Physical Therapy
21st Century Rehab is a physical therapist owned company that was founded in 1995 by Steve and Michelle Cassabaum. Since that time, 21st Century Rehab has added four partners, all physical therapists who work in our clincs. 21st Century Rehab has seven outpatient clinics locations in Altoona, Boone, Carlisle, Indianola, Madrid and Osceola and have seven hospital partnerships with Dallas County Hospital (Perry), Franklin General Hospital (Hampton), Greene County Medical Center (Jefferson), Knoxville Hospitals and Clinics, Story County Medical Center (Nevada), Van Diest Hospital (Webster City) and Wayne County Hospital (Corydon).
21st Century Rehab provides quality physical therapy, occupational therapy, speech therapy, sports medicine, and occupational health services to patients and communities. These services range from joint and muscle pain, trouble with daily activities, range of motion issues, headaches, vertigo, TMJ issues, Women's and Men's health, and more.

She took the time to explain everything and worked through the issue step by step, always making me feel confident in the process. Because of her care, I’m back to doing normal daily activities like household chores, lifting groceries, picking up my daughter, and even weightlifting again. I truly feel like I got my life back. I’m extremely grateful for her dedication and would recommend her to anyone in need of physical therapy.
