Ochsner LSU Health - Transitions of Care Clinic, located in Shreveport, Louisiana, is a follow-up appointment scheduled shortly after you leave the hospital, usually within 7–14 days. Appointments will be done virtually through MyOchsner. If an in-person visit is needed, our clinic is located on the third floor of the Comp Care Building which is to the left of the main hospital entrance at 1541 Kings Highway.
