Cosmetic Surgery

Every year, 100,000 men and women choose cosmetic eyelid surgery to improve the way they look. Why? The eyes are the first place people look at us and also the first place to show aging. People considering eyelid enhancement want to improve: Droopy, puffy eyelids Sagging skin that weighs on the eyelashes Bags under the eyes A tired appearance These problems can make you look older, and droopy eyelids can sometimes impair vision. Eyelid surgery improves them, so that you look less worn and tired. The surgery, called blepharoplasty, can be performed on the upper eyelids or the lower eyelids or both together. The general medical term for eyelid surgery is blepharoplasty. It contours the fat and muscle, and removes excess skin from the upper and lower eyelids. Other options include ptosis repair, and repair of malpositioned eyelids from previous surgery, trauma, or aging.

Where face lifts tighten and lift the lower two thirds of the face, brow lifts (also called forehead lifts) do the same for the upper face, making you look younger and more rested. Problems addressed by browlifts are: Droopy or sagging eyebrows “Hooding” at the outer part of the eyes Wrinkles and creases, such as worry lines “Creping” appearance of the skin No one wants to have a “surprised” expression and we are very careful to avoid excessive elevation of the eyebrows.

A facelift tightens the tissues of the lower two thirds of the face. It’s effective for jowls, blurring of the jawline, sagging cheeks, and wrinkles that run from the sides of the nose to the corners of the mouth. In general a facelift is performed to fix that droopy, sad look that belies our natural good spirits. It is often, but not always, combined with a neck lift since the neck often sags and wrinkles. A facelift may be all you need. But for many people, facelifts are performed in conjunction with other procedures, such as blepharoplasty (eyelid surgery), laser resurfacing, fat grafting, or Botox® injections. Dr. Fante will listen to your wishes and advise you as to how you can best arrive at the look you want. A facelift can take about 10 years off your apparent age.

Also known as a cheek lift, this procedure lifts sagging cheeks, smoothing out those furrows that run from the nose to the corners of the mouth, and lifting up the corners of the mouth. It helps improve early jowling, and also helps reverse and camouflage aging changes below the eyes. For many of us, aging causes the shape of the face to become more oval. The mid-facelift restores a more triangular shape with the youthful appearance of higher cheekbones. Since the improvement comes without visible incisions, this minimally invasive mid face lift is very popular among both men and women. It can be done by itself, or in combination with other procedures, such as a eyelid surgery (blepharoplasty), brow lift, a full facelift, or laser skin resurfacing. The endoscopic mid face lift is popular worldwide and Dr. Fante is among the innovators who created and refined the procedure. He has published peer-reviewed articles and lectured on the subject to his fellow physicians in the USA, South America, and Europe.

Neck Lifts tighten sagging skin on the neck and improves muscle integrity and banding. This lifting of the neck creates a more youthful look by improving the jawline and removing a “turkey neck”. Neck lifts are often sought by people who are aware of disproportionate aging of the neck, or who have had facelifts previously, and are noticing some recurrence of sagging neck skin. A neck lift doesn’t have any effect on the cheeks, although it will tighten your jawline. The incisions are smaller than those for a full face lift It’s a good procedure to correct the effects of gravity, sun exposure, and aging at the neck when they are more noticeable or more bothersome than the aging of the face Most often, it’s done for people between the ages of 40 and 60, but can be done very successfully for people much older Neck lifts can be performed alone, but are often performed at the same time as eyelid surgery, since so many people notice aging changes in both places at once. It can also be done in conjunction with other procedures such as: Liposuction underneath the chin, where a pad of fat tends to grow as we age Chin implants, which give the lower face more bony definition and often improve the profile of the neck as well

Often, otoplasty (ear surgery) is done for children aged about 4 to 14. It’s done to prevent teasing or humiliation when the ears protrude too much or “look funny”, and since our ears are fully grown by age 4 or 5, it’s best done as early as possible. Just as often, those kids don’t have it done and grow into adults who can benefit just as much from ear surgery, so that many men and women decide to have their ears improved as adults. Adults don’t incur any extra risk, and for many, it is actually easier since they can have surgery under local anesthesia. Otoplasty can remedy many conditions of the external ear including: Protruding ears that stick out too much Cupped ear – a very small ear Lop ear – the tip bends forward and downwards Shell ear – the outer curve and rim is missing Over-large or stretched earlobes Highly creased or wrinkled ear lobes Spock’s ear – a pointed top and an extra fold Ears too large or too small Excessively large “gauge” ear piercing

Laser resurfacing treatments remove damaged skin and rejuvenate a patient’s appearance. State-of-the-art laser treatments can provide remarkable results even on the most environmentally damaged and discolored skin. In more mild cases, light chemical peels can tighten wrinkles, lighten age spots, and soften skin texture around the eyes. In addition, laser treatments or chemical peels are often combined with Botox and fillers to optimize your appearance without surgery.

