Frequently Asked Questions Q: What is home care? A: “Home care” encompasses a wide range of health and social services for individuals recovering from an acute illness, or managing a chronic illness. These services are delivered at home to recovering, disabled, chronically or terminally ill individuals whose needs may be medical, nursing, social, therapeutic treatment and/or assistance with the essential activities of daily living. Home care staff travel to the client’s home. This includes a single-family dwelling, apartment, assisted living facility, group home, residential housing facility, adult day care, or congregate living environment. Q: When is home care beneficial? A: Home care is considered the go-to health care service for individuals recuperating from an illness or surgery. It is also an alternative to long-term institutional care. Often, families may require higher-levels or on-going caregiving support for their loved one; home care services allow clients to maintain independence and privacy as they receive regular support and from their home care team. Q: How do I qualify for skilled home care services? A: To receive home care services, physician must certify that the individual - has a skilled need e.g. physical therapy, skilled nursing etc - has a homebound status - must have a physician’s order for home care services Individuals who are interested in home care services should contact our home care team to learn more and discuss their eligibility. Q: Who pays for home care services? A: Medicare, Medicaid, and private insurance pay for home care services. On-going assistance with activities of daily living are paid out-of-pocket by clients or their families. Q: How often does a home care professional come to the home? A: Each home care client receives a personal care plan tailored to their needs. It is not uncommon for clients to receive one visit several days a week depending on how many services are involved. Each visit usually lasts from 1-2 hours in length. Q: What Makes Triniti Home Health & Hospice Different? At Triniti Home Health & Hospice, we are committed to giving our hospice clients the most comfortable and honoring end-of-life journey possible. We strive to give families the gift of peace-of-mind and comfort, knowing that their loved one is receiving the best care from an interdisciplinary team of professionals, and allowing them to enjoy and create meaningful moments with their loved one without the burden of caregiving. Hospice offers special care for individuals in their final stages of life or who wish to focus on comfort rather than a cure. We recognize the unique and difficult challenges of terminal illness, and our goal is to provide emotional, physical, and spiritual support to the individual and their family. Hospice services are provided in the client’s home, whether that is a private home with family or in a care center or assisted living facility. Why should I consider hospice? A: The term ‘hospice’ can refer to a few things. First, it is a philosophy that focuses of providing palliative care to individuals at the end of life. Hospice care focuses on comfort and quality of life until death, instead of curative treatments that may postpone death or diminish quality of life. In short, hospice is all about care instead of cure; it values quality over quantity of life.. Q: What is the goal of hospice? A: The goal of hospice is to focus on symptom management and quality of life, so individuals and their family can enjoy and make the most of the remaining time they have together. Q: What does hospice offer? A: Our hospice team of interdisciplinary members offer excellent pain and symptom management, respite, as well as socio-emotional and spiritual support for individuals and their family. In addition, Triniti Home Health & Hospice provides bereavement services for families who have lost a loved one. Over a 13 month period, the bereaved will receive resources and support from our bereavement coordinator and her team. Q: How does a person qualify for hospice? A: To qualify for hospice, a doctor must indicate that you have a terminal illness with 6 months or less to live. Hospice is not limited to individuals with cancer. Anyone with a life-limiting illness such as heart disease, lung disease, end stage dementia and other chronic diseases can benefit from hospice care. Q: What services are offered by hospice? A: Hospice services are designed to provide socio-emotional and spiritual support to individuals and their family. In addition to your core team of caregivers consisting of Registered Nurses, an attending Physician and Social Worker, you may choose to receive home health aide, chaplaincy, volunteer, music and massage therapy services. Q: What qualifications and training do hospice staff have? A: Besides having a special heart and calling to serve clients and families during end-of-life, all hospice staff are licensed/certified and have received specialized training to administer end-of-life care. In addition, Triniti Home Health & Hospice nurses and social workers have been trained in how to care for veterans at end of life (level II We Honor Veterans provider) Q: Do I have to pay for hospice? What costs does hospice cover? A: Hospice is a Medicare benefit. Anyone who has Medicare part A that meets eligibility can receive hospice services paid for by Medicare. Aside from Medicare, Medical Assistance, and most private insurance programs also pay for hospice services. This typically means that cost of services, supplies, medications and equipment related to the individual’s terminal diagnosis are covered. Hospice provides different levels of care depending on the stage of illness, and does not typically provide 24-hour care. If your loved one requires round-the-clock care, family members may need to pay out-of-pocket for to arrange for additional services. Our social workers work closely with families and RN care managers to understand and anticipate each individual’s unique caregiving situation. They partner with families throughout the end-of-life journey, providing the needed resources to ensure individuals are safe and cared for as they transition through their end-of-life.

    Listen and Silent have the same letters with the obvious rearrangment of the letters. To listen a person must be silent. This group needs training in hearing what their customer is telling them. You probably have knowledge and feel you are superior to the people you are serving , you still SHOULD be taught the skill of listening. I should have taken my Mother to her home and continued to be the Care Giver. The Patient was your customer, but ti me she is my Parent.. I realize you have a list of patients and more will come. There is no emotional attachment to the people. You are paid to serve them. You should work on empathy along with sympathy. Just because I disagree with you does not translate to dislkiing the group. The Music was fabulous! The Massage Person was excellent. The Caley House isn't where Helen should have been for the care she needed. I put more hours in then theie employees. I needed assistance in finding an appropriate place for the care she needed. Best wishes BP
