You can reduce your risk of complications by closely following our instructions (both before and after surgery). Planning Your Surgery During your consultation, Dr. Hayes will discuss your goals for the surgery and ask you about certain medical conditions that could cause problems during or after the procedure, such as uncontrolled high blood pressure, blood-clotting problems, or the tendency to develop large scars. Be sure to tell him if you have had previous facial surgery, if you smoke, or if you take any drugs or medications, including aspirin or other drugs that affect clotting. If you decide to proceed with a brow lift, Dr. Hayes will explain the surgical technique, the recommended type of anesthesia, the type of facility where the surgery will be performed, the risks, and the costs involved. Don't hesitate to ask any questions you may have, especially those regarding your expectations and concerns about the results of surgery. Chin surgery (mentoplasty) is a procedure that reshapes the chin using carefully selected implants designed to create a more pleasing appearance. Chin implants are often one of the most satisfying for Missoula plastic surgeon Dr. David Hayes because the visual impact is immediate, often dramatically improving the patient’s appearance in small, subtle ways that others will notice, but not necessarily recognize exactly what has been done. If You're Considering a Chin Augmentation... In many cases, patients do not realize that they have a weak chin, thinking instead that they have a larger nose or a neck that appears too fat. In reality however, they may simply need a stronger chin to balance out the contours of their face. When done correctly, a chin implant can improve the proportions of the face, defining and improving the overall facial profile. Although frequently done as an isolated procedure, chin augmentation can place the finishing touches on other facial surgery. The Best Candidates for a Chin Augmentation The best candidate for chin implant surgery is a physically healthy man or woman who is realistic in his/her expectations and looking to augment the chin in order to provide better balance to the whole of the face and the profile. Chin implant surgery is typically performed with either local anesthesia and sedation, or with general anesthesia. This procedure takes anywhere from 30 minutes to an hour to complete. Planning Your Chin Surgery During your consultation, Dr. Hayes will discuss your goals and will show you the actual size of the implant that he thinks will balance out your facial features. If you decide to proceed, Dr. Hayes will explain the surgical technique, recommended type of anesthesia, type of facility where the procedure will be performed, and the risks and costs involved. Don't hesitate to ask any questions you may have, especially those regarding your expectations and concerns about the results of chin augmentation surgery. Ear Pinning (otoplasty) to reshape or remove small sections of cartilage from the back of the ears and allow them to lay closer to the head. It can also be used to reshape or reduce the size of your ears to make them more balanced and harmonious. Besides large or protruding ears, Dr. Hayes is an expert at dealing with other ear issues such as “lop ear” (when the tip seems to fold down and forward), “cupped ear” (which is usually a very small ear), and “shell ear” (when the curve in the outer rim, as well as the natural folds and creases, are missing). Surgery can also improve appearance of overly long, large or stretched earlobes, or lobes with large creases and wrinkles. Otoplasty for Children Otoplasty is commonly performed on children between the ages of four and 14. Ears are almost fully grown by age four, and the earlier the surgery, the less teasing and ridicule the child will have to endure. Ear surgery on adults is also possible, and there are generally no additional risks associated with ear surgery on an older patient. Planning for Surgery Dr. Hayes recommends that parents stay alert to their child's feelings about protruding ears; don't insist on the surgery until your child wants the change themselves. Children who feel uncomfortable about their ears and want the surgery, are generally more cooperative during the process and much happier with the outcome. In the initial meeting, Dr. Hayes will evaluate your child's condition (or yours if you are considering surgery for yourself), and recommend the most effective technique. We will also give you specific instructions on how to prepare for surgery. Where Your Surgery Will Be Performed Ear surgery is usually performed in the outpatient ambulatory surgery center, for adults we may perform your surgery in our state of the art on site surgical suite. Anesthesia and Surgery General anesthesia is used during this surgery for children, twilight sleep may be used for adult patients. Ear surgery usually takes about two hours, although complicated procedures may take longer. The technique Dr. Hayes uses depends on the nature of the issue you are seeking to correct. Dr. Hayes often utilizes a simple technique that requires only a small incision in the back of the ear to expose the ear cartilage. He then sculpts the cartilage and bends it back toward the head. (Self- dissolving stitches may be used to help maintain the new shape.) Occasionally, a piece of cartilage may be removed to provide a more natural-looking fold when the surgery is complete. Getting Back to Normal Adults and children are usually up and around within a few hours of surgery. The patient's head will be wrapped in a bandage immediately following surgery to promote the best molding and healing. Some patients report that their ears are mildly sore or may ache slightly for a few days. This is usually managed by medication. After a few days, the bandages will be swapped out for a lighter dressing, similar to a headband. Otoplasty in Missoula, MT Any activity in which the ear might be bent should be avoided for a month or so. Most adults can go back to work about five days after surgery. Children can go back to school after seven days or so, if they're careful about playground activity. You may want to ask your child's teacher to keep an eye on the child for a few weeks. Most children are extremely happy with their new ears and find that they are no longer teased about them.

