We are a primary care office committed to the health and well-being of our community in Evergreen Park, IL. Park Primary Care was founded by Dr. Thomas Joseph in 2003 on the on the belief that a primary care physician should be a “go to” resource for all of a person’s health care needs. We believe your relationship with your physician is designed to be long-term and ongoing in order to provide continuity care and allow you to make the most informed decisions about your health care needs. At Park Primary Care we place a strong emphasis on preventative care, health education and appropriate disease management.
