Hospice care is for when there is a prognosis of six months or less. This type of care is provided by an interdisciplinary team of doctors, nurses, chaplains, social workers and other specialists, such as massage and music therapists. Our team can make this difficult time and its complications easier for everyone. When under our care, you can call us any time, day or night, to get your questions answered.
Our hospice care is available in your own home, nursing home, or assisted living facility and is provided within Bryan, Chatham, Effingham, and Liberty counties.
Our unique in-patient facility, is for patients that have hospice medical issues that cannot be resolved at home, and offers respite care for weary caregivers.
