Face

Age, talking, laughing, years of sun exposure, etc. take a toll on your face. The plastic surgeon will evaluate your skin, muscles, and bone structure of your face as well as your general health to determine the appropriate treatment options. He will also consider your concerns and choose or suggest various other procedures. The face lift addresses the mid-portion of the face, jowls, and neck. It tightens the muscles and removes the excess skin based on the tightened muscles. Ancillary procedures may be suggested and indicated to achieve a smooth transition from one area to the other. Procedures like skin resurfacing, fillers, Botox, and facial implants will further the result. The best way to tell whether a face lift will help you is by consultation with your plastic surgeon and discussing with him your personal goals with regards to your facial appearance. Eyelid surgery (blepharoplasty) is designed to remove excess skin and fat from the upper and lower eyelids giving you a more youthful, “awake” look. The procedure can correct drooping upper lids that tend to sag with time and the puffy bags below the eyes. Sutures are generally removed after three or four days. Any bruising may be easily covered by makeup. Blepharoplasty can be done as a separate procedure, or in conjunction with other facial surgery such as a facelift or a browlift. Procedure Details: Eyelid Surgery Procedure: Correct drooping upper eyelids and puffy bags below the eyes by removing excess fat, skin, and muscle (Upper-eyelid surgery may be covered by insurance if used to correct visual field defects). Because of this, fixing the forehead and brow area can be amongst the most rewarding parts of the face to address, as it can greatly “freshen up” the face. With many newer endoscopic techniques, this can often be accomplished with minimal incisions and very little down time. Surgery to reshape the nose, (rhinoplasty) is among the most common plastic surgical procedures. This procedure can reduce or increase the size of the nose, remove an unwanted hump, change the shape of the tip or the bridge, narrow the width of the nostrils, or change the angle between the nose and the upper lip. The procedure is also used to correct problems resulting from injury and can help relieve certain breathing difficulties. The most important factor is having realistic expectations and the options to achieve your expectations can be discussed with your Plastic Surgeon during your consultation.

Chin surgery can change the shape of the chin to reduce a prominent chin or to extend the chin from the face. The contour of the chin can be reshaped to be smoother, or more angular in appearance. This is done by placing a chin implant through the mouth or through a small scar under the chin. Similarly, cheeks can also be enhanced with implants place through the mouth to help accentuate your cheekbones. Lip augmentation creates fuller, plumper lips and reduces fine wrinkles around the mouth. Lips may be injected with injectable fillers such as Juvederm or Restylane or with fat transferred from another part of your body. However, both fillers and fat are absorbed over time and therefore repeat treatments may be necessary to maintain results. Please discuss your options with your plastic surgeon or healthcare provider to determine if you are an appropriate candidate.

