I am new to NYU Langone. Will I need to complete any forms?

Yes. New patients are asked to review and complete forms such as the Faculty Group Practice Patient Demographic Form, Electronic Health Information System Consent, Financial Policies, Patient Responsibility Form, and Notice of Privacy Practices Acknowledgment. Many forms can be reviewed and completed before your visit via the patient forms page.

Doctors at NYU Langone participate in most major insurance plans. Appointment coordinators can review insurance details with you when scheduling.

What is the 'no-show' policy?

If you cannot keep your appointment, call your doctor’s office as soon as possible to cancel. Missed appointments without cancellation may result in additional fees.

Why did I receive a bill for a doctor that I did not see?

You may receive charges from clinicians who provided services behind the scenes (for example anesthesia, radiology, pathology). These providers are part of your care and may bill for services even if you didn’t see them face-to-face.

What forms of payment do you accept?

Practices accept cash, personal checks, money orders, and credit cards (Visa, MasterCard, Discover, American Express). Traveler’s checks are not accepted. Your credit card will be charged only for amounts you authorize. Payment plans may be arranged through an onsite billing coordinator.

Can I access my medical information online?

Yes. NYU Langone uses MyChart (NYU Langone Health MyChart) to let patients view test results, medical records, medications, immunizations, schedule appointments, request refills, message providers, and access billing statements.

How can I get a copy of my test results and medical records?

Accepted Answer

Call your doctor’s office to request copies and sign a consent form if records are to be released to another office or hospital. You can also view recent test results via your MyChart account.

What do I do if I run out of my medications?

Submit prescription refill requests via MyChart or call your doctor’s office to request a refill.
