Home Health Care
Skilled Nursing
Skilled Nursing
Skilled Nurses provide wound care, infusion therapy,
pain management, diabetic and respiratory care, as well
as medication management. Nurses also educate both
the patient and family about chronic disease symptoms
and management.
Respiratory Therapy
Respiratory Therapists provide assessment and care for patients with disorders affecting the cardiopulmonary system. Therapists offer comprehensive treatment and disease management for COPD, emphysema, asthma, pneumonia and other breathing disorders.
Physical Therapy
Physical Therapists provide exercise and teaching to help patients achieve the highest level of mobility and functional abilities to improve quality of life. Treatment of physical impairments and disabilities that interfere with safe mobility is the primary focus. After complete evaluation, PTs provide one-on-one, personalized exercises to promote strength, balance and flexibility, while helping people manage pain.
PT/INR Monitoring
Home Health Nurses provide on-site PT/INR monitoring for patients with clotting disorders who are taking prescribed anti-coagulation medication. This in-home testing provides immediate results so abnormalities can be reported immediately to your health care provider, preventing cardiovascular complications.
Occupational Therapy
Occupational Therapists help patients improve their ability to perform tasks in living and working environments. They work with individuals who suffer from mentally, physically, developmentally or emotionally disabling conditions. The Therapist helps patients improve their basic motor functions as well as find ways to compensate for permanent loss of function. The goal is to help patients have independent, productive, satisfying lives.
Speech Therapy
Speech Therapists evaluate and treat voice, speech, language and swallowing disorders, and hearing impairments that affect communication. Therapists help patients regain the ability to produce and understand speech, and facilitate communication skills.
Lymphedema Therapy
Lymphedema Therapists use a gentle application of exercise and massage to encourage lymph drainage and help relieve swelling for patients with pain due to edema.
Medical Social Workers
Social Workers provide emotional support and counseling, as well as assistance with financial matters.
Home Health Aides
HHAs are trained, certified health care workers who provide assistance in the home with personal care (hygiene and exercise) or light household duties (meal preparation and cleaning), and monitoring the patient’s condition.
Dieticians
Certified Dieticians provide nutritional consultations tailored to the patient’s specific illness and dietary health needs.
Bridge Program
The Palliative Care Team bridges Home Health and Hospice with treatment focused on comfort care for patients who desire both curative and palliative care. This transitional program ensures that patients receive the full spectrum of care from the time of diagnosis throughout the course of the illness.
CareLink
This electronic relay device allows the in-home client to signal for help in an emergency.
TeleHealth
This electronic device monitors vital statistics and weight in the patient’s home. Information is transmitted to our office for review so abnormalities can be reported immediately to your health care provider, allowing earlier intervention.

Skilled Nursing
Skilled Nurses and the attending Physician work together to provide a plan of care that helps manage pain and physical symptoms while providing needed medications, supplies and equipment. Comfort and dignity are enhanced during end-of-life moments.

Hospice came into our lives in late October when my husband’s health declined. I cannot imagine taking that journey without the caring and capable hands of VNA Hospice. They were with us every step of the way and brought so much practical support as well as emotional comfort and spiritual aid to the most difficult period of our lives.
My husband was adamantly opposed to hospice in the beginning but by day 3 was wondering aloud why he had waited so long. Sherry Brown was his nurse for the entire time and he derived such comfort from her compassionate care and she truly became a friend as well as a caregiver. She offered options to him and always let him feel like he was in charge of his healthcare decisions. As more and more options exhausted themselves, that aspect of his care became more and more important. Hospice took care of all the billing and Rx needs as well as medical equipment that he needed as his world began to shrink. It gave me, as his wife, such a great gift of not having to worry about the logistical needs of his care.
Yes, I believe you can face end of life issues without hospice, but after what we experienced, why would you want to travel that road without their caring and loving support?
