Affinis Hospice (Pooler)

Affinis Hospice provides a comprehensive approach to the care and well being of patients facing a terminal illness. Individualized care provided by a collaborative team of medical personnel, chaplains and local volunteers, is designed to meet the varied needs of both the patient and their family members. We are centrally located in the Savannah Market serving 8 of the surrounding counties as well as working closely with St. Joseph Candler, St. Joseph’s and Memorial Hospitals in the region to care for patients at the next part of their health care journey. We serve patients in the community in their home environment as well as having multiple contracted facilities in the area to provide Hospice Care in Skilled Nursing Facilities.
