Endoscopic Facelift As time goes on, virtually all individuals experience a loosening of the skin on the face and neck. Revision rhinoplasty typically is performed under twilight sedation or general anesthesia. Like primary rhinoplasty, revision can be performed through either a closed (endonasal) approach, during which incisions are made inside the nose, or open (external) rhinoplasty where incisions are made on the strip of skin between the nostrils. The skin of the nose will be elevated from the supporting bone and cartilage. Any missing structures removed during the previous surgery can be replaced or repositioned. External dressings on the nose and sutures are usually removed 1 week post-operatively. These are placed to help control swelling and stabilize the nose. Patients who undergo revision rhinoplasty typically experience less pain, swelling and bruising than with their first procedure. Bruising will fade over the course of about 10 days and swelling will gradually subside too. While swelling will noticeably decrease during the initial weeks after surgery, the last bits of swelling can take several months and up to a year to fully fade.

This low down-time facelift is ideal for those individuals with sagging tissues along the jawline and upper neck. It differs from most of the other ‘mini-lifts’ because it is not simply a skin suspension procedure or a simple SMAS (superficial musculoaponeurotic system) suturing procedure. The UpLift Facelift involves complex neck reconstruction with a muscle reattachment procedure, liposuction if needed, as well as tightening of the support system of the face (the superficial musculoaponeurotic system or SMAS) with a SMAS flap. Incisions are hidden inside the ear, around the earlobe, and under the chin.

A very small cannula is used for the fat transfer. Because the fat is not a foreign substance, it becomes incorporated as living tissue and there is no risk of rejection. There is usually an initial two week period of mild bruising and swelling.

Lip augmentation, or lip enhancement, is a procedure designed to restore fullness to the aging lips or provide augmentation to genetically thin-lipped individuals. Bellafill® is a filler that is injected into the dermis for correction of moderate to severe facial wrinkles and folds. It is the first non-resorbable (permanent) filler approved by the FDA. Bellafill® is polymethylmethacrylate (PMMA) coupled with a purified bovine collagen gel carrier. The PMMA beads form a support matrix for your own collagen, which gradually replaces the bovine collagen carrier gel. Bellafill® is approved to treat the nasolabial folds that form around the mouth. Bellafill® usually lasts for years, unlike many of the non-permanent injectable fillers. All patients benefit from a second treatment 3 to 6 months after the initial injection session. Due to biological variables (each person’s ability to make collagen varies), some patients may require a third touch-up treatment one or two years later.

BOTOX® is the most common non-surgical cosmetic treatment in the United States among both men and women, as more people who are not ready for surgery turn to non-invasive means to look younger. BOTOX® Cosmetic is the trade name of Botulinum Toxin Type A, a purified toxin produced by the bacterium clostridium botulinum. Very low doses are administered via a few tiny injections directly into overactive muscles that cause these lines to form. Botulinum toxin injections gently block the transmission of nerve impulses to the injected sites and prevent the muscles from contracting and forming a crease or a wrinkle. Botulinum toxin injections are now FDA approved to treat Glabellar lines, which are the wrinkles located between the brows, and has been shown to soften and reduce crow’s feet, forehead wrinkles, and the neck line. Frown lines are caused by the bunching of skin from facial muscle contraction. When we frown, the skin between our eyebrows is gathered into a fold which eventually causes a chronic furrow. This gives our face a frustrated, discouraged, angry look which is distracting to others and a bother to ourselves.

In June 2006, the FDA approved the JUVEDERM Injectable Gel family of soft tissue fillers for facial wrinkles and folds. JUVEDERM Injectable Gel is a hyaluronic acid derivative, similar to Restylane, which has two consistencies available. JUVEDERM Injectable Gel helps target wrinkles and folds of various depths, and is also used for lip and tear trough (around the eyes) augmentation. Because lidocaine is now built into the Juvederm Injectable Gel, the discomfort from this procedure is minimal.

Plump, full, contoured lips are considered feminine and beautiful. They can dramatically improve the appearance and balance facial features. As we age, our lips become thinner and longer. Injectable gel is the first and only filler FDA-approved to instantly add volume to the cheek area. It is made of hyaluronic acid derivative, a sugar molecule normally found in human skin. JUVÉDERM VOLUMA XC gives you a subtle lift with minimal down-time, helping to restore contour and a more youthful profile, for up to 2 years, in patients over the age of 21. It’s different than JUVÉDERM XC and works on a different area of the face. JUVÉDERM XC smoothes out moderate to severe wrinkles and folds around the nose and mouth, like parentheses. JUVÉDERM VOLUMA XC, however, adds volume to the cheek area. JUVÉDERM VOLUMA XC is an excellent product for treating cheek hollowing and deflation. It contains a small amount of lidocaine which helps to numb the treatment area during the injection procedure. Patients may require a touch-up treatment after their initial treatment to achieve optimal results.

Kybella is an FDA-approved injectable cosmetic treatment used to eliminate the fat that causes double chin. As the only non-surgical method for addressing this common cosmetic problem, Kybella is quickly growing in popularity alongside Botox and other injectables. This treatment involves injecting a synthetic version of bile salts that the liver naturally makes to break down fats. Kybella actually destroys the fat cells, which the body naturally eliminates on its own following each session. Kybella is administered over a series of sessions that vary in length based on the extent of treatment needed for the individual.

