Primecare Young Family Health Center

Your medical home is where you and your family can go for regular checkups or when you’re sick. We provide medical, behavioral health, and oral health care for people of all ages, regardless of ability to pay. Your medical home is also the place to come for referrals, get help from a case manager, or sign up for insurance. For your convenience, we offer appointments during the day, evenings, and on the weekend. You can also communicate with your provider via the patient portal for easy access. At your medical home, you have a care team to ensure all your needs are met to the highest quality.
