Physical Therapy
21st Century Rehab is a physical therapist owned company that was founded in 1995 by Steve and Michelle Cassabaum. Since that time, 21st Century Rehab has added four partners, all physical therapists who work in our clincs. 21st Century Rehab has seven outpatient clinics locations in Altoona, Boone, Carlisle, Indianola, Madrid and Osceola and have seven hospital partnerships with Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Greene County Medical Center (Jefferson), Knoxville Hospitals and Clinics, Story County Medical Center (Nevada), Van Diest Hospital (Webster City) and Wayne County Hospital (Corydon).
21st Century Rehab provides quality physical therapy, occupational therapy, speech therapy, sports medicine, and occupational health services to patients and communities. These services range from joint and muscle pain, trouble with daily activities, range of motion issues, headaches, vertigo, TMJ issues, Women's and Men's health, and more.

- LHLisa HerrstromFeb 12, 20265.0I went to see Cole and Josh for my knee and back pain. They really listened closely to me and tried to figure out what could be causing my pain. The exercises were not too hard and encouraged me to stop if anything hurt. They also gave me many exercises that I can do while at work, which was exactly what I was looking for. Renee at the desk is always outgoing and has a smile for you at every visit. I would highly recommend 21st Century Rehab in Madrid!
- SCSara CarlsonFeb 8, 20265.0I was told by my dentist that I need to go to an TMJ specialist and wear a mouth brace every night for the rest of my life. Not what I wanted to hear. I reached out to Cole about my TMJ, not knowing if he would be able to help. After meeting with him and talking through my history, causes, treatment etc., Cole spent time massaging, dry needling and going through exercises that I could do to improve my TMJ. Within 24 hrs my pain had significantly decreased and I had better ROM. I highly recommend Cole!!
