Consumer Directed Services is a State Plan Personal Care program, which includes but is not limited to personal care, housekeeping, and transportation. Independent Living Waiver (ILW) Services beyond the limitations of the CDS State Plan Personal Care Program is available to a limited number of eligible consumers. ILW Services include: Case Management, Personal Care, Environmental Accessibility Adaptions and Specialized Medical equipment and supplies. We are currently accepting Medicaid and Private Pay