With the advent of injectable fillers for facial volume augmentation, the popularity of permanent facial implants has diminished. However, since all of the FDA approved injectable fillers are temporary (typically 6-24 months for hyaluronic acid gels, for example), sometimes a permanent implant makes more sense. In addition to the lack of ongoing maintenance and potential for fluctuation in appearance, permanent implants offer another benefit: they can provide support for aging, sagging, or damaged soft tissues in a way that gels just cannot. As a consequence, implants continue to have a place in both cosmetic and reconstructive oculofacial plastic surgery. Dr. Fante has given courses on permanent facial implants to his colleagues at national scientific meetings, and has experience with many types of implants.

There has been a small revolution in our thinking about the aging face over the past twenty years. For decades, doctors had focused on the roles that gravity and sun damage play in making people look old, but had largely ignored the loss of youthful fullness that also occurs. This was probably because medicine could offer no good solutions to that problem, even though it was obvious that young people have full, softly contoured faces and older people don’t. Two big advances occurred in the 1990’s: 1. hyaluronic acid fillers emerged as a practical short-term method to restore facial volume, and 2. Dr. Sidney Coleman in New York City pioneered the surgical transfer of body fat to the face as a long-term method to restore volume. These techniques served as an epiphany for doctors interested in facial rejuvenation, and facial plastic surgeons around the world have gradually developed increasingly comprehensive strategies to help people look as young as they feel.

Oftentimes our appearance isn’t an accurate representation of how we feel on the inside. As we age, our youthful face starts obeying the law of gravity, as the years of downward pulling start to show. The force of gravity is assisted by the sun, as our skin slowly becomes drier, loses collagen, and forms wrinkles. The sun also damages the integrity of the skin, contributing to discoloration, texture changes, and in some cases even skin cancer. Beneath the skin our facial fat, which is a key component of a youthful appearance, is lost or redistributed, often creating relative emptiness where there once was fullness, and vice versa. Facial Rejuvenation With Dr. Robert Fante Dr. Fante offers us ways of rejuvenating the face to provide a rested, relaxed, and more youthful appearance that is all you. There are many procedures to choose from, and in your initial consultation, you can learn about possible combinations that range from simple injections to minimally invasive cosmetic surgery. Cosmetic Surgery Procedures The cosmetic procedures offered at our practice include: Eyelid Surgery Blepharoplasty contours the fat and muscle, removing excess skin from the upper and lower eyelids. Dark circles under the eyes can be due to under-eye bags, which may also be improved through this procedure. In addition, some people have eyelid surgery to correct inherited characteristics. Laser Resurfacing Laser resurfacing treatments remove damaged skin and rejuvenate a patient’s appearance. We use a variety of lasers and other light-based therapies to address skin texture and color changes. Botox, Fillers and Skin Care We offer a varitey of non-surgical treatments for skin care and rejuvenation including Botox, Fillers, Microneedling, Kybella and more. These treatments can reduce the appearance of fine lines and wrinkles, as well as other skin imperfections. Facial Implants If you are interested in permanent facial contouring, which cannot be achieved with temporary filler, Dr. Fante offers a number of different facial implants to meet your needs. Depending on your desired outcome, you may be a candidate for chin implants, cheek implants, temple implants or lip implants. Fat Transfer Facial fat transfer (grafting) is a surgical procedure that involves harvesting fat from an area in the body. These areas include the belly, hips, neck, or thighs. Dr. Fante uses specialized and gentle liposuction under local anesthesia or twilight sedation. Bodytite BodyTite is a great solution for individuals who are looking to reduce fat without the saggy, wrinkly skin. It also helps improve skin laxity after weight loss or due to aging. BodyTite is a minimally invasive procedure that shapes, contracts and lifts without the scalpel or large scarring. You are left with surgical-like results without prolonged downtime. Facetite FaceTite is a new procedure that is comparable to a surgical facelift, without the large scars. Reconstructive Surgery

Ectro & Entropion Surgery

Ectropion denotes an eyelid that hangs away from the eye, while entropion describes an eyelid that is turned toward the eye so that the lashes constantly rub the surface. Eyelid ectropion and entropion can occur from aging, trauma, Bell’s palsy, previous surgery, and other diseases. Both conditions can be permanently repaired with outpatient surgery typically lasting less than one hour. Dr. Fante has performed thousands of these procedures and has co-authored the training manual for student surgeons.

