    Just go somewhere else. I have had more trouble with this doctor's office then any other doctor I've seen in the last 50 years. They say they're going to do something and they just don't do it and it takes anywhere from 5 to 10 phone calls to get any action or even a call back. It looks like other people had a completely different experience and I can respect that but trust me I am not making this up.

Please bring your insurance card(s), valid photo identification, and list of current medications to every office visit. If you are a new patient to the clinic, the following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time you will save significant time at the time of your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.