Improve Your Beauty Profile When considering any surgery, you want to have confidence that your surgeon has the qualifications for the task. Dr. David Hayes is considered a leading expert in rhinoplasty (also known as a nose job) and he performs more of these delicate surgeries than any other plastic surgeon in the area. As one of your most prominent features, your nose has a significant ability to alter the appearance of your face. Its shape, length, width and proportions all combine to give your face its unique structure and character. Dr. Hayes recognizes that the diversity in nasal anatomy requires that the surgeon be versatile in multiple techniques. Rhinoplasty is complex because the esthetic goals must also coordinate with the functionality of the nose itself. Soft-tissue variances and the complexities of cartilage make each nose comparable to a unique work of art. Dr. Hayes uses his trained eyes and hands to create symmetry and balance to the nose so that it is proportional to your entire face. And because he’s board-certified in both facial plastic surgery and otolaryngology (ear, nose and throat specialty), you can feel confident that whether the surgery is for personal enhancement, resolving prior issues or to correct a condition, Dr. Hayes has the experience and recognized skills necessary. If You're Considering Rhinoplasty Rhinoplasty can reduce or increase the size of your nose, change the shape of the tip or the bridge, narrow the span of the nostrils, or change the angle between your nose and your upper lip. It may be performed in conjunction with other procedures to improve breathing. The goal is to improve the nose making it harmonize better with other facial features. Candidates for Rhinoplasty The best candidates for rhinoplasty (nose job) are people who are looking for improvement, not perfection, in the way they look. If you're physically healthy, psychologically stable, and realistic in your expectations, you may be a good candidate. Age may also be a consideration. Many surgeons prefer not to operate on teenagers until after they've completed their growth spurt which is around age 14 or 15 for girls, sometimes later for boys. It's important to consider your teenagers' social and emotional adjustment when considering surgery of the nose. In the consultation visit, Dr. Hayes will help parents determine the driving force for undergoing a Rhinoplasty. Planning Your Surgery Good communication between you and your physician is essential. In your initial consultation, Dr. Hayes will ask what you'd like your nose to look like, evaluate the structure of your nose and face, and discuss the possibilities with you. He will also explain the factors that can influence the procedure and the results. These factors include the structure of your nasal bones and cartilage, the shape of your face, the thickness of your skin, your age, and your expectations. Dr. Hayes will also explain the techniques and anesthesia he will use, the type of facility where the surgery will be performed, the risks and costs involved, and any options you may have. Most health insurance policies don't cover purely cosmetic surgery; however, if the procedure is performed for reconstructive purposes, to correct a breathing problem or a marked deformity, the procedure may be covered. After consultation, you can check with your insurer prior to obtain pre-authorization for your surgery. You will want to advise us if you've had any previous nose surgery or an injury to your nose, even if it was many years ago. You should also inform us if you have any allergies or breathing difficulties; if you're taking any medications, vitamins, taking recreational drugs or if you smoke. It is important to ask Dr. Hayes any questions you may have, especially those regarding your expectations and concerns about the results. Septorhinoplasty As a double-board-certified surgeon specializing in both otolaryngology and facial plastic surgery, Dr. Hayes is well known in a 3-state area for his expertise in difficult septorhinoplasty surgeries. A septorhinoplasty is performed to improve nasal airflow and therefore the ability to breathe well through the nose. Many people may have restricted airflow through their noses due to congenital deformities or from a trauma such as a broken nose that was not repaired correctly at the time of injury. Septorhinoplasty is a difficult surgery requiring skill and experience to achieve the desired results. Some insurance companies will cover this surgery with documented medical necessity. Men Have Really Been Missing Out Women have known for some time that society places a high value on looking fit and youthful. Now, as the marketplace grows more competitive, men want to stand out in the job market (and everyday life) by looking and feeling their best too. That’s why more and more men each year are turning to cosmetic surgery to rejuvenate, balance and improve their faces. From eyelid surgery (blepharoplasty) to nose jobs, to removing excess neck and jowl tissue to chin implants, if you want a more youthful, fit look, Dr. Hayes is the plastic surgeon to see. Double board certified with decades of experience, Dr. Hayes is an expert in helping men attain the rugged good looks that are so popular. Cosmetic procedures for men are very similar to those for women, reversing the signs of aging and improving problem areas, except that Dr. Hayes takes into consideration certain factors such as facial hair, skin thickness and the like. And, whereas women tend to want rounder faces, men usually go for more rugged, angular looks we associate with male virility and strength. Dr. Hayes also understands that you lead a busy life. That’s why he works to tailor surgical procedures to your needs, while streamlining the process whenever possible. Depending on the type of procedure you need done, you may be able to return to work with minimal downtime. Dr. Hayes