Cataracts

Your eye has a clear lens through which light passes, allowing you to see. When the lens loses its transparency, the cloudy tissue that develops is known as a cataract. Cataracts cause progressive, painless loss of vision. The lens clouds naturally as we age, so people over the age of 65 usually see a gradual reduction of vision. No one is exactly sure what causes cataracts. In younger people they can result from an injury, certain medications, or illnesses such as diabetes. Prolonged exposure to ultraviolet light may also play a role in the formation of cataracts. Studies have also shown that people who smoke cigarettes have a higher risk of developing cataracts than non-smokers. Although cataracts usually develop without apparent pain, some indications that a cataract may be forming are: Blurred or hazy vision Double vision Poor vision in bright light Seeing halos around lights Yellowish tinged vision Night vision difficulty If visual impairment interferes with your ability to read, work, or do the things you enjoy, then you will want to consider cataract surgery. Cataract surgery is a relatively painless procedure. It has a very high success rate and more than 90 percent of cataract surgery patients regain useful vision. Cataract Surgery During cataract surgery, a small ultrasonic probe is inserted into the eye which breaks up, or emulsifies, the cloudy lens into tiny pieces and gently sucks, or aspirates, those pieces out of the eye. Phaco surgery requires a small incision of only 3.2 mm or less. To make your procedure as painless as possible, anesthesia is a combination of local and/or topical along with IV sedation. With the recent advance of foldable IOLs, artificial lenses can be implanted through the same small incision that is created in the phaco procedure. These IOLs are made of a flexible material, allowing them to be folded for implantation. Once inside the eye, the lens unfolds and returns to its original shape. Premium IOLs There are a wide range of replacement lenses available to cataract patients, each offering different advantages for your post-surgery vision. The most effective lens depends on each patient's individual preferences and goals for their vision. The latest lenses help eliminate the need for glasses or contacts after cataract surgery, providing convenient, effective results for your specific vision conditions. Multifocal IOLs In the past, cataract lenses were only able to correct distance vision, often leaving patients with the need for reading glasses. Multifocal IOLs offer patients freedom from glasses after cataract surgery by improving vision at all distances and at any time of day. These lenses can quickly change focus to accommodate your vision needs. Up to 80 percent of patients do not need to rely on glasses with multifocal IOLs. Accommodative IOLs Accommodative IOLs offer a full range of restored vision for cataract patients. Many people experience a loss of near vision with age and often need to rely on glasses after cataract surgery. Accommodative IOLs such as Crystalens® replace the natural eye's natural lens and mimics its accommodation capabilities, allowing you to see clearly across a wide range of distances. The cornea is a thin, clear, spherical layer of tissue on the surface of the eye that provides a window for light to pass through. In a healthy eye, the cornea bends or refracts light rays so they focus precisely on the retina in the back of the eye. There are many diseases that can affect the cornea, causing pain or loss of vision. Disease, infection or injury can cause the cornea to swell (called "edema") or degrade (become cloudy and reduce vision). Common diseases and disorders that affect the cornea include: Allergies Bullous Keratopathy Conjunctivitis ("Pink Eye") Dry Eye Corneal Dystrophies including Fuchs' Dystrophy and Lattice Dystrophy Glaucoma (High Eye Pressure) Infections Keratitis (Viral Inflammation) Keratoconus Ocular Herpes Pterygium Shingles (Herpes Zoster) Stevens-Johnson Syndrome Treatment for corneal disease can take many forms, depending on the underlying problem as well as the patient's preferences. Some conditions resolve on their own and many can be treated with medication. If the cornea is severely damaged or if there is a risk of blindness, a corneal transplant may be recommended to preserve vision. Cornea Transplant The cornea is the clear covering of the front of the eye which bends, or refracts, light rays as they enter the eye. For clear vision to occur, the cornea must have the correct shape and clarity to focus incoming light rays precisely on the retina at the back of the eye. When the cornea becomes cloudy or misshapen from injury, infection or disease, transplantation may be recommended to replace it. Corneal transplants are