Eyelid or facial surgery or trauma can cause the eyelids to lose normal mobility and position. In some cases, this scarring may make it impossible to properly close the eye(s) so that drying, irritation, and infection can result. When the lower eyelid is affected, there is “too much white showing” and the eye often alternates between feeling gritty and then watering excessively. Outpatient surgical repair can usually restore more normal appearance and function. Careful examination during your consultation with Dr. Fante will determine the exact details of the repair that will be required.

A wide variety of benign and malignant tumors, cysts, and growths can occur in the eye socket behind or near the eye. Patient symptoms vary from none (family or a physician notices asymmetry between the eyes) to double vision, loss of vision, pain, redness, and severe swelling. MRI, CT scans, and other tests can often assist in diagnosis. Depending on the situation, treatment might include medications, injections, and surgery. Management of these conditions is highly specialized and sometimes requires a multidisciplinary team approach. Dr. Fante works with oncologists, ear, nose, & throat surgeons, neurosurgeons, and craniofacial surgeons when necessary to obtain the best possible outcome.

Trauma to the face can often result in broken bones around the eye socket, also known as the orbit. While the cheekbone and forehead are both quite strong, many of the bones around the eye are delicate and fragile. Some anatomists believe that the delicate orbital bones act like the “crumple zone” built into automobiles to protect the passengers: when the orbital bones break and absorb the impact, the eyeball itself is safeguarded. This type of orbital fracture is called a “blow-out” fracture. In any case, many kinds of trauma can cause orbital fractures, including being hit in the upper face with a baseball, tennis ball, steering wheel, or fist, or falling and striking the face on furniture, stairs, trampolines, or many other objects.

Car accidents, dog bites, fights, sports accidents, and other injuries unfortunately often damage the eyelids and tear drainage system. Repair can be simple or complex, depending on the injury. Evaluation at an emergency room is usually necessary. Outpatient surgery typically results in excellent restoration of function and appearance.

Infections near the eye can occur just under the skin, or sometimes deeper in the tissues around the eye (or orbit). For children and adults, orbital infection is usually associated with sinus infections. When the infection occurs deep in the eye socket area, it can be dangerous, causing blindness and even death. Certain conditions including diabetes increase the risk of serious infections, and blunt facial trauma will occasionally trigger severe infection. Prompt treatment with intravenous antibiotics is often necessary, sometimes combined with surgical drainage of abscess.

Autoimmune thyroid related disease can cause a wide range of symptoms and problems for the eyes, including bulging, redness, tearing, irritation, double vision, pain, pressure, and rarely, loss of vision. The eye problems can start before, after, or during the thyroid problem, and typically progress slowly over about 18-30 months. For a minority of people, the problems are minor and completely resolve, but for most people, the discomfort and change in appearance are long-lasting.

Ptosis is the medical term for a drooping upper eyelid. This problem affects approximately 5 percent of the population and can affect vision and appearance. The prevalence of this condition increases in the elderly. Surgery can be performed to repair drooping eyelids and restore them to their natural shape and contour. Dr. Fante has performed thousands of ptosis repair operations. The procedure is performed usually under local anesthesia as an outpatient. The surgery is painless, takes less than an hour and patients can usually resume their normal activities in about a week.

Blepharospasm is a disease in which there is repeated and uncontrollable muscle contraction around the eyes and eyebrows. Blepharospasm does not occur during sleep and is often exacerbated by stress, dry eyes, and bright lights. Blepharospasm can be disabling and was the original reason that Botulinum toxin (Botox) was developed in the 1980’s. Blepharospasm can be treated with Botox. Botox is a medicine that can reduce involuntary muscle spasms around the eyelids and eyebrows. Botox is administered in the office in small amounts through tiny injections. Typically, no anesthetic is necessary, and for this purpose, Botox treatment is usually covered by health insurance.

