Physical Therapy
21st Century Rehab is a physical therapist owned company that was founded in 1995 by Steve and Michelle Cassabaum. Since that time, 21st Century Rehab has added four partners, all physical therapists who work in our clincs. 21st Century Rehab has seven outpatient clinics locations in Altoona, Boone, Carlisle, Indianola, Madrid and Osceola and have seven hospital partnerships with Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Greene County Medical Center (Jefferson), Knoxville Hospitals and Clinics, Story County Medical Center (Nevada), Van Diest Hospital (Webster City) and Wayne County Hospital (Corydon).
21st Century Rehab provides quality physical therapy, occupational therapy, speech therapy, sports medicine, and occupational health services to patients and communities. These services range from joint and muscle pain, trouble with daily activities, range of motion issues, headaches, vertigo, TMJ issues, Women's and Men's health, and more.

- DMDeb MarquardtAug 7, 20265.0My experience with Josh, Cole and Renae was top notch. I came in after a year of struggling with frozen shoulder and after the first visit I felt hope for relief. The exercises were spot on for noticing immediate improvements. Not only was their professionalism appreciated but their personal side made me feel comfortable from the beginning. What a blessing it is to have this trio in Madrid to serve local and not local patients. I really can’t say enough. I will be very ready to use their services again if necessary. Thank you!!

He explains everything in terms that are understandable with references that help you remember how to do the exercises. He is very personable and caring. I extended my vacation to be able to get in more PT sessions since I was getting so much better. I would wholeheartedly recommend them.

I've enjoyed 21st Century Rehab because my appointments always start on time and the staff is super friendly and knowledgeable. I feel like my questions are always answered and if I need help I know they will get me taken care of. When going through the exercises, it felt reassuring to me that they check in several times to make sure I am feeling alright and not just going through the motions. Would recommend to anyone!