usually performed with local anesthesia so there is no pain. During the procedure, the cornea is replaced with one from a human donor. Diabetic retinopathy is a complication of diabetes that weakens the blood vessels that supply nourishment to the retina (the light-sensitive lining in the back of the eye where vision is focused). These weak vessels can leak, swell or develop thin branches, causing a loss of vision. Changes to your vision may not be noticeable at first. But in its advanced stages, the disease can cause blurred or cloudy vision, floaters and blind spots – and, eventually, blindness. This damage is irreversible. Diabetic retinopathy is the most common diabetic eye complication and a leading cause of blindness in American adults. Macular edema, which is leaking fluid that causes blurred vision, often occurs with diabetic retinopathy. Fortunately, diabetic retinopathy is preventable. People with diabetes are most susceptible to developing it, but your risk is reduced if you follow your prescribed diet and medications, exercise regularly, control your blood pressure, and avoid alcohol and cigarettes. Regular eye exams are an integral part of making sure your eyes are healthy. Diabetic retinopathy can be detected through a visual acuity test, a dilated eye exam or tonometry. Although damage caused by diabetic retinopathy cannot be corrected, patients diagnosed with the condition can be treated to slow its progression and prevent further vision loss. Flashes and Floaters Flashes and floaters are symptoms of the eye that commonly occur as a result of age-related changes to the vitreous gel. When we are born, the vitreous is firmly attached to the retina and is a thick, firm substance without much movement. But as we age, the vitreous becomes thinner and more watery, and tissue debris that was once secure in the firm gel can now move around inside the eye, casting shadows on the retina. Flashes in vision occur as a result of pressure on the retina in the back of the eye, and causes patients to see flashing lights or lightning streaks. Floaters occur when fibers move across the vitreous and into your field of vision, causing patients to see specks, strands, webs or other shapes as the fibers cast shadows on the retina. These spots are most visible when looking at a plain, light background. Although flashes and floaters are common, especially as we age, it is important to see your doctor if you experience them, as they may indicate a retinal tear or hole. Your doctor can distinguish between harmless flashes and floaters, and those that may require treatment for an underlying condition. Most flashes and floaters will become less noticeable with time as patients adjust their vision. Dry eye is a common condition that occurs when the eyes are insufficiently moisturized, leading to itching, redness and pain from dry spots on the surface of the eye. The eyes may become dry and irritated because the tear ducts don't produce enough tears, or because the tears themselves have a chemical imbalance. Patients with this condition often experience irritating symptoms and can suffer from more serious vision damage if this condition is left untreated. It is important for patients with this condition to take special care of their eyes in order to alleviate symptoms and prevent complications. Your doctor can diagnose dry eye after a thorough evaluation of your eye and tear production through a Schirmer tear test. Causes of Dry Eye People usually begin experiencing dry eye symptoms as they age, but the condition can also result from certain medications, conditions or injuries. Macular degeneration, also known as age-related macular degeneration (AMD) is a common condition in older adults and the leading cause of vision loss and blindness in people over the age of 65. Macular degeneration affects the macula, the part of the retina responsible for the crisp, detailed vision needed for reading or driving. As we age, the tissue in the eye responsible for central vision slowly begins to deteriorate which can significantly affect a patient's quality of life. Types Macular degeneration can be classified as either wet (neovascular) or dry (non-neovascular). Dry macular degeneration is the more common diagnosis, and is considered to be an early stage of the disease. This type of the disease usually develops as a result of aging and thinning of macular tissues and the depositing of pigment within the macula. Only about 10% of patients see their condition progress to the more advanced and damaging wet macular degeneration. In wet macular degeneration, new blood vessels develop beneath the retina and cause a leakage of blood and fluid. This leakage can lead to permanent damages in the central vision and the creation of blind spots. Although less