Transitional Rehab Center at St. John’s United is a carefully designed service facilitated by a skilled nursing and professional staff that aid in the transition from hospital care to home or an alternative care setting. During the transition the staff works together to see that patients achieve their short-term goals with rehabilitation and transitional care services.
While providing comprehensive therapy services throughout the duration a case manager is provided to assist with discharge planning as part of our continuum of care. Our care amenities include an onsite pharmacy, laundry services, private rooms with private bathrooms, local phone, cable and internet access.
