Counseling & Support
We offer extensive counseling and support services to meet your needs, whether your loved one has just received a serious or life-limiting diagnosis or is already receiving our care. Our counselors are available to provide emotional support before and after a loss through one-on-one grief counseling, support groups and remembrance events. And our trained volunteers provide additional support through assistance with errands, companionship and respite for families. If you are not sure what services you need, our care navigators are available to answer your questions and connect you to resources that can help.
Elder Medical Care
Older individuals living with serious illnesses often find themselves in an unending cycle of health crises that lead from home to the hospital to rehabilitation and back home again. Gilchrist’s team of physicians, nurse practitioners and social workers are experts in providing coordinated geriatric and comfort care, and guidance to improve quality of life. We help ensure that those who often fall through the cracks of the health care system—those who have difficulty traveling to office appointments and for whom treatment is becoming less effective—receive the care and attention they need when and where they need it.

Thank you for the amazing care my mom received while under your care for 6 weeks (inpatient ) as well as 2 months of home hospice. I can’t possibly say enough positive things about the staff from the time you walked through the door. I wish every person could receive the phenomenal care that you all provided. We will forever be grateful!! Our mom (Jane) despite being spunky, appreciated the level of care and understanding she received. Thank you from the bottom of our hearts !

This place was great about year or year in half and they have completely changed....rude nurses that call to do intake and then tell me before they even come out im not qualified u cant say that without seeing me....and u cant say just because gastroparesis isn't terminal doesn't mean mine isn't im malnutrition lost all hair not they care and over 120 pounds barely can get out of bed barely able speak I'm so sick and barely remember day to day I tried palliative care even though there's no more treatments they can do I tried everything palliative care saying sorry u need hospice. With blood pressures in its 70 and 80s pulse drops to 40s pps score of 4 with palliative and 3 with other hospice company im just trying tk choose my best thing here im dire pain im having my caregivers type this and there like no promises on iv meds well no crap but u all at beginning tried oralxand u saw how well that worked out I ended up inpatient with a pca pump... I cant take oral anything. Can't walk nkw on oxygen you guys need to stop judging on the typical terminal illness bullcrap and care for the patients symptoms I been told multiple companies i qualified and all I was doing is trying to go with someone I knew and loved and now they don't care about pain nausea vomiting the one nurse Darlene I believe her name was who called with nasty attitude said u have to have dementia or cancer. And that people live with gastroparesis for 20 years no shit but mine isn't like typical people I have severe gastroparesis cant keep 0 things down and the doxtors say they have tried everything and that I would not survive another surgery duento my significant decline especially over last 2 week. Thanks alot let me due suffering crying in pain so much for comfort care. Careful what u choose and get into if u don't have a typical terminal illness. Because your screwed if u don't. Plus what about respecting patients wishes on dnr/dni and don't want no stupid transfer to hospital crap no artificial nutrition and I have don't fluids but I don't want that anymore either because all it's doing is dragging me along not helping. Plus these people trying to claim I have. Picc line when I told them I don't have one anymore. Only reason I gave them a 2 star is because of how wonderful they were last time I have had them and what I can say is im much worse off now then I was then. But people if your loved ones want to be comfortable and cant be orally don't choose this company.

I'm expressing my sincere gratitude for the care provided at your location.
The facilities were lovely, the views were wonderful, and the cleanliness was excellent. Most importantly, my grandmother's last days here were incredibly comfortable from start to finish.
I would definitely recommend this location to others seeking a place for their loved ones. Thank you for everything.

I highly recommend this facility. My father spent his remaining days here. I can't express enough gratitude for the staff, their support, kindness, professionalism and experience. I dont have much family to rely on for support, but not once did I feel alone or lost throughout the process, and that is a beautiful thing! I also did not feel that my dad was left alone at times that I couldn't be by his side. The staff kept me well informed as he progressed through the stages.
The facility itself was also very nice and well kept, and was very peaceful to be in. I loved the birds, I know my dad would have loved them too.

My dad spent 5 days here on respite. The staff was very kind and attentive! My dad really enjoyed the food, which was nice because he did not eat very much at home. He had a beautiful room, looking out onto a courtyard. Deer visited nearby every day. It was clean and bright, and we are so thankful for the time he spent there.