Breast

A woman’s breasts represent an important part of her self image. Some may feel that their breasts have always been too small, while others may have lost breast tissue after pregnancy or weight loss. For these women, breast augmentation (enlargement) may provide a way to improve their appearance and enhance their confidence and self esteem. What is breast augmentation? Breast augmentation is a surgical procedure in which a breast-implant is used to increase the size of the breast in order to increase fullness or projection. Although this procedure enhances the size of the breast, it does not always correct sagging. In order to achieve this, the augmentation may have to be combined with a procedure called a mastopexy or breast lift. The procedure is performed in an outpatient setting and is well tolerated by healthy patients. There is usually a brief recovery period required before the patient can resume most daily activities and this may vary depending on the person. The return to a full active lifestyle can sometimes take even longer. Consult your Board Certified Plastic Surgeon for more details. Procedure Details: Breast Augmentation Procedure: Enhance the size of breasts using silicone filled gel implants or saline implants. Length: 1 to 2 hours. Anesthesia: Local with sedation, or general. In/Outpatient: Usually outpatient. Side Effects: Temporary soreness, swelling, change in nipple sensation, bruising. Breast sensitive to stimulation for a few weeks. Over time, the effects of gravity along with the loss of the skin’s natural elasticity begins to have an effect on the appearance of a woman’s breasts. These factors, combined with pregnancies and nursing will often cause the breasts to lose their youthful shape and firmness. A breastlift, or mastopexy, is a surgical procedure to raise and reshape sagging breasts. The procedure can also reduce the size of the areola (the darker skin surrounding the nipple). Breast implants inserted in conjunction with a mastopexy procedure can increase both firmness and size. Large, heavy, pendulous breasts may cause a woman to potentially experience various medical problems related to the excessive weight of the breasts. These problems can include issues with posture, skeletal deformities, back, neck and shoulder pain, skin irritation, bra strap grooving as well as problems with breathing which are often seen with this condition. In addition there are often issues of self consciousness that can accompany excessively large breasts especially in teenage girls. The breast reduction procedure removes fat, glandular tissue and excessive skin resulting in smaller, lighter, firmer breasts that have a better shape and are in a much better proportion to the rest of the body. The procedure is now often done on an outpatient basis and is immediately rewarding in alleviating symptoms due to macromastia (large breasts). Gynecomastia (enlarged male breasts) although rarely discussed, is a common condition. Gynecomastia affects approximately 40 to 60 percent of men. There is no known cause for gynecomastia, although there has been some correlation with various drugs or medical conditions. Men who feel self-conscious about their appearance are helped with breast-reduction surgery. This procedure involves the removal of fat and/or glandular tissue from the breasts and in extreme cases may even involve removal of excess skin. This results in a more contoured chest that is flatter and firmer. Procedure Details: Breast Reduction Procedure: Reduce the size of overly large, pendulous breasts by removing excess skin and tissue and repositioning remaining tissue and nipples. Reconstruction of the breast after a mastectomy due to cancer or other diseases is one of the most rewarding surgical procedures for patients who are often trying to deal with the many issues of being diagnosed with breast cancer. Using many new and innovative techniques you and your plastic surgeon can devise a plan to re-create a breast that is similar in form and appearance to a natural breast. This ability to reconstruct the breast helps patients “feel whole again” while still being treated effectively for their disease process. Procedure Details: Breast Reconstruction Procedure: Raise and reshape sagging breasts by removing excess skin and repositioning remaining tissue and nipples. Length: 3 to 5 hours. Anesthesia: Local with sedation, or general. In/Outpatient: Inpatient. Side Effects: Loss of sensation in the breast is possible; however, some feeling may return. Permanent scars. Recovery: It may take up to six weeks to recover from a combined masectomy and reconstruction. Overhead lifting, strenuous activity and light exercise may be resumed after 3 to 6 weeks after surgery. Duration of Results: Variable.