will explain any restrictions, such as sports activities that might bump or injure your face that you will need to be mindful of. Rhinoplasty (Nose Jobs) for Men Men, like their female counterparts, can often benefit from rhinoplasty (also called a nose job). Rhinoplasty is the most common elective procedure for men. Many men desire a more balanced facial look and or an improved profile. This can often be achieved by correcting an off-centered or broken nose. Others may be looking to redefine their nose, getting rid of a bulbous tip, changing a nose that appears to droop, is overly large, too wide or narrow, has a ski-slope appearance, or one that just fits more with the rest of their face. Chin Implants for Men Society often associates a strong chin with manliness and determination. A strong chin not only improves your profile, but balances out your entire face. Chin augmentation (chin implants) come in a wide variety of styles and forms, providing natural looking chin enhancement for men who dislike their weak chin. Dr. Hayes, a skilled and knowledgeable, double board certified plastic surgeon and facial expert, understands the ways that even subtle changes can provide dramatic improvements. Chin implants are very small and the change, though discreet, is noticeable. Your friends may not know what exactly is different about you, commenting that you look more rested and confident, but people will certainly notice. Dr. Hayes will take the time to evaluate your face, make recommendations about what is right for you. Always happy to explain procedures and techniques, he will answer any questions you might have and guide you through the process to give you the masculine, firm looking chin you have always wanted. Neck Liposuction Neck liposuction is an excellent way to contour and shape your neck, while enhancing your profile. Excess neck skin can make you appear heavier, more tired and less vibrant. Men with good skin elasticity are often good candidates for neck liposuction because removing the underlying fat allows the neck to become tighter and firmer, much like it was when you were younger. Neck liposuction is very focused, targeting only the problem areas of your neck, and requiring only a small incision. BOTOX® & Fillers Like women, many men are concerned about the wrinkles, creases and crows-feet lining their faces. BOTOX®, provides a non-surgical solution to smoothing and restoring the more youthful appearance of your skin. The procedure is usually performed in the office, with no anesthesia required. Most patients go right back to work after an injection. Once treated, you may find that your face continues to improve, even as much as week later. The effects of BOTOX® are both natural looking and subtle, allowing you to dramatically enhance your reappearance with only minimal effort. The effects of BOTOX® are long lasting, but not permanent. Most treatments last for up to four months at a time. (In some cases longer.) Dr. Hayes personally administers all BOTOX® treatments. During your consultation, he will answer all of your questions and explain each step of the procedure. He has more experience with BOTOX® than any other provider in the local area. Many men are using facial fillers such as Restylane, Sculptra and Radiesse to achieve a more youthful look without feminizing their facial features. Typical areas for men to use fillers are the cheek, temples, jawline and nasal labial folds.

A More Youthful Look, Without the Downtime of Surgery As we age several changes occur. From the age of 30 on, we begin to lose bone and collagen. The fat pads in the face begin to droop and fall. The result of these changes is thinning skin, loss of the roundness of the cheek, loss of volume in the lips, folds and wrinkles around the mouth and a sagging jawline. There are many types of injectable fillers that can address these early changes to restore the youthfulness to your face. There is very little downtime associated with the use of fillers when they are injected by an experienced injector. Dr. Hayes was one of the original injectors of collagen, the very first ever injectable filler on the market and he has extensive experience with all types of fillers. Fillers in Missoula, MT Sometimes even a small change can make a big difference in how you look and feel. Dr. David Hayes offers you the safest, most advanced treatments to enhance your appearance and boost your self-esteem. And he performs all injectable procedures himself, so you can trust you are in good hands. Procedures offered include: Restylane® Restylane® Lyft Restylane® Silk Radiesse® Juvederm® Sculptra® BOTOX® & Dysport®

Morpheus 8 is an advanced microneedling treatment that uses radiofrequency energy to stimulate collagen production. When we age, our skin begins to lose elasticity, with wrinkles, fine lines, and sagging starting to appear. Morpheus 8 penetrates the skin at a deeper level than other microneedling treatments, resurfacing your skin and helping to restore a more youthful appearance. Morpheus 8 combines radio frequency technology, needling, and fractional treatments to address a variety of skincare problems. Tiny needles penetrate the skin and heat the tissue to stimulate the production of collagen. What makes Morpheus8 different than other microneedling treatments is the depth the needles penetrate the skin. It is the only treatment that can mold fat under the skin to obtain optimal skin tightening. Morpheus 8 can be used on all skin types making most patients a good candidate for treatment. We typically recommend three sessions spaced one month apart. Visible results will vary but typically patients will begin noticing results around 6 weeks with improvements continuing up to a year. We require a consultation prior to treatment with Morpheus 8 where our estheticians will discuss the treatment, downtime and expectations with you. Call us today! 406-541-7546.