common, wet macular degeneration accounts for 90% of the blindness caused by all cases of this condition. Symptoms Patients with macular degeneration may notice gradual changes to their vision, including shadowy areas in the central vision, or fuzzy and distorted vision. These areas grow larger as the disease progresses, and can eventually turn into blind spots. Patients may also have difficulty seeing color and fine details. If the disease progresses to the wet form, patients may also see straight lines as wavy. With wet macular degeneration, central vision loss can occur rapidly, sometimes in as little as a few days or weeks. Your doctor may be able to detect early signs of macular degeneration, before any symptoms occur, through a regular eye exam. Any signs of this condition can be further confirmed by testing your central vision with an Amsler grid test. Regular eye exams are important in detecting macular degeneration and other serious eye conditions as early as possible, so that permanent side effects can be avoided. Causes and Risk Factors Many cases of macular degeneration are a result of aging and the natural deterioration of the eye tissue that is needed for clear vision. This disease can also be related to a genetic factor in patients who have a gene variant known as complement factor H. Nearly half of the blinding cases of macular degeneration are linked to this genetic deficiency. Macular degeneration is most common in females and whites, and the risk for all patients increases with age. This condition is the leading cause of blindness in the US for patients over the age of 65. Over 14% of adults between the ages of 70 and 79 have been diagnosed with advanced or intermediate age-related macular degeneration. Other factors that may increase your risk of macular degeneration include: Obesity Smoking Heredity High fat diet Prolonged sun exposure High blood pressure Lighter eye color Side effects of certain drugs Patients can minimize their risk of macular degeneration by practicing a healthy, active life and getting regular eye exams. The retina is a thin sheet of nerve tissue in the back of the eye where light rays are focused and transmitted to the brain. The vitreous is a gel-like substance that fills the eye and is connected to the retina, optic nerve and many blood vessels. Problems with the retina and vitreous -- including retinal tear and detachment, macular degeneration, diabetic retinopathy, infection and trauma -- can lead to vision loss and blindness. Early detection and treatment are critical in correcting problems before vision is lost and preventing further deterioration from occurring. Retinal Exam and Consultation A healthy retina is essential to maintaining clear vision and overall eye functioning. A retinal exam may be recommended by your doctor if any potential retinal abnormalities were detected during a general eye exam, or for patients at a higher risk of developing retinal conditions such as glaucoma, macular degeneration, retinal detachment and more. During a retinal exam and consultation, your doctor will perform series of diagnostic procedures in order to evaluate the retina for any signs of disease or abnormalities. Over 140 million people in the U.S. wear eyeglasses, and over 30 million wear contact lenses. Glasses and contact lenses improve vision by adjusting the way the eyes bend and focus light. Ideally, light rays are refracted (bent) as they pass through the cornea so that they focus on the retina in the back of the eye. In a healthy eye, this means that objects can be seen clearly. However, many people’s corneas have a shallow or steep curvature which causes light rays to focus in front of or behind the retina. Objects may then appear blurry at certain distances or at all distances. Glasses and contact lenses correct these refractive errors. Prescriptions are measured for each eye so patients can enjoy optimal vision clarity, usually 20/20. Eyewear may be used for certain activities, such as reading for farsighted (hyperopic) patients and driving or watching television for nearsighted (myopic) patients, or may be worn at all times. Glaucoma is the leading cause of blindness and visual impairment in the US, and can affect patients of all ages, many of who do not experience any symptoms and may not be aware that they have the disease. Glaucoma actually refers to a group of diseases that cause damage to the optic nerve as a result of increased pressure within the eye, but can also be caused by a severe eye infection, injury, blocked blood vessels or inflammatory conditions of the eye. There are two main types of glaucoma, open-angle and angle-closure. Open-angle glaucoma is the most common type of glaucoma and involves fluid in the eye not draining properly through the trabecular