The decision to have breast implant revision surgery is extremely personal. You’ll have to decide if the benefits will achieve your goals and if the risks and potential complications of breast implant revision are acceptable. The possible risks of breast implant revision surgery include, but are not limited to: Bleeding Infection Poor healing of incisions Hematoma Anesthesia risks Fluid accumulation (seroma) Skin loss Numbness or other changes in skin sensation Skin discoloration and/or prolonged swelling Unfavorable scarring Recurrent looseness of skin Fatty tissue found deep in the skin might die (fat necrosis) Deep vein thrombosis, cardiac and pulmonary complications Asymmetry Suboptimal aesthetic result Possible need for revision surgery Persistent pain These risks and others will be fully discussed prior to your consent. It’s important that you address all your questions directly with your plastic surgeon How should I prepare for breast implant revision? In preparing for breast implant revision surgery, you may be asked to: Get lab testing or a medical evaluation Get breast or implant imaging, such as a mammogram, ultrasound or MRI Take certain medications or adjust your current medications Stop smoking Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding Breast implant revision surgery is typically performed in a hospital or licensed ambulatory surgery setting, and will likely use general anesthesia. If your breast implant revision is performed on an outpatient basis, be sure to arrange for someone to drive you to and from surgery and to stay with you for at least the first night following surgery. What are the steps of a breast implant revision procedure? Step 1 – Anesthesia Medications are administered for your comfort during the surgical procedure. The choices include intravenous sedation and general anesthesia. Your doctor will recommend the best choice for you. Step 2 – The incision The type of incision used in breast implant revision surgery will vary depending upon the type of procedures that will be performed. The incision can include, but is not limited to, any of the following: Inframammary incision – in the crease under the breast Breast lift incision – either vertical incision or Wise pattern incision Peri-areolar incision – incision part way or all the way around a areola Step 3 – Closing the incisions Sutures, skin adhesives, tapes or clips close the skin incisions. Step 4 – See the results What should I expect during my breast implant revision recovery? Following your breast implant revision, gauze dressings or bandages maybe applied to your incisions. Depending on the details of your surgery, you may or may not be placed in a support bra or other type of garment. In some cases, a small, thin tube may be temporarily placed under the skin to drain any excess blood or fluid that may collect. You will be given specific instructions that may include: How to care for your surgical site(s) following surgery Medications to apply or take orally to aid healing and reduce the risk of infection Specific concerns to look for at the surgical site or in your general health When to follow up with your plastic surgeon Be sure to ask your plastic surgeon specific questions about what you can expect during your individual recovery period. Where will I be taken after my surgery is complete? What medication will I be given or prescribed after surgery? Will I have dressings/bandages after surgery? When will they be removed? Will there be drains? For how long? When can I bathe or shower? When can I resume normal activity and exercise? When do I return for follow-up care? Healing will continue for several weeks as swelling decreases. Continue to follow your plastic surgeon’s instructions and attend follow-up visits as scheduled. What results should I expect after breast implant revision? The final results of breast implant revision surgery will be initially obscured by swelling. It will take at least six weeks for the swelling to resolve and the breasts to feel softer and more natural. In some cases, it can take 6-12 months for the breasts to feel “normal.” Healing time is different for everyone and sometimes it can take longer for the breasts to soften, for normal sensation to return, for scars to fade and for the patient to get used to the feel of having an implant. Please discuss realistic expectations with your surgeon regarding healing. The practice of medicine and surgery is not an exact science. Although good results are expected, there is no guarantee. In some situations, it may not be possible to achieve optimal results with a single surgical procedure and another surgery may be necessary.

