Additional services may be available at our clinic in New Orleans East, 9900 Lake Forest Blvd., NOLA 70127. Appointments required at both locations. Services are provided on a sliding scale based on income. We accept Medicaid, Healthy Louisiana Plans with value added dental benefit, Medicare, Medicare Advantage Plans, Dual-Eligible Medicaid/Medicare Plans, and several private insurance plans.