meshwork. Angle-closure glaucoma involves a sudden buildup of pressure in the eye and poor drainage because the angle between the iris and the cornea is too narrow. Many patients do not experience any symptoms during the early stages of glaucoma, including no pain and no vision loss. This makes it difficult for many patients to know if they have the disease. But as glaucoma progresses, patients may experience a loss of peripheral or side vision, along with sudden eye pain, headache, blurred vision or the appearance of halos around lights. Diagnosing Glaucoma While some patients may experience symptoms from glaucoma as the disease progresses, others do not learn they have the condition until they undergo a routine eye exam. There are several different exams performed to diagnose glaucoma, including a visual field and visual acuity test. These tests measure peripheral vision and how well patients can see at various distances. Other tests may also be performed, such as tonometry to measure the pressure inside the eye and pachymetry to measure the thickness of the cornea. Once glaucoma has been diagnosed, treatment should begin as soon as possible to help minimize the risk of permanent vision loss. There is no cure for glaucoma, so treatment focuses on relieving symptoms and preventing further damage from occurring. Most cases of glaucoma can be treated with eye drops, laser surgery or microsurgery. The best treatment for your individual case depends on the type and severity of the disease, and can be discussed with your doctor. Eye drops are used to reduce fluid production in the front of the eye or to help drain excess fluid, but can lead to redness, stinging, irritation or blurry vision. Patients should tell their doctor about any allergies they have to minimize the risk of side effects. Laser surgery for glaucoma aims to increase the outflow of fluid from the eye or eliminate fluid blockages through laser trabeculoplasty, iridotomy or cyclophotocoagulation. Microsurgery involves a surgical procedure called a trabeculectomy, which creates a new channel to drain fluid from the eye and reduce the pressure that causes glaucoma. Surgery is often performed after medication and laser procedures have failed. Preventing Glaucoma While there are no surefire ways to prevent glaucoma from developing, regular screenings and early detection are the best forms of protection against the harmful damage that the disease can cause. While anyone can develop glaucoma, some people are at a higher risk for developing disease. These people may include those who: Are over the age of 60 African Americans over the age of 40 Have a family history of glaucoma Have poor vision Have diabetes Patients should have a comprehensive dilated eye exam at least once every two years, especially if they have a higher risk of developing glaucoma. Laser Vision Correction (LASIK)

LASIK is a commonly performed refractive surgery procedure that can safely and effectively improve vision by reshaping the cornea to correct nearsightedness, farsightedness and astigmatism. Patients of all ages experience these vision problems as a result of an abnormal corneal shape that blurs or distorts vision. Short for Laser-Assisted In Situ Keratomileusis, LASIK uses targeted excimer laser beam energy to gently remove, or ablate, corneal tissue in order to correct refractive errors and help patients see clearly. Millions of patients choose to undergo LASIK each year, and achieve clear vision without the need for glasses or contact lenses, while also benefiting from minimal downtimes and little to no post-operative discomfort. Candidates for LASIK Although LASIK is considered a safe procedure, it is not ideal for everyone. Your doctor will perform a thorough evaluation to determine whether LASIK is appropriate for you, or if you would benefit from another type of refractive procedure. The ideal LASIK candidate: Is over 18 years old Has had stable vision for at least six months Has a health cornea thick enough for a flap Has refractive errors that fall within the treatable range It is also important for patients to fully understand the details and risks of the procedure, and maintain realistic expectations. LASIK Procedure LASIK is performed on an outpatient basis using only numbing eye drops to reduce any potential discomfort during the procedure. The entire surgery takes less than five minutes to perform, although patients can expect to spend a few hours at the office. If requested, patients can receive an oral sedative prior to surgery to relieve any anxieties about the procedure. During the LASIK procedure, the patient will lie down while the doctor positions the laser precisely over the eye. A speculum will be used to keep the eye open, the eye is cleaned and numbing drops are placed.