Are you starting to notice laxity in your neck? Maybe your jawline isn't as defined as it used to be but you aren't quite ready for a facelift? FACEtite is an innovative procedure that uses radiofrequency to stimulate collagen production to help tighten and firm your lower face and neck. FACEtite is a minimally invasive procedure performed by Dr. Hayes under local anesthesia in our office surgical suite. It is often combined with facial/neck liposuction or Morpheus 8 to achieve optimal results. Through three small needle holes Dr. Hayes will insert the FACEtite wand and using radiofrequency energy, heat the tissues of the lower face and neck. The entire procedure takes approximately 1 hour. Chemical peels can be used to treat a wide variety of skin problems from fine lines and wrinkles to acne, melasma, and mild acne scarring. Each of our peels are customized to treat your specific skin issues and may be done in a series space a few weeks apart. Have you ever wanted to improve the clarity, tone or texture of your skin? Have you been told that you have melasma, or the "mask of pregnancy?" A chemical peel may be just what you need to gain control over your acne, reduce uneven hyperpigmentation, reduce sun damage, and improve clarity, while softening the overall appearance of fine lines and wrinkles. The Perfect Derma™ Peel The Perfect Derma Peel is a safe, effective medium depth peel for all skin types and ethnicities. It is the only peel on the market that contains the powerful anti-oxidant Glutathione (which inhibits the production of melanin to produce lighter and brighter skin). The Perfect Derma Peel takes about 15 minutes to administer and the patient may feel a slight stinging sensation which usually subsides after 30-60 seconds. Immediately after the peel has been applied the skin will appear red, as if you have had a sunburn. The peel solution should not be washed off for at least 6 hours. Peeling generally begins on day 3 and should be complete by day six or seven. Depending on the severity of the skin condition, a series of 3-4 peels spaced four weeks apart is recommended for optimal results. The Perfect Derma Peel will reduce hyperpigmentation, sun damage and Melasma. It also helps control acne and reduce acne scars. Environ Peels TCA and lactic acid peels can be used to treat photo damaged skin, roughness, irregular texture, fine lines and acne. The system avoids heavy deep peeling and achieves safer, effective and healthier results through a series of lighter repetitive peels. Noticeable results are achieved with a minimal risk of burning, irritation, tissue damage, hypo or hyper pigmentation and without the downtime normally seen with other peels. To learn more about how Chemical Peels can improve the texture, tone and clarity of your skin, call today at 406.541.7546 and schedule a personalized consultation. You can also contact us online. Affecting millions of people, acne is a medical issue that can be effectively treated by newer procedures and targeted acne products. Acne vulgaris, the most common form of acne, is characterized by inflamed lesions (pimples) caused by obstructed sebaceous glands (which secret oil to lubricated skin and hair follicles), leading to an overgrowth of acne causing bacteria. Acne can present as pimples or larger nodules and cysts on areas such as the face, neck, back, chest, shoulders and arms in both adolescents and adults. Left untreated, acne can cause significant scarring. Acne can also be influenced by several factors, and is frequently associated with fluctuations in hormone levels (such as during adolescence or menopause). Our highly trained medical estheticians are proud to offer a variety of effective treatments for all forms of acne. From intensive deep pore purification and medical microdermabrasion to photodynamic acne treatments (PDT) for severe, cystic acne, they have the knowledge and skills to help you get the lovely, smooth skin you have always dreamed of. No more waxing, shaving or endlessly plucking unwanted hair! Laser hair reduction is a noninvasive, safe, effective way to permanently reduce and remove unwanted body and facial hair. This is not your mother’s hair removal system. Today’s advanced laser hair removal technologies provide effective hair removal in areas such as the face, arms, legs, bikini and underarm zones. By targeting the natural melanin pigments within the hair follicle, a specially designed laser sends a pulse of light down to the root of the hair. This disrupts the hair follicle growth cycle, causing it to die and fall out. Please note: this treatment requires multiple treatments space 6-8 weeks apart, (the marked melanin is only readily available during a portion of the hair growth cycle), laser treatments offer permanent hair reduction after only a few visits; laser hair removal is safe and effective for all skin types.
