  To whom it may concern, My name is Ukyia Jones, and I am the legal guardian for my father, Harold N. Jones, who currently receives care at your Eno River location in Durham. I am writing to formally address a serious issue regarding a proposed transfer and a recent interaction involving your social worker. Yesterday, February 11, I was contacted by staff at Pettigrew Rehabilitation Center, where my father resides. I was informed that your social worker is attempting to transfer my father to a different location with a new chair time starting this coming Monday. My father has suffered multiple traumatic brain injuries; he is incompetent and unable to communicate effectively. As his legal guardian, I am responsible for all decisions regarding his care. It is highly inappropriate that the social worker bypassed me to seek "consent" from my father, given his medical status. Furthermore, Tina from Pettigrew, who assists my father with his treatments, reported that when she questioned this process, the social worker made unprofessional remarks regarding my father’s behavior and my responsiveness to phone calls. A change in location or schedule would require the same chair time to ensure my father's compliance, as well as coordinated transportation and staffing. This short notice and the lack of communication with me as his guardian are unacceptable. I work during business hours but can be reached . I look forward to your immediate attention to this matter.