Perlane was approved by the FDA in May of 2007 for soft tissue augmentation of moderate to severe facial folds and wrinkles. Perlane is in the same family as Restylane, with a larger particle size. Because of its larger hyaluronic acid particle size, Perlane is injected into deeper folds than Restylane. It is most commonly used in the nasolabial folds, marionette lines, and for augmentation of the cheeks. Radiesse is a synthetic injectable filler that non-surgically reduces most wrinkles, creases and scars by augmenting your body’s natural collagen supply. Composed of microspheres in a biocompatible gel, Radiesse® is made from the same minerals found in our bones (calcium hydroxyapatite), so it carries almost no risk of allergic reaction – just one of its advantages over alternative treatments. Radiesse can be used on many areas of the body, but the most effective areas treated are the nasolabial folds (laugh lines), peri-oral lines, depressed scars and oral commissures (downturned corners of the mouth), and hands. Treatments typically take 30 minutes and require only local anesthetic. Side effects such as swelling or bruising are minimal and should fade within a few days. Results are visible immediately after the injection and can last about nine months to one year, although the procedure can be repeated as often as you wish.

Approved by the FDA, Restylane® is designed to smooth wrinkles, sculpt lips and shape facial contours. Restylane® is a clear transparent gel composed of hyaluronic acid, a natural substance that is found throughout your body. Restylane® is not of animal or human origin, which means that there is almost no risk of allergic reaction. The most common areas for a Restylane® treatment are the nasolabial folds (from the root of the nose to the angle of your mouth), the lips (to provide fullness and shape), corners of the mouth (to restore their horizontal youthful position instead of a downturned angry position), and glabellar lines (between the eyebrows). With its unique ability to bind with water, Restylane® remains in your skin for about 6-7 months. The Erbium YAG laser allows the physician to gently wash away layers of skin that are much thinner than that of the CO2 (Carbon Dioxide) laser. This ultra thin laser wash lets the Erbium behave like a lunch hour peel for a light skin refinement or with multiple passes to smooth deeper wrinkles. Lasers were first introduced to treat medical disease in the early 1970’s. Since then, technological advances have resulted in the development of cosmetic lasers. Today, cosmetic lasers are used to perform very specialized specific tasks with accurate and consistent results, with minimal risk. With this in mind, cosmetic lasers are currently being used to help wash away wrinkles and sun damage, improve skin texture, remove vascular red skin spots, brown spots, tattoos, and assist in many other procedures.

The Aesthetic Surgery Center is proud to offer SkinTyteTM technology as part of its Sciton Profile BBL platform. This innovative new technology is used to selectively heat the dermal collagen deep in your skin while simultaneously cooling the epidermis (outer layer of skin) prior-to, during, and after treatment, reducing discomfort and minimizing thermal injury to non-target skin and skin structures. The result is partial coagulation and contraction of collagen. This treatment takes about 45 minutes per area, and is done in the office. The most commonly treated areas are the face, neck, and upper chest. There is minimal, if any, facial swelling the first day, but no break in the skin. You can wear make-up immediately. Because of the novel mechanism of action, optimal results from dermal remodeling are seen over a three to six month period. Most patients require an average of 3-5 treatments spaced one month apart. However, because each patient’s skin is different, the treatment number can vary.

The Ultrapulse 5000 CO2 (Carbon Dioxide) Laser has been used at the Aesthetic Surgery Center since August of 1995 and has become an integral part of the contemporary approach to restoring the aging face. Facelifts, forehead lifts and blepharoplasties have little or no effect on fine to medium facial wrinkles. These procedures cannot pull skin tight enough to remove fine to medium facial wrinkles, especially wrinkles around the eyes and mouth. Crepey eyelid skin, vertical lip and frown lines can be smoothed effectively with the use of the Carbon Dioxide laser. An Ultrapulse 5000 CO2 Laser Wash to the entire face produces a tightening effect of the entire facial area that many times mimics the results often seen as a “non-surgical facelift.” Skin damage that has been promoted by smoking, alcohol, and excessive exposure to sunlight responds exceedingly well to laser resurfacing. ThermiSmooth is a procedure performed by our licensed aestheticians at the Aesthetic Surgery Center, utilizing the ThermiRF Radiofrequency skin tightening technology. During a series of 4 weekly sessions, a temperature regulated probe is applied externally to the skin in order to heat the skin to the desired level (42-44°C). Heating of the skin to this temperature induces skin tightening by collagen contraction and remodeling, as well as long term stimulation of collagen production. Results are seen 2 to 12 months after the series of 4 treatments. The most commonly treated areas are the upper and lower eyelids to tighten the skin and improve fine lines, the mouth area to improve lip lines, and crepy skin along the neck. ThermiTight is a procedure performed by our physicians at the Aesthetic Surgery Center, utilizing the ThermiRF Radiofrequency skin tightening technology. This is a new technology whereby a fine temperature regulated probe is inserted under the skin to deliver heat directly to the dermis and subdermis. Heating of these layers has been shown to induce skin tightening by collagen contraction and remodeling. Results are seen 2 to 12 months after a single treatment. The ideal candidate has loose skin, with limited excess fat, although it is possible to combine this procedure with liposuction if desired. The most commonly treated areas are the jawline and neck, upper arms, abdominal wall, thighs, and knees.
