Do I need a doctor's referral or prescription to start physical therapy?
Not always. Michigan allows you to begin physical therapy without a prescription for up to 21 days or 10 visits (direct access). Some insurance plans do require a referral, so call us at (810) 620-8042 and we will check your plan and coordinate with your physician if one is needed.
What insurance plans does Horizon Physical Therapy accept?
We accept Medicare, Michigan Medicaid plans (Blue Cross Complete, McLaren, Meridian, Molina, UnitedHealthcare Community Plan), Blue Cross Blue Shield of Michigan and Blue Care Network, HAP, Priority Health, Aetna, Cigna, Humana, UnitedHealthcare/Optum, auto insurance and workers' compensation claims, among others. What conditions do you treat?
We treat back and neck pain, orthopedic and sports injuries, post-surgical recovery (joint replacements, rotator cuff, ACL), auto-accident and work-related injuries, balance and vestibular disorders, neurological conditions, TMJ/jaw pain, women's health and pelvic floor conditions, and lymphedema. Our services include manual therapy, dry needling, Kinesio taping, therapeutic exercise and Complete Decongestive Therapy.
What should I bring to my first physical therapy appointment?
Please bring a photo ID, your insurance card, your physician's prescription or referral if you have one, a list of current medications, and any recent imaging or surgical reports. Wear comfortable clothing that lets us see and move the area we are treating. Plan on about an hour for your first evaluation with Dr. Adams.
Do you offer lymphedema treatment in Flint?
Yes. Horizon Physical Therapy and Rehabilitation provides lymphedema and swelling-disorder care in Flint, including a full swelling evaluation, Complete Decongestive Therapy (CDT), manual lymphatic drainage, compression fitting and a home program to prevent flare-ups.
