I can’t say enough amazing things about Nicole Poirier and the staff at MVPT-Salem. From the very first appointment, I felt heard, understood, and genuinely cared for. I’m very happy with the care I’ve received. This is my second series of PT treatments (two different diagnoses) and the team there is amazing. Nicole listens and is exceptionally knowledgeable. I’ve been to other PT practices and MVPT is by far my favorite. I highly recommend them.

What should I bring to my first appointment?
Please have your insurance card available when you call. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.
To bring on your first visit:
- Your photo ID and insurance card
- Print out the appropriate patient forms, fill them out, and bring them with you. You can find patient forms here (insert link for "here" - https://www.mcrchiro.com/online-forms)
- In case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.