Body

Being a mom is one of the most challenging, frustrating, yet rewarding experiences a woman can have. You put forth blood, sweat and tears, not to mention an untold strain on your whole body during pregnancy. It really boils down to the genetics of your skin and fascia as to how well you can bounce back after the weight gain and abdominal stretching from your baby. Many women lament that despite dieting, exercising and lots and lots of willpower your body is never the same as it was prior to child bearing. Fortunately, for the masses of dissatisfied women, there are options in Plastic Surgery to boost your self-confidence and your body image. Here is a list of body corrections (Mommy Makeover) that can be performed in tandem or separately to improve the shape and how you feel about your physique. Breast During pregnancy, the glandular component of the breast must enlarge to ready for the enhanced production of milk. Additionally, weight gain with fat storage all governed by the hormonal changes can increase the breast size immensely. Unfortunately, fetal regulation also makes skin “stretchy” to accommodate the increased girth of the breast and abdomen. After the resumption of normal non-pregnant hormones and the lack of need for lactation, the breast volume will revert back to baseline and can even reduce in overall size compared to the original baseline. Unfortunately, many times the skin does not have the ability to shrink to the original size and will stay expanded resulting in a larger skin brazier with same or even less fill than before resulting in “saggy” breast shape. With increased skin length from the breast fold to the nipple it moves the breast mound lower on the chest with less upper pole fullness. I hear many women call their breasts “sad” in appearance when they come in for consultation. Okay, enough depressing thoughts… what can be done? If the skin is not excessively enlarged, the nipple-areolar complex is correct and the fold to nipple distance is not greatly elongated, the solution can be to place a silicone gel implant to act as a shaper and a filler of the breast envelope restoring a pleasing breast shape. If the breast has a low positioned nipple-areolar complex, with or without increase skin length from the fold to the nipple-areolar complex, then a mastopexy (breast lift) is required. Often times tissue volume might need to be enhanced to provide better shape and upper pole fullness, thus an implant or fat grafting can be incorporated at the same time. Sometimes upper pole fullness can be accomplished by transposing extra breast tissue in the lower pole and positioning it on the chest wall deep to the rest of the breast to enhance fullness. Abdomen Hormones during pregnancy direct adaptive body changes that allow healthy growth of the baby, but unfortunately create lasting changes to a women’s body. The abdominal skin stretches and often can result in stretch marks (tears in the dermis with intact epidermis). Excess skin rarely retracts in these cases, leading to hanging skin that drapes over the pelvis. Next, the abdominal wall is distorted to accommodate the growing fetus resulting in separation of the paired rectus muscles leading to increased abdominal circumference with permanent bulges in the abdominal wall both above and below the umbilicus. This unfortunately cannot be reduced with abdominal exercise because the stretched fascia between the muscles is not contractile and will not respond to muscle strengthening. Additional weight changes with aging often can be manifested with increased abdominal girth from excess fatty tissue. So what can a Mommy Makeover achieve in the abdomen? If skin quality is excellent and the abdominal wall is only minimally distorted often liposuction with the addition of additional energy sources such as ultrasonic energy (VASER), oscillating power hand-piece (PAL), or hydraulic power can result in removal of excess fat and create skin retraction/redistribution to improve contour. However, many women will need a full abdominoplasty with correction of excess fat with liposuction, removal of excess lower abdominal skin with tissue resection, and abdominal wall restoration by returning the rectus muscles together with a permanent suturing technique. This can result in excellent contour of the abdomen but you will trade the contour for a scar. I find most people are surprised how long the scar has to be to create the contouring necessary. Once acquainted with the results, most women will gladly trade a scar for the results of a flat smooth abdomen. Back and Flanks Some women can realize their goals by correcting the anterior abdomen by itself. However, many women if given the opportunity would love to three dimensionally correct their torso by sculpting their midback (bra line folds) and flanks (muffin tops) at the same time as their abdomens to circumferentially adjust their shape. This can make a tremendous change on how cloth

Abdominoplasty or “tummy tuck,” is a procedure designed to contour the abdomen. The procedure involves removal of excess abdominal skin and fat as well as tightening of the muscles of the abdominal wall. This procedure is especially helpful for any condition causing laxity of the abdominal wall ie. weight loss and/ or pregnancy. If performed appropriately by a Board Certified Plastic Surgeon this procedure usually involves minimal scarring that can be hidden beneath the bikini line. Procedure Details: Abdominoplasty Procedure: Flatten abdomen by removing excess fat and skin and tightening muscles of abdominal wall. Length: 2 to 5 hours. Anesthesia: General, or local with sedation. In/Outpatient: Either depending on individual circumstances and extent of surgery. Side Effects: Temporary pain. Swelling, soreness, numbness of abdominal skin, bruising, tiredness for several weeks or months. Tumescent: Temporary fluid drainage from incision sites. Recovery: Back to work: 2 to 4 weeks. More strenuous activity: 4 to 6 weeks or more. Fading and flattening of scars: 3 months to 2 years. Duration of Results: Permanent, with sensible diet and exercise.