Eyelid tumors and lesions are very common. Most of these tumors and lesions are benign and involve the sweat glands, oil glands, eyelash follicles, and skin elements. They have unusual names, such as hidrocystoma or seborrheic keratosis, but are not dangerous at all. But some eyelid tumors are malignant, the most common being a skin cancer known as basal cell carcinoma. Dr. Fante and Dr. Hawes are the authors of the definitive chapter on eyelid reconstruction in the #1 surgical textbook on the subject: the authoritative Local Flaps in Facial Reconstruction, considered among the essentials in plastic surgery education. Traumatic accidents and treatment of ocular and orbital cancers, blind and painful eyes, and other diseases sometimes lead to the need for reconstruction of the orbit (eye socket) and removal of an eye with subsequent placement of an orbital implant. With advances in orbital implant materials and surgical techniques, the cosmetic outcome is frequently excellent after surgery, and pain is almost always permanently relieved. Along the Front Range, Dr. Fante works closely with several excellent Ocularists who create custom eye prostheses for patients who have lost an eye.

Excessive tearing is often due to a blockage of the nasolacrimal duct (the main drainage duct from the eye to the nose). When the system is infected, it is called dacryocystitis. The most common symptoms of a blocked nasolacrimal duct are frequent tearing and a mucous discharge in the eye. At times, the eyelids will be stuck together in the morning. A tender area is sometimes present in the area between the eye and the nose.

Reconstructive Services Oculofacial plastic surgery is a specialty devoted to restoring and enhancing the function and beauty of the eyes and face. Ectropion and Entropion Ectropion denotes an eyelid that hangs away from the eye, while entropion describes an eyelid that is turned toward the eye so that the lashes constantly rub the surface. Eyelid Scarring and Retraction Damage to the eyelids can prevent them from working properly to protect and lubricate the eye. When scars or injuries pull the lower eyelid downward (or the upper eyelid upward), it is called eyelid retraction and interferes with blinking, which can cause eye irritation, pain, blurry vision, tearing, and permanent damage to the cornea. Orbital Tumors Orbital tumors can be primary, meaning they originate in the orbit. They can also be secondary, meaning that they invade the orbit from the adjacent paranasal sinuses or face. Or they can metastasize from cancer occurring elsewhere in the body, particularly the breast. Orbital Fractures Trauma to the face can often result in broken bones around the eye socket, also known as the orbit. While the cheekbone and forehead are both quite strong, many of the bones around the eye are delicate and fragile. Eyelid Trauma Eyelid trauma may occur in many different ways. Common sources of trauma to the eyelid and surrounding structures include dog bites, auto accidents, sports injuries, falls, and physical altercations. Orbital Cellulitis Infections near the eye can occur just under the skin, or sometimes deeper in the tissues around the eye (or orbit). For children and adults, orbital infection is usually associated with sinus infections. When the infection occurs deep in the eye socket area, it can be dangerous, causing blindness and even death. Facial Gender Affirming Surgery Facial feminization and facial masculinization surgery (FFS and FMS, respectively), are types of gender affirmation procedures. BOTOX® is a trade name for a refined version of the botulinum toxin type A, originally isolated and refined by researchers seeking to help people with eyelid and facial spasms. It has been safely used for this purpose since the 1980’s. Dr. Fante is the author of scientific articles on the medical uses of botulinum toxin type A and has vast experience with this medication. Xeomin® is an injectable composed of botulinum toxin type A, a naturally occurring protein that is produced by the bacteria Clostridium botulinum. When Xeomin® is injected into the muscles it reduces their activity and the result is a smoother and less noticeable wrinkle. Xeomin® does not stop the muscle from moving entirely; instead, it modifies the contractions. This is achieved by inhibiting a body chemical called acetylcholine from signaling all of the nerves within the muscle to contract. Dysport® is an injectable that is FDA approved. It temporarily lessens or eliminates moderate-to-severe frown lines between the eyebrows. Dysport® is made from a neurotoxin produced by the bacterium Clostridium botulinum; it is the same neurotoxin used in BOTOX® Cosmetic.

An alternative injectable treatment to facial fillers is Sculptra. Sculptra is not a filler but a chemical that encourages your body to form new natural collagen to enhance your appearance.
