WARNING: Incompetent and Reckless Care for Cardiac/Endocrine Patients
I was recently seen at Guthrie Big Flats Pain Clinic by a provider who is a dual-licensed Physician Assistant Jason Paulman (License #024096) and Chiropractor (License #012544). This provider was shockingly dismissive and professionally arrogant. Despite a clear MRI report documenting **L3-S1 spinal stenosis, multiple bulging discs, and bone spurs**, he chose to label my sharp, shooting pain as 'mild.' When I pointed out the specific findings on the report, he incredibly stated he 'did not have to agree' with the Radiologist’s findings.
He then attempted to push a steroid injection—a treatment he admitted causes 'palpitations'—despite my documented Inappropriate Sinus Tachycardia (IST) and current thyroid toxicity. For a patient with my heart condition, 'a couple of palpitations' can mean a 3-day cardiac crisis. His disregard for patient safety in favor of a 'take it or leave it' injection was appalling. He even suggested chiropractic manipulation for a spine with confirmed stenosis, which many experts consider high-risk. When I confronted him about his lack of help over several visits, he actually tried to take credit for 'getting me the MRI.' In reality, my insurance **denied the claim** because he Failed to submit the necessary clinical paperwork to authorize it. He is taking credit for a test he essentially blocked me from getting covered. Now I have to deal with a $6,000 bill.
Furthermore, he attempted to diagnose me with **SI Joint Dysfunction**, completely ignoring the fact that my pain radiates from the **L3-L4 region** (as well)—well above the SI joint. This felt like a 'distraction diagnosis' intended to steer me toward chiropractic adjustments rather than addressing the documented **spinal stenosis and bone spurs** higher up in my spine. He actually made the comment that a chiropractor would help me more than an injection. He was trying to treat the 'wrong' part of my back just to fit his chiropractic narrative, while ignoring the high-voltage nerve pain caused by my **1.7 T4 thyroid toxicity** and **IST**.
To cap off the experience, this provider—a PA who also holds a chiropractic license—referred me to a 'walk-in' chiropractor at the local mall that accepts $30 cash. Suggesting a cash-walk-in 'adjustment' for a patient with documented spinal stenosis and a complex heart condition (IST) is the height of medical negligence. It felt more like a shady referral to a friend than a legitimate medical recommendation. Guthrie should be embarrassed to have a provider steering patients away from evidence-based care and into high-risk, 'mall' chiropractic shops.
When I advocated for my own health and questioned his ability to interpret imaging, he became defensive and very aggressive and told me to 'go see someone else.' I will be doing exactly that. If you have complex cardiac or endocrine needs, or if you want a provider who respects medical imaging, avoid this clinic at all costs!
