I have been able to utilize their services for many patients that needed more assistance when transitioning back out of rehab. The team at home instead make it easy and seem less to transition patients and that ultimately is what makes the patient successful in staying in their home. I could never be more happy when a patient agrees to use their services as I know the discharge plan is going to be wonderful.
