University Health Community Care Raytown provides family-centered medical services for patients of all ages. Our board-certified family medicine providers and nurse practitioners will take care of acute and chronic care needs, and provide preventative and well care services. A diabetic educator is available as well. Providers will arrange for referrals to specialists as needed, and coordinate care through medical specialists affiliated with University Health Lakewood Medical Center. WIC nutrition services are also available on site.