Liposuction or lipoplasty (or Lipo) is a procedure for removal of localized fat deposits from specific areas of the body. It is well suited for women and men who are of a relatively normal weight but are having difficulty losing localized fat deposits that will not disappear despite weight loss or exercise. Liposuction can be performed in a patient of any age as long as their skin has retained elasticity. The elasticity of the skin is what allows it to shrink and create a smooth contour. Any redundant or excess skin may have to be removed. Most liposuction procedures consist of softening the fat using fluid infiltration and removal of the fat with suction. Other options for softening the fat prior to suctioning include ultrasound and laser. Final results are usually achieved and visible after a couple of months. Procedure Details: Liposuction Procedure: Improve body shape by removing exercise-resistant fat deposits with a tube and vacuum device. Can be performed using the tumescent technique, in which targeted fat cells are infused with saline containing solution with a local anesthetic before liposuction to reduce post-operative bruising and swelling. Common locations for liposuction include chin, neck, upper arms, abdomen, buttocks, hips, thighs, knees, calves, and ankles. For larger volumes of fat or for fibrous body areas, power-assisted lipoplasty may be used. Length: 1 to 2 hours or more. Anesthesia: Local, epidural, or general. In/Outpatient: Usually outpatient. Extensive procedures may require short inpatient stay. Side Effects: Temporary bruising, swelling, numbness, soreness, burning sensation. Tumescent: Temporary fluid drainage from incision sites. Recovery: Back to work: 1 to 2 weeks. More strenuous activity: 2 to 4 weeks. Full recovery from swelling and bruising: 1 to 6 months or more. Use of tumescent technique may decrease post-operative bruising and swelling. Duration of Results: Permanent, with sensible diet and exercise.

Relatively recently the importance of skin laxity and collapse of the outer hip, thigh and buttock regions has been recognized. In such cases, just as removing the excess skin and fat from the front of the body (abdominoplasty) makes sense, so does removal of excess skin and fat from the sides and back of the body. This is the concept of the “body lift.” The Body Lift is an extension of the “abdominoplasty” principle to reverse collapse of the whole flabby mid body, including abdomen, hips, thighs and buttocks. Because skin tightening and lifting occurs to all these areas, an entire body lift can be achieved with effective results and a return to a youthful figure. The operation is quite involved and takes around 6 to 8 hours of surgery. A hospitalization of 5 days is usually required with a recovery period of 3 to 4 weeks. The results achieved, for those who are willing to invest the extra time and effort, are, however, dramatic. Suitability will depend on formal consultation where fitness for surgery will also be assessed and it will be decided if liposuction alone, abdominoplasty with liposuction or the body lift is more appropriate for the individual patient.

Buttock augmentation, BBL, or gluteal augmentation surgery, is sometimes commonly referred to as a Brazilian butt lift. Arm lift surgery is the solution for what is commonly known as the “batwing deformity.” During this operation, a surgeon removes the loose skin and tissue and sews it back up again. The incision is typically a long T- shaped one that extends from just beneath the underarm all the way to the underside of the elbow. Mainly natural aging causes batwings, with gravity being the main culprit when it comes to causing underarm tissues to droop. With aging often comes either a loss of supporting fat tissue or a loss of muscle tone, but in either case each has the same effect of sagging upper arm skin due to weight loss. Dieting worsens the condition and exercise has very little effect when it comes to tightening up loose skin beneath the upper arms. Sagging underarm tissue can also be caused by extreme weight loss (for instance after a major operation) or by your weight gain and loss for years and years. The operation may also be performed on individuals who simply feel that the size or texture of their upper arms does not match the proportions of the rest of their body or who feel that their upper arms are inordinately large compared to the size of their lower arms and wrists.

Thigh lifts are often a very effective procedure to help tighten the loose skin in the thighs and buttocks area often seen after massive weight loss. The procedure does not change the texture of the skin but helps to pull the skin up making it more taught and thereby helping to make the crepiness and wrinkles less noticeable. The procedure is usually done on an outpatient basis and the incisions are often designed in a way that they can be disguised with a bathing suit. The procedure is also often done together with liposuction to help contour the thighs at the same time, but this will be determined by your surgeon at the time of the consult.

Labiaplasty is a vagina surgery, which involves reduction of the labia. The popularity of Vaginal Rejuvenation (vaginal surgery) has skyrocketed over the last few years. Public awareness of these procedures has increased because of the frequent articles in women’s magazines such as Elle and New Woman. Women request labiaplasty surgery for two reasons. Some feel discomfort during sexual intercourse and desire a change